South County Community health team is an agency under South County Hospital that works with Primary Care Physicians to assess and assist at risk patients. The funding for the six employees comes from a grant through the Care Transformation Collaborative. A department of office of the health insurance commissioner and the executive the office of health and human services to “promote integrated, patient and family centered care data driven quality improvement, and prepare practices to perform successfully under Alternative payment models” (CTC, p. 2) .The community health team is compiled of six women ranging from 22-60 years old. All of which are Caucasian except myself of Native American and African American decent much like the Washington county of population that is served. The team consists of one IT programmer, a LICSW, a Behavioral Health Care Manager (BHCM), a team lead and two Community Health Workers (CHW). My role on the community health team is a visiting Community health worker with an active caseload of 25 clients. I work closely with clients to provide resources to help them manage their care and wellbeing. Patients get referred from their primary care physician that have high risk factors reported by the patient’s insurance company. The Community Health team works alongside Nurse Care Managers in Primary Care provider offices to assist clients that have complex medical conditions, behavioral health issues, substance abuse complications, social disorders. A non-communicable disease (NCD) is defined as a disease which is not infectious. Such diseases may result from genetic or lifestyle factors. Such diseases may result from genetic or lifestyle factors. Current evidence indicates that four types of NCDs (i.e. cardiovascular diseases, cancers, diabetes and chronic respiratory diseases) account for almost two third of all deaths globally, with 80 per cent of these deaths occurring in low-income and middle-income countries. NCDs are a leading threat to health and development. Yet, these diseases are preventable. By eliminating shared risk factors such as tobacco use, unhealthy diet, physical inactivity and the harmful use of alcohol, almost 80 per cent of heart disease, stroke and type 2 diabetes and over a third of all cancers could be prevented. In addition, improved disease management can reduce morbidity, disability and death and contribute to better health outcomes. Overall, proven cost-effective strategies do exist to prevent and control this growing burden.

In early April 2013 a measles outbreak was discovered in North Carolina. By mid-May the outbreak had been identified in Stokes and Orange Counties via 23 active cases. Every case was linked back to a family that had spent 3 months in India and had not been vaccinated. By the 16th of April the state laboratory of Public Health was able to confirm the diagnosis, with the last known case being confirmed on May 7th. The investigation of this outbreak revealed 4 patients with a confirmed diagnosis that had received one of vaccination of the two part series. The other 19 cases had not ever been vaccinated.
Disability- many people suffer from discrimination when they have a disability. This is because they are different from others and can not physically do the things others can. This leaves them left out, not been able to socialize and feeling upset. For example if there was a man in a wheel chair in an office with his work colleges and everyone ignored him because he was different this would be discrimination.
Health Promotion This is the process of allowing people to have more control over their health as this will help improve their health and well-being. It focuses on individual behaviour towards a wide range of social and interventions. Health promotion is how health is shown for example to challenge obesity change 4 life is used so people can have a guidance on how to make their lifestyle healthier. It also shows that health promotion is aimed at individuals whose health is at risk from any diseases from lifestyle factors, which lets individuals take control over their health. By being able to have control over their health it shows individuals what may have happened if nothing was done of it

Honesty is something I value in another person, and I have struggled with service users who haven’t been honest about something. I have tried hard to understand the reasons why someone would not be honest with me. Usually after I have spoken to another member of staff, and heard their input and theories I can begin to see why and move on from the incident.
Over the year both positive and negative influences have influenced my learning. The skill I have developed for learning is my aspiration is to become a midwife, this influences my learning in the way that it gives me the hunger to learn new things and improve my knowledge and grades. By improving my knowledge I will be able to go onto the next stage of my development. I believe that if I did not have such a desire to be a midwife, it would affect my learning by creating a mental blockage which would stop my ability to take things in and learn as I would become lazy and not interested from the lack of direction in my life. As soon as I realised what I wanted to do as a career, it really highlighted my priorities and I found that I have
