Total Quality Plan Implementation
What happened to Sears is not that uncommon. History has been witness to the rise and fall of many great empires. Most businesses go bankrupt on the way up, not on the way down. Sears failed to remain the top retailer for two reasons (1) a lack of focus on operating strategies and, (2) a lack of long-term vision from top management. The retail industry is very competitive and requires a flexible management team able to adapt and adjust to changes in competition and changes in consumer tastes.

The palm pilot will decrease the amount of information a salesperson will need to remember because it will all be at the click of a button. As a result, sales staff can be easily placed into different departments as the demands shift between product categories due to seasonal trends, advertised sales, or employee emergencies.
The second step is the longest and most detailed. The creation of a web-based supply chain will be created and carried on a mainframe computer located at the head office. Sears will have access to customer information, sales trends, supplier conditions, employee records, single-store demographics, and product information. The data will be shared between the warehouses, the stores, the suppliers, and the customers.
The logistics and IT departments will head this stage of the process. The objective will be to speed the flow of information to the various users while reducing costs, especially in inventory. The system will be online, making it accessible on a "real time" basis and will connect the mainframe to an Internet service provider where anyone anywhere in the world can log on. Customers will benefit from the new system the most. They will be able to check on product information such as prices, sizes, colors, availability, and even upcoming sales.

The nurse is challenged with the care of patients over a lifespan. Each stage of life brings its own physical and emotional changes which directs the care needs. The care needs of the pediatric patient will be much different from the needs of the geriatric population. The geriatric population has very specific needs which has prompted the government to establish the Quality Assurance & Performance Improvement (QAPI) program. The QAPI provides the framework for nursing facilities to develop and implement changes which address deficiencies the facility was found to have. Also, the QAPI program requires practices and policy be put in place to monitor care of the residents. The purpose of this paper is to list some of the changes the elderly go through as they age, and demonstrate these changes in a quality improvement project. After review of literature, I will discuss the challenges, barriers, and solutions as related to quality improvement. Lastly, I will discuss the quality of care for the geriatric in the future.
Falls are a major cause of injury and death in the frail older population with Alzheimer’s disease. Residents in the long-term care settings experience falls for many reasons and are likely to endure injuries more than those in other settings. Preventing falls in this setting posed significant challenge, many have cognitive problems, aged related changes, chronic medical conditions, medication effects, and physical limitations requiring dedicated interdisciplinary efforts (Vance, 2011).
In order to understand the actual implementation of the Plan-Do-Study-Act (PDSA) model for continuous quality improvement, I conducted an interview with my sister’s husband who is a physician that owns a practice in Germany. The interview took place on October 9, 2015 at 7 pm their time, which is 1 pm in my Eastern Time zone over the internet on the visual phone called Skype. I chose to interview Dr. Henkel because owning a practice in Germany requires more attention to customer satisfaction with the quality of care, since the patient has government health insurance and can be more discerning on where to go for health services. Therefore, this led me to conclude that he would have had to use the PDSA cycle at some point to keep his practice running. Nevertheless, I was not disappointed with our conversation on how he still uses the PDSA model to work continually on improving customer satisfaction with the health services received at his practice.
As a hospital, quality care should be a priority for patients that are going to be treated for a sickness, or any type of procedure that is going to take place. A lot of times a patient gets an infection while they were at the hospital, on top of being treated for what they original came in for. Health facilities should be environments of healing, which they are, but they also have tons of various types of germs and infections, which grasp onto individuals that have weak immune systems/are sick. Some infections that are at hospitals are Tuberculosis, VRE, VAP, C-Diff, UTI, and MRSA. After the interview with my nurse manager, I came up with the PICO question which states: “Does the computerized physician order entry (CPOE) system reduce the number of medication errors compared to the common paper system being used today?” This question is important and I selected it because the population that the Belvoir Community hospital serves includes army officers of all ages both active and retired including their spouses and children. This group includes two sub groups of highly vulnerable persons which include the very young and the very old, who have a high-risk effect for medication errors because the potential adverse drug event is three times greater than an adult hospitalized patient (Levine et al., 2001). CPOE is not a panacea, but it does represent an effective tool for bringing real-time, evidence-based decision support to physicians. Nurses are the last defense level of protection against medication errors, and are solely responsible for the dispensing, administering, and monitoring of medications. In healthcare, computers can be used to help facilitate clear and accurate communication between health care professionals. Wal-Mart operates fewer stores than Sears but is ahead in terms of total selling area by a ratio of 3.4:1. Between 1995 and 1997, Sears’ retail store revenue per selling square foot was not only lower than that of Wal-Mart but in decline.
Since Wal-Mart is a mass market retailer, its primary source of value that it adds to the company is derived from its supply chain. Wal-Mart has suppliers located all over the world and it purchases goods from a wide range of different types of vendors. Many of the company's primary vendors are directly connected to Wal-Mart's IT systems through what is referred to as an electronic data interchange (EDI). An EDI can instantaneous transmit data between Wal-Mart and their vendors. Such information can consist of order information, stock supplies, demand forecasting and many other key supply chain metrics. The advantages of such a system are clear as they can greatly assist creating efficiencies in the supply chain. However, not all suppliers have developed sufficient IT technologies to participate in an EDI program with Wal-Mart. Another option for greater coordination between parties in the supply chain is web-based supplier integration. Although the web-based systems are not quite as sophisticated as an EDI, they are more accessible for many of the smaller suppliers and they have shown to improve long-term coordination, cooperation, and commitment.

Sears is facing a huge problem when it comes to making profits anymore and they are continuing to shut down stores. They have already shut down over 100 locations due to them not being as successful as they used to be. They also have a failed stock, drained all their resources, and they are loaded with debt. This company is not able to pull through and make profits anymore because their CEO is not capable of making changes to the company to bring it out of it. He sees that the company is not as presentable as it used to be and it does not have the same appeal. People are not going to want to go to a store where it does not have anything to offer anymore. According to Kern and Rucci, Sears developed a conceptional model of performance that links employees, customers, and shareholders in a causal chain. The model is known for three C’s, creation of a compelling place to work, compelling place to shop, and a compelling place to invest” (Kirn,Rucci, Huselid,and Becker,1998). In 1995, Sears put together a team of skilled econometrics to help gather data from 800 stores throughout the system (Kirn,Rucci, Huselid,and Becker,1998). The analyses gathered confirmed that Sears corporate financial performance is the issue that is lagging the organization behind but this information didn’t really provide detailed information. After experimenting, Sears was able to create a conceptual model of a casual chain, involving employees, customers, stakeholder (Kirn,Rucci, Huselid,and Becker,1998). The strategic
