On 3/7/16 at 4:50 P.M, Shift Supervisor (S/S) Enmanuel Cabrera was notified by Security Officer (S/O) Zachery Smith, that Yard Jockey Sean Barchik found a box of Trident gum in the Truck Yard by Dock door 59. S/S Cabrera then went out to the Truck yard to take pictures and collect a witness statement. Upon arrival, S/S Cabrera was met Yard Jockey Sean Barchik, he explained that he found the box of gum while performing a Yard inspection. Upon discovery, the Yard Jockey immediately notified B Shack security of the merchandise that was found. Security will launch an investigation to see if we can discovery how the box of gum get there.
Upon arrival, I made contact with King, Demetrius D. DOB 02/21/89, who related he was at White Castle, so his cousin (Staples, Cierra N. DOB 08/04/85) could use the restroom. I observed a open bottle of Remy Martin on the passenger floor board of his vehicle (04' Pontiac Temp 877S801). Two (2) photos were taken on the scene and later placed into inventory. King and Staples were checked through LEADS via dispatch. The name check revealed they were clear with no wants or warrants. I made contact with Stasik, Katarzyna DOB 08/02/89, who was next to her vehicle (00' Honda IL Temp 206T552). Stasik related that she purchased Heroin at the White Castle from King. Stasik further related that she had nine bags of Heroin concealed in her pants. Stasik removed the nine bags from inside her pants and threw them on the ground. At
On 07/11/2016 at approximately 1149 hours, I, Security Officer James A-10280, was notified by a guest of a African American male selling a iPod on the Tulalip Casino Gaming floor. I made contact with a guest now known to me as Wade Maurice DOB: 08/24/83, who stated that he was trying to sell the IPod. I explained to Mr. Wade that selling any items in the casino is not aloud, and he would have to leave all Tulalip Gaming establishments for no less then 24 hours. Security Shift Supervisor Randy Stewart was notified.
On 11/4/16, I, Account Manager Enmanuel Cabrera, was notified via email by Shift Supervisor Massiel Perez that Security Officer Bailey Forte no called no showed. S/O Forte is usually off on Fridays, but was scheduled in advance, 0200-0600, to cover the open post. S/S Perez made several attempts to call the officers from 1st shift to cover the open post, but was unsuccessful to get someone to come in. Also on 11/3, during the afternoon parking lot detail AVP1 was 1 flagger short, all vital positions where filled with the exception of the parking lot supervisor/rover. As well as on 11/4, morning parking lot detail was short 1 flagger. Daniel Canela no call no show for the AM parking lot detail. All vital positions in the parking lot were filled for the exception of the supervisor/rover position.
On 12/8/16 at approximately 11:30 AM, An Outbound Desk Clerk notified Security Account manager Enmanuel Cabrera that a yard jokey found a trailer with damage. Upon being notified, A/M Cabrera instructed Security Officer Larry Mayer to take pictures of damage and to gather the information needed. Upon arriving at the location where the incident occurred, S/O Mayer was met by RJ davis Yard Jockey Mark Billus JR. Yard Jockey Mark Billus JR stated that, while pulling XPO trailer W17150 out of dock door 6, he notice it was too close to dock door 7 for the TDR crew to properly seal the trailer. A number of police officers set up a sting in which there were to be buy of crack cocaine outside an apartment complex in Lexington, Kentucky. Undercover Officer Gibbons watched the deal from an undercover car in a parking lot not far the from the sale area. After the deal took place, Office Gibbons radioed several other police officers. He instructed them to close in on the suspects. He advised the officers to “hurry and get there” because the suspect was making his way towards the breezeway of an apartment building. Officers arrived at the parking lot, leaving their vehicles and run to the breezeway. As soon as they enter the breezeway they hear a door shut and can detect a strong odor
On 26 June 2017 at 1516 hours, Officers were dispatched to the area of 305 East Penn Street for a theft of a vehicle. Officer were advised the actor was a Rhiannon Meyer and that she had stolen her dad Dodge truck green in color with a plow on the front. and was last seen heading towards Main Street. Officer began to check the area of Main Street, when Unit 191 called out with the vehicle at 100 Pennsylvania Ave, in Shenandoah Heights. Your Affiant (Patrolman William Moyer) along with Patrolman Tyler Dissinger responded to Unit 191 (Chief Livergood) location. At arrival Chief Livergood had MEYER in custody along with the stolen vehicle. MEYER'S was still yelling about someone having a gun in the window of the residence and was not making sense
On 2/17/16 at approximately 8:30 P.M, Shift Supervisor (S/S) Enmanuel Cabrera was notified by Safety Specialist Matthew Marsicano that IH services associate Elizabeth Andrisoqie was asked to be removed by IH services Project Manager (P/M) Courtney Berger via cell phone. Upon arrival, S/S Cabrera spoke with IH services associate Elizabeth Andrisoqie and advised her that IHS P/M Berger asked for her removal. At this time, IH services associate Elizabeth Andrisoqie refused to leave. After, S/S Cabrera advised IH services associate Elizabeth Andrisoqie, that her refusal to leave the premises forces security to contact the State Police for trespassing. This officer responded to Priced Rite Tree Service in reference to a stolen vehicle. Upon arrival, this officer contacted Middleborn, who advised they just noticed their 1988 Chevrolet black pickup was missing. Middleborm advised the vehicle was stolen sometime within the last 3-4 days. Middleborn stated an employee was supposed to take the vehicle in for repairs. Middleborn stated he called the company and learned the vehicle was never dropped off. Middleborn stated he spoke with his employee and learned the employee thought the vehicle was already in for repairs and did not check with anyone.
