CM was out on vacation from the period of 5/31/2017 to 6/14/2017. On 6/20/2017, CM met with the client to complete Bi-Weekly ILP Review. Today, client was calm and quiet in the meeting. She was dressed appropriately for the weather. Client affects and mood was in appropriate. Client continues to appear to have cognitive impairment and she doesn’t grasp information. e.g. when try to explain herself she will have flight of ideas. She doesn’t provide a complete story of the event before she jumped into another subject. Client continues to report no social support system. Client also reports she sometimes goes to Bread of Life for moral, food and spiritual support. Client is undocumented and has no source of income. Client has an Open ACS case and Ms. Ayala/Case Planner at Edwin Gould Foster Care Agency. Client child visitation is temporarily suspended until the further notice. CM reiterates the shelter rules and regulation for the client to abide with the 10pm curfew. Client was a victim of domestic violence and she is a candidate for the U-Visa, but the client need to submit Police Report and documents to proof her allegation; in which the client doesn't have. Client continues to report she actively participating in Court mandated Individual Mental Health session at Woodhull Hospital and next upcoming appointments are as follows Dr. Faisal Chaudhry, MD for 6/28/2017. Client continues to reports no substance history. Client housing package is incomplete. Missing income and Green Card and income. Client continues to display slow intellectual impairment improvement. Client reported she does have family members here NYC. She has an uncle residing in the shelter systems. She doesn’t have any community resources. CM advises the client to enroll in ACE Job readiness program.
Client reported to CM she has a new granddaughter and tomorrow she will be spending the day with her. Client report no community support. CM enrolled the client to Adult Recreation Center.
Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice.
At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
Social Services: On 08/08/2016, client Denise Worthy and her 13 year old daughter Sanai McDuffie met with assigned Case Manager Ms. Gilgen for Intake Assessment and Initial Independent Living Plan (ILP). Client is 37 years African American female. Case Manager asked client how and why she became homeless. Client stated that the reason she became home was due eviction. Client stated that she was living with her husband in New Rochelle but due to domestic violence she decided to move to Alabama. Client stated that she was living in Alabama for about a year. Client stated that she lost her job and couldn’t continue paying for the rent. Client stated that after the eviction her family was sleeping in abandons houses for about a month before coming to NYC. Case Manager asked client if she has any family member in Alabama, client stated no. Client stated that she moved to Alabama to get out of her domestic violence situation.
Client is non complaint with the shelter rule and regulation. She has numerous Infractions from Security stating that the client does not vacate the dorm at 9am. CM reiterates the shelter rule and regulation. Client must comply with curfew and maintained a clean area in her bed area clean. Client must adhere to 10PM Curfew.
Rules, Regulations, and Curfew: Ms. Hair was reminded of the facility’s curfew, rules, and Regulations. Case Manager asked Ms. Hair if she was aware that she has violated curfew several times since her last ILP meetings. Ms. Hair stated that she was aware. Ms. Hair was reminded that during intake she was informed that violating curfew is considered non-compliance. Ms. Hair stated that she was aware. Ms. Hair stated that the reason that she has violated curfew was due to her work schedule; however Ms. Hair has failed to submit proof of her work scheduled.
On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
Rules and Regulations/Curfew: Case Manager reminded Ms. Williams of the rules and regulations of the facility. Case Manager asked Ms. Williams if she was aware that she violated curfew on 0922/2016. Ms. Williams stated that she is aware of the curfew as well as the rules of the facility. Case Manager reminded Ms. Williams the facility’s 9pm curfew. The patient was placed on HOLD to see the writer to address his non-compliance with treatment. The patient was reminded about his Step 3 of the patient engagement. According to the patient as the writer reviewed the patient case history of his no show for counseling, group attendance, and continuously AWOL, the patient only response was, " I, know." The writer then inquired of the patient efforts to engage in mental health services through ICRC. The patient admits that he haven't done the intake when the deadline was extended for the third time. The writer discussed with the patient about the risk of facing an intent to discharge due to his non-compliance and addressed alternatives such as suboxone and transferring to a clinic in Massachusetts to accommodate the work location. The patient declines the writer's suggestion as he wants to remain with HCRC-Hartford due to the positive treatment and said. " You guys really care....I do not want to be discharge.....I, mean what is the process of the intent of discharge?" The writer explained to the patient about the appeal process as his record will be reviewed by the Practice Manager to determine as to whether or not to forward with the discharge or the discharge to be overturn.

CM received report from LAMP VASH worker Laurie Garza regarding client’s missed appointment on 11/25. LAMP VASH worker Laurie Garza inquired about client’s drug use and general well-being due to client’s lack of motivation. LAMP VASH worker suggested client explore different housing programs due to client’s resevations regarding weekly meetings with case manage . CM will discussed new housing referral options with client. CM will notify Laurie Garza about client’s decision regarding housing program.
On 6/30/2015, client attended to her ILP meeting with her youngest son Keury. Child was well dress for the weather. Cm observed child very anxiety and he was telling client that he wanted to go to his unit because wanted to watch cartoon.
On 8/30/2017, CM met with the client to complete Bi-Weekly ILP Review. Client was dressed with proper attire for the weather. In the meeting client appears to be sad and quiet. CM inquires the reason client appears to be quiet and sad. Client looked at CM and didn’t say a word.
On 7/23/2015, client walk in the social service office and CM completed Bi-Weekly ILP Review. In the meeting client reports she was having migraine headache and she takes pain killer. She also reports she is actively seeking support in the Russian & Jewish community but she doesn’t have any luck. Client continues to relate Russian and Jewish community doesn’t get along with Uzbekistan citizen.
