CM was unable to reach Destiny (youth) regarding an updated on therapeutic services and employment. CM left youth a detailed message and requested to be contacted. CM provided CM’s contact information.
CM spoke to Maureen O’Dea (IIC) regarding a follow-up on youth. CM was informed that youth was staying in Newark, NJ with her mother and is now residing with her father in Jersey City, NJ. IIC and CM discussed concerns regarding youth judgment and employment search; IIC reported she has a scheduled meeting with youth on Sunday, 6/4/17. IIC will continue to keep CM updated regarding youth.
Clinician provided support to youth during this transitional period. Clinician assisted the youth with positive ways to communicate with MHS and foster sister in the home. Clinician encouraged the youth to get involved once enroll into the summer enrichment program and community. Clinician will assist the DSS worker and MHS
UCM: CPSW received a call from Ms. Messerli reporting that she needs the lock number for her storage from foster provider family. She also stated that she needs the boy's cell phone back. CPSW reminded Ms. Messerli that this writer will ask the foster provider about the phone and storage numbers. CPSW reminded Ms. Messerli to focus on her recovery and not to disrupt her children as well as the foster provider and if she needs anyting to let the CPSW aware of it. Also, CPSW reminded Ms. Messerli not to contact or call both children until the theripist approves it. Ms. Messerli stated that she will try. Howerver, Ms Messerli sounded unhappy and really upset about not contacting her children Nick and Alexander.
MSTT was unable to work with Jose and Juminia on this weeks goals because youth runaway from home on Friday evening. Youth remained out in the community for five days before he was find by his probation officer. Youth will be having a hearing to determine what going to happen next with him. MSTT will collabrate with Juminia and Ricky (JPO) to explore the situation and review all the outcomes. MSTT will continue to reach out to Juminia and Ricky during this time until a final decisoin is made by the court.
WCC Taylor updated WYP Ngo the youth was released from custody and advised WYP Ngo to not go out into the field. WYP Ngo will have to schedule a meeting with the youth secondary probation officer to meet on site with the youth in YRC (Youth Reporting Center). The youth will be living with his grandmother, due to his mother does not want to take him back. The youth is planning to go back to his mother. The next one on one will be on May 30th, 2017.
R.O.Y Reclaiming Our Youth Inc. is a facility dedicated to independent living located in the state of Ohio. We will work with youth and young adults who have been incarcerated due to having an undiagnosed mental of physical disability, that cause them to become incarcerated. We will work with those who are looking to overcome disabilities/mental disabilities, those at risk of dropping out, youth involved or at-risk of becoming involved with the courts or juvenile justice system. It offers assistance to youth and young adults, frequently homeless, as they transition into adulthood.
Case management services were provided through a face to face interaction in La Habra. Present at the meeting were WYP Ngo and the youth. The signatures were not obtained, because the youth refused to sign in. WYP Ngo checked with the youth’s teacher. The youth is not doing his assignments. The youth is sitting alone and staying away from his peers. The youth is showing no improvement from the last time WYP Ngo checked in with the youth’s teacher. WYP Ngo discussed with the teacher about counseling. The school does provide counseling. The youth’s mother will have to fill out some papers for the youth to start counseling. WYP Ngo monitored the youth and the youth seemed upset. The youth walked away from the one on one meeting. The youth went
Sergio Paredes (Court Liaison) faxed CM, Dre’quan (youth) court order from today’s (1/20/17) court hearing. Youth has to continue with court GPS bracelet, follow BPS recommendations, and must obey Sasha Walker (caregiver niece) rules. Youth next court hearing is 2/10/17 at 1:30pm.
CM spoke to Sebastian Bliffeld (IIC) regarding services for Leandro (youth). IIC and CM discussed the youth current situation and different residential treatment programs. IIC stated that he will meet with youth on Saturday, 12/3/16 at 10:00am. CM informed IIC that youth scheduled f/f meeting is on 12/6/16. CM will continue to keep IIC update regarding the youth.
CM spoke to Kim Shipmon (DCPP) via email in regards to a follow-up on mentor services for Joseph (youth). CM is aware that youth is currently on DCPP waiting list for mentor services; CM provided DCPP with a link to the Big Brothers Big Sisters application. DCPP will complete application for a mentor.
I called the constituent at (541) 391 6477 and spoke with Mr William Neria. He stated that was not following the appropriate channels, so he had some communication issues with his CW caseworker Mary.
One of her early clients, Three Rivers Youth, under the leadership of President and CEO Peggy Harris, was in need of assistance in brokering relationships between the police, the community
CM called Karen Williams (DCF Coordinator) at 856-339-6726 to inform her that youth is missing from Daytop program. CM explained to DCF Coordinator that case manager left a message on CM voice message on 1/26/17 at 7:10pm in regards to youth being missing. CM was informed to use code EL110; violation of probation.
CM spoke to Ms. Meghan (EMYL Clinician) regarding services for Jason (youth). CM and Ms. Meghan discussed the family concerns regarding a new clinician for youth. Ms. Meghan informed CM that she will discuss this matter with her supervisor. Ms. Meghan reported that Jason is doing well at EMYL and the team is planning for youth to reside in a group home closed to EMYL where youth can continue his education. Ms. Meghan will discuss this in more detail at the upcoming treatment team meeting on 12/15/16.
WYP Ngo attempted to see the youth in YRC (Youth Reporting Center), but was denied permission because the youth's secondary probation officer did not grant permission (did not have an appointment). WYP Ngo had contacted the probation officer (PO) and left a voice mail, but the PO did not respond. WYP Ngo was informed the youth has changed PO to PO Rivera (the youth did not change PO), due to the grandmother is a spanish speaker. WYP Ngo discussed with PO Rivera to confirm the statement. WYP Ngo will attempt to contact PO WIlson to get permission to meet with the youth again. The next one on one will be when WYP Ngo gets permission.
CM spoke to Maureen O’ Dea (IIC) regarding a follow-up on therapeutic services for Destiny (youth). IIC informed CM that youth has good coping skills and does not need IIC services. IIC and CM discussed transitioning youth out of Hudson CMO. CM will conduct 1st transition meeting for Tuesday, 4/25/17. IIC and CM talked about some challenges that youth endures due to her environment. CM noted she met with youth on 3/28/17 and provided her with school and employment resources. IIC and youth continues to meet weekly for 1 hour.
