I was dispatched to 113 W. Orange Street in reference to a domestic call. Upon arrival, I made contact with Brenda Colvin. Colvin advised that he husband picked up a chair and threw it at her feet. Officers did notice swelling to Colvin left ankle and a flipped over wooden chair located on the front porch. Colvin advised that her husband(James Wigfall) had been drinking and that’s what was making him angry about everything they were talking about. Colvin advised that she confronted him to talk about him drinking so much and things that needed to be done around the house. Wigfall then got an attitude. Colvin advised that she told Wigfall that he needs to leave the house for the night and he refused so she called the police.

CW went to 1261 Everton Dr, Apt 9 Akron, OH 44307 because they were called out by Akron Police Department (APD) for allegations of abuse towards 2 children. CW Jonathon Henry and CW Sonya Cole pulled up to the apartment complex and observed that there were over 15 people sitting outside. Both CW could tell that this was the correct address because there were 3 police cars that were sitting outside. As the CW's were walking towards the apartment complex an APD officer asked if the two workers were from CSB. CW Cold responded that they were. The officer then told CW's that there were not 2 children, however, there was 3. The officer stated that there was one child in the bedroom sleeping that they did not know was in the home. CW's were also told by the officer that Mr. Brian Burrell (AP) was in the home and that he was very agitated. The officer told the CW that they had called him down, however, suspected that he would get upset after entering the home. The officer also said that he already knew that the kids were in the custody of CSB and that he would most likely not be happy that the two CW's were at the home. The officer the led both the CW upstairs to apartment 9.
When I arrived on scene, I contacted the caller identified as Jordan Lundy. Lundy stated he was with his girlfriend and they went to the dispatched address to speak with her cousin. Lundy was inside the residence where a female confronted him. The female was identified as Alesha McCool. Lundy stated McCool was his girlfriend's cousin. Lundy said McCool began yelling at him calling him a thief and other names. Lundy let McCool yell at him because he wanted to amend the relationship with McCool. Once McCool stopped yelling, Lundy said he turned around and walked out of the house. Lundy advised McCool followed him out of the house continuing to yell at him. Lundy reached the street and he told McCool "Fuck you" but she continued to follow him into the street. McCool approached Lundy in the street where she pushed him with both hands. Lundy informed me he was pushed so hard he "flew" backwards but stayed on his feet. Since Lundy stayed on his feet, he stated McCool walked up to him and slapped him across the face. On the afternoon of November 14, 2015 at approximately 5:07 P.M. Deputy Singleton responded to a domestic disturbance call at 4547 Old Salkahatchie Hwy in Early Branch SC. When I Deputy Singleton arrived on scene I interviewed Sheree Washington who stated that after Deputy Ayer had left the scene from the previous call that her boyfriend Christopher Williams who resides at the residence said "bitch I should kill" you then proceeded outside, and retreived a light with a piece of pipe attached to it, and then struck her in the chest. At this time Washington called dispatch, and asked for a Deputy to come back out to her residence. I entered the residence and was directed to a bedroom on the north side of the residence. I entered the bedroom and observed a white female standing next to the bed. She had a large, blue in color, lump under her left eye. She was unable to open her left eye very much. I later identified her as Anna Williams by Arkansas drivers license. I asked Anna what had happened and she said she was trying to leave the residence and they hit her. I asked Anna who "they" was and she said Gilbert and Judy. Anna said they hit her with their fists. At about this time, Mulvane EMS arrived on scene and started to render aide to Anna as I collected her personal information.

The reporting party (RP) stated resident Sofia Miramontes age 27 DOB: 11/23/88 has developmental delays, deaf in right ear and is bipolar. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The Officer informed the Director of agency Sherry Winston that the resident was coherent and walking to her boyfriend’s home. The RP contacted the facility and spoke with a staff

On 10/02/2016, at 0431 hours your Affiants were dispatched to 11 1/2 West Oak Street where the caller, David Grady, reported his ex-girlfriend broke into his home and was not authorized to do so. Grady indicated he had left his home on 10/01/2016, at approximately 1545 hours, to attend a class reunion and afterwards went out with friends. While at a friend's home his ex-girlfriend was there and asked if she could come over. He replied maybe later and that he would let her know. Shortly later she left his friend's home and he began receiving text messages from her which he showed Officers the messages. She messaged him saying his door was locked and he replied I know. I responded to Suntrace MHP Lot 37 in reference to a domestic call. Upon arrival, I made contact with Jessica Ray. Ray stated that this moring around 04:00 her boyfriend (Carlos Stewart) hit her in back of the head. Ray stated that he always hit her but this time sheve had enough and want something done about it. Ray stated that Stewart did hit her on back of the head this morning but officers didn’t see anysigns of a physical altercation on Ray. Ray did have some bruising but she advised that it was from a past altercation.
On April 8, 2016 at approximately 1500 hours, I made contact with Heather Curtis at 8307 Centennial Road in reference to a domestic dispute. Mrs. Curtis stated that her husband Marc Curtis sent her multiple text messages to her phone stating that she couldn’t have the kids and that the police would have to kill him before he goes to jail or give her the kids back. This was the result of an argument after Mrs. Curtis refused to pick him up from work because he had taken her EBT card. Mrs. Curtis stated that he hasn’t been acting normal and may possibly be going through a psychotic break. On 9/12/2016 at approximately 0740 hours Deputy Chavez B1871 and I responded to a Assault DV in 9615 South 7th avenue Phoenix, Arizona 85003. When we arrived at the residence we heard Amy Frieberg scream, we entered the house and Ron Frieberg and his wife where having a verbal argument and where in each other faces. We separated them both I took Amy outside to talk to her and my partner stayed inside to talk to Ron. I noticed that Amy had a swollen right eye and a red mark above her right cheek. I responded to 410 South Brundidge Street in reference to a suspicious circumstance call. Upon arrival, I spoke with Michael Peacock. Peacock advised that he heard a loud "boom" and someone going through the residence. Peacock stated that he just thought it was one of his roommates. Peacock stated that someone started kicking his room door and he grabbed his knife and ran to the bathroom. While in the bathroom, Peacock called 9-1-1 and locked the door. Peacock couldn’t advise on anything missing at this time. There was a busted out window in the back of the unit. Photos were taken on my body cam.
On 12-09-16 at about hours 0748 hours, Officer Petrella and I responded to the above location, regarding a PC 273.5(a)-Domestic Battery investigation. The victim and Suspect Goldbaum are currently living together and have been in a dating relationship for the past two years with a common son. The victim stated, on 12-09-16 at approximately 0740 hours, she was involved in a verbal argument with Goldbaum. While inside of the their bedroom, Goldbaum forcefully pushed the victim onto their bed. Goldbaum then used his right fist and punched the victim twice in her stomach. Goldbaum then placed a cover and pillow over the the victim's face for approximately two minutes. The victim had a hard time breathing with the cover and pillow over her face.
Requested to 605 West Penn Street for an unknown situtation. Schuylkill County Communication Center advised they could hear a lot of screaming in the background. Your affiant arrived on scene along with Shenandoah Area Ambulance. We were directed in the home to the second floor where the female was lying in bed. EMS attempted to assess the female's reported condition and while doing so she became verbally and physically aggressive with them.
