For Shelby and Mark, they are faced with determining what insurance policy will fit their needs now and in the future. With Shelby owning her own business, this health insurance policy will cover both her and Mark, so consideration needs to be given to the health of both individuals. The health care insurance planner sheet will allow them to review the numbers behind both policies to determine what is best for them financially, as well as in the event that one of them should suffer some type of medical crisis. This form will give them the tools to make an informed decision concerning the out of pocket costs for both HMO and a traditional policy. This past week, two failed cases occurred days in between each other. Eight-year-old Michelle Moore was found in what has been deemed a homicide in her Newark home and twelve-year-old, Genesis Rincon was arrested for possession of narcotics with intent to distribute. These two clients were both supposed to be under the supervision of our case managers. Upon the conduction of an internal review from our Inspector General, it was found that not only did these caseworkers have a higher amount of absenteeism regarding client check-ups but one of the caseworkers is romantically involved with her superior - an Assistant Commissioner who was recently transferred from a different department.

The weather has been gloomy and cold, thus, the youth needs to keep warm. WYP Ngo discussed with the youth about any concerns. The youth wants some video games to play in his mother’s console. WYP Ngo is open to the idea, but the youth must do some community services before getting any video games. The youth has not started his community services. The youth also want weights to workout

T-con dated on the 8 September 2015, Mother of patient could benefit from pediatric s case manager. On 9 Sept, Case Manager met with Mother of patient and documents that this visit was an extensive late visit; at the conclusion of this visit case manager stated that she would contact specialty providers and ultimately secure appointments. On the 23 Sept, the case manager had received two TCON’s regarding assistance with medication management. The case manager documents that she left a message on Mother’s phone, that she should contact he child’s psychiatrist for appointment
D: Case manager met with participant on 5/4/2018. The participant comes to classroom prepared to participate each day. She is always on time and has a positive attitude. Today the participant appeared to be in a good mood and reported that she and the family are doing well. Participant is making good progress in class room actively and independent assignments. However, Case manager is still exploring type activity would be most suitable for the participant’s current skill sets.
