My third rotation was at Urbandale Clinic- Aurora. This rotation allowed me to experience what the clinic setting was like and see what is similar and different from the hospital laboratory. I began my day at the clinic at 7am, in order to see QC and the opening up of the lab. The Urbandale clinic lab has a chemistry analyzer, a small hematology analyzer, a Clinitek and rapid ID microbiology tests for influenza and test similar in nature. There were many similarities differences between the clinic lab and the hospital lab. Three differences that I noticed about the clinic lab compared to the hospital lab are the technologist draws blood, only basic testing is completed onsite and the machines are of lesser capability than those at the hospital. First the technologist draws all the blood that needs to be collected before the patient leaves, as soon as it is drawn it is placed on the machine or in the centrifuge, if the testing is available. First, the processes of QC and QA are exactly the same, with the same methods of QC for the chemistry and the hematology machines, and similar recording methods to keep track for accreditation. Second, there is a lot of problem solving, maybe more so in the clinic than the hospital because you are the only person in the clinic lab whereas in the hospital, there are more people to trouble shoot with. Third, the samples are run as quickly as possible, ensuring the that the patient comes first. Samples that are able to be run on site are run as soon as they are back into the lab, this ensures that the doctor can contact the patient as soon as possible with results and get them the treatment or further help if needed as quickly as
In the Core Lab, I was assigned to hematologist named Melissa. She was very experienced in her work since she knew how to operate machines that help her detect any abnormalities in the blood samples. Regularly, the hematologists and other lab workers would work independently, and I like the method of independent labor because there won’t be any obstacles while testing in the lab. I also liked observing the blood samples through microscope and see whether the blood have any kinds of disease or irregularity. The hematology was very interesting and enjoyable; I was able to experience and learn that hematology is another way to test and figure out life threatening unusualness in the body.

My field placement is at Highland Clarksburg Hospital. I work with an amazing woman named Amy, she is a case manager on the children’s unit. Highland Clarksburg Hospital is acute care hospital. So the patience can be there for a little bit depending on their insurance. Why the hospital is there is because there aren’t that many hospitals around to help children in need of behavioral health. History of Highland-Clarksburg Hospital dates back to February 1, 2010 when Clarksburg’s City Hall announcement was made UHC would donate the facility. The United Hospital Center built a new facility. They opened of the Children and Adolescent Unit in August 2013 and the reminder in January 1, 2014.

According to the Centers for Medicare and Medicaid Services, cost is expected to rise to twenty percent by 2015. Nearly, seventy percent of medical decisions are based on laboratory tests, yet the costs of lab tests account for only four percent of the total in health care costs. Annual sales for clinical laboratory testing in the U.S. in 2001 were thirty-five billion, and are expected to grow at four percent annually (Johnson, 2008). Upon looking at annual global growth rate, laboratory testing is projected to increase by 5.5 percent (Johnson, 2008). As a result, laboratories as a business plays a significant role, on how health care dollars are spent. Additionally, laboratories offer a tremendous service for a reasonable price by providing quality, state-of-the-art services and fair reimbursement that is essential.
South County Community health team (CHT) is small agency housed in the larger South County Hospital that works with Primary Care Physicians to assess and assist at risk patients. Care Transformation Collaborative funds six employee positions on the CHT through the department of health insurance commission and the executive office of health and human services to “promote integrated, patient and family centered care data driven quality improvement, and prepare practices to perform successfully under Alternative payment models” (CTC, 2014, p. 2) .The CHT is compiled of six women ranging from 22-60 years of age. All of which are White except myself of Native American and African American decent which mirrors the demographic in Washington county that is served. The team consists of one Data Analysis, a LICSW, a Behavioral Health Care Manager (BHCM), a team lead and two Community Health Workers (CHW). My role on the CHT is a part time visiting CHW with an active caseload of 25 clients. I work closely with clients to provide resources to help them manage their care and wellbeing. Patients get referred from their Primary Care Physician that have high risk factors reported by the patient’s insurance company. The CHT works alongside Nurse Care Managers in Primary Care provider offices to assist clients that have complex medical conditions, behavioral health issues, substance abuse complications, social disorders. Healthcare system provides services to those who can pay for them; unfortunately, not all people can afford to have insurance and get medical care when they need. There are many poor people who are left behind this system and something needs to be done in order to provide these people with appropriate care and equal opportunities. Opening a free clinic is a way out of the situation as it will allow people without insurance rely on the system of healthcare in the country and significantly improve the their life quality. This project needs to include the analysis of economic issues and special needs in order to develop a coherent outline for further steps in project implementation.