Feik said at approximetley 0200 hours Feik entered Kwic Shop to buy chewing tobbaco. Feik said he parked his blue Honda Insight VIN# JHMZE2H71AS038802 Kansas Tag 468GDG in the front of Kwik Shop, east side by the propane tanks. Feik said the only other car in the parking lot at the time was a Black SUV parked on the west end. Feik said he noticed only one customer leaving when he entered. The customer leaving was a hispanic male, in his late 30's, approximetly 6'0 feet tall and 225lbs. The hispanic male was wearing a red and white baseball cap, and a dark colored heavy jacket. Feik said there were no other customers in the store while he made his purchase at the counter. On 9/28/16 at 8:30 AM, Area Manager Daniel Ahner notified Shift Supervisor Ashley Lorah that a RJ Davis Yard Jockey caused damage to a trailer while backing up into dock door 4. RJ Davis Yard Jockey Danny Gorski, driving tractor 241 pulling trailer P5076637 out of slip location F-957, accidently damage the trailer’s door while backing up into a dock door. Mr. Gorski stated that while he was backing the trailer into the dock door, he notice that trailer’s door unlocked and hit against the building. No damage was found to the dock door but, the hinge on the trailer’s door was damaged.
On 6/2/16 at about 7:30 PM, RAD 1 associate Christopher Veizaga (veizaga) asked security if he would be able to leave out of the B building bypass with a large box. The box contained 17 TC-55 also known as dolphins. Shift Supervisor (S/S) Enmanuel Cabrera then asked Mr. Veizaga, why exactly did he have to remove the box out of the building? Mr. Veizage stated that it was a last minute shipment to another site and did not think that he needed to put an asset removal form because it is not product. At approximately 2050 hours, an unidentified inmate put a note in the Tower Trap stating " That wasn't a tray that was dropped earlier, it was the inmate in A05001 that got assaulted.”
Facts: On December 20, 2014, LAPD dispatchers requested that LAPD Officer Luis Provenza respond to a noise complaint at an event that was taking place at the California State University of Long Beach. As Officer Provenza approached the on campus event he noticed a red Honda Civic driving out of the parking structure. He believed that the driver of the red Honda Civic resembled Professor Peter Plum, a professor he had found drunk at this same event the year prior. Officer Provenza mentions that he saw the driver of the red Honda Civic say “oh no” when he noticed the Patrol Vehicle and proceeded to put a stick of chewing gum in his mouth. As a result, Officer Provenza started to follow the red Honda Civic for several blocks. No moving violations
Monday, August 1st, 2016 at approximately 2:10 p.m., a follow-up interview was conducted by Detective L. Donegain withy Dana Dudley at the Police Department. Dana Dudley admitted to knowing the person who committed the shooting at the Tobacco Mart. Dana Dudley stated he was Anthony Smith’s residence with Jamarqus Hurley playing video games. Dana Dudley stated he Anthony Smith and Jamarqus Hurley walked to the Tobacco Mart to purchase a drink and cigarettes. Dana Dudley stated while they were standing at the night service window a vehicle backed into a parking spot in-front of them. On 6/21/16 at 7:40 PM, Security Officer (S/O) Judy Peffer, who was at the exit gate, notified Shift Supervisor (S/S) Enmanuel Cabrera, that the trailer at Dock Door 3 appeared to be collapsed.
