Ms. McGinnis is an 82-year-old female, with history of Alzheimer dementia, diabetes, and hypertension. Her primary caregiver is her son, Michael McGinnis. Mr. McGinnis has admitted to the ER doctor, multiply employees at Senior Care and to DHR worker on the phone that he gave his mother methadone. Ms. McGinnis is not prescribed Methadone and is mental unable to consent to taking methadone. Ms. McGinnis is currently at Cordova NH waiting on Medicaid approval for long-term placement. Therefore, a case is being opened to assist with application and Ms. McGinnis transition into NH.

Julie Mialaret is my mother and she is 53 years old. My mother exercises very rarely because of her intense work schedule. She says it is hard for her to have the energy everyday to work out after long work hours. She has never smoked in her life because she does not find it appealing. Mrs. Mialaret eats a very low-fat diet and eats very small portions. My mother eats very healthy and is able to maintain her weight without exercise. She faces no barriers when it comes to eating healthy because growing up she ate little portions and couldn't afford fast food. I would like to change my eating habits to better reflect my mom's diet however, I would like to incorporate some sort of exercise into my daily routine. My mom hopes to start exercising

Rosa Cunningham (full name is Rosa Lee) is a 53 year old African American female client of average height, slight build, and is appropriately groomed. She has 8 children, 2 of them being female and 6 males, all adults. Rosa is a widow and reached this status after being separated from her deceased husband for many years. Rosa is currently hospitalized for pneumonia, and has been hospitalized several times in her life for diferent illnesses. Rosa’s medical history as self-reported is HIV and seizures. Rosa is a heroin addict and has been this way for several years. Rosa is involved with the local methadone clinic and receives 55mg of methadone daily. Even by receiving this daily dose of methadone, Rosa continues to use heroin. Rosa has several legal and health issues that are present also, despite which she continues to use heroin. Rosa has a lengthy criminal history to include arrests for prostitution, larceny, and selling drugs. The reason for today’s assessment is a referral made by the social worker at the hospital in which Rosa is a patient at and discharge planning is to be made for aftercare.

The client for this paper is a twenty-seven-year-old single mother to a six-year-old son. The client was raised in a hardworking middle class family in which her father built a successful business. Her parents divorced when she was ten years old, and she reports she never heard them argue before they announced to the family they were separating. The client describes the divorce as “ugly.” She started smoking marijuana with her friends at the age of fourteen which led to her opioid intravenous addiction by the age of eighteen. The client’s drug of choice is hydrocodone. She admits she has never tried to abstain from her drug use and has been arrested over twelve times as a result of her addiction. Her last drug charge resulted in a six-year prison sentence at the age of twenty-three. The client is currently court ordered to successfully complete an in-patient drug treatment program for up to eighteen months. If she successfully completes the program, she will be released two years early from her sentence. The client has been diagnosed with 304.00 (F11.20) Opioid Use Disorder – Severe which continues to make her eligible for treatment. Client is Hepatitis C positive.

Ms. Clemmons worked on the following contracts: Wells Lamont (assembling clips with buckles and rings and using machine to cut out pieces for gloves), Star Herald (collating inserts), and Anthony's Syrup (labeling and packing containers). She also did janitorial work at two churches and an office building. The instructor noted the some limitations such as too many absences and fluctuations in daily work levels (moves too slowly at times). Also, she had four absences within a 3-to-4 week period. She was also late on March 10, 2017. There are times, she cannot provide a valid excuse for absenteeism or lateness. However, she did provide an excuse from the dentist on March 03, 2017 for her lateness.
Mr. Robinson substance abuse problem is alcohol. Thus, he does have an uncontrollable opiate dependence due to chronic pain problems of his illnesses. According to clinical screening his mental status/mood is depressed and his memory is impaired short and long term. Ms. Lewis the case manager states when interviewing Mr. Robinson, he stated, “I do not do drugs but I drink and I am tired of alcohol because life has passed me by and if I could get help what would I do with my life, my legs, ankle and foot are damaged and my memory is lost”. “My family hates me and my children disrespects me”. Mr. Robinsons mother said, “she knows her son has seizures convulsions and high blood pressure, mental illness, however he will not stop drinking”.
