These modalities and venues of communication may entail benefits and challenges to both consumers and providers. The one communication modality that sticks out the most and that is used in health care as well as marketed is the Electronic Medical Records (EMR). The EMR protects patients records by not letting them become exposed to the public. These records can also be kept by the patient as well as the doctor or any medical professional. The EMR is a software program used to enter patients information into a computer, which organizes and stores the information. I believe most offices will keep the paper charts in a very safe location or shred it. But I think because of the confidentiality that any and all information or records relating to patients is considered privileged. basically saying keeping all information about the patient confidential.

Electronic Patient Record (EPR) is where essential patient information during each visit or admission is stored. Historical events pertinent to each patient are also stored within each individual record. This system has also enabled professional care givers to quickly, securely, and effectively transfer patient information to other professionals on transfer, consult, or for advice (Wager et al., 2009).
In today’s healthcare industry, technological advances for the treatment of diseases are on the raise and constantly evolving. There is an increase on the usage portable devises capable of record patient’s information, computerized provider order entry (CPOE) stations for prescription drugs order entry use by authorized license health professionals and other systems that can process data and share by other healthcare professionals. With the introduction of an EMR which is a digital version of a paper chart in a doctor’s practice or office. An EMR consist of the medical record history of the patients in one practice, it provides the physician, nurse or clinician the capability of following up patient’s preventive screenings, vaccinations, and records the physician’s notes for future diagnostics and treatment.

Our society has made many advancements with communication. The use of cell phones has given us the ability to talk to anyone, anywhere, anytime. Computer technology has also made communication easier. Electronic Health Reporting, or EHRs, has contributed to patient safety by decreasing communication errors.
The focus on EMR utilization persists today in the legislation proposed by the Centers for Medicare and Medicaid Services (CMS) to improve meaningful use of EMR systems. The comment period for Stage 3 of the meaningful use proposal ended June 2015. However, many are hopeful that the improvement of EMR use will not stop there. EMR use should not be limited to each individual physician office but instead should be accessible to patients in an easily accessible format. Currently, it is difficult for patients to become fully involved in their healthcare because it is difficult to access healthcare records and healthcare records are not consolidated in one place. While the meaningful use proposal does aim to improve EMR systems, there is still much work to be done to fully consolidate records into a format that is accessible to patients.

Since their discovery, electronic medical records have been increasingly used by primary care physicians as a way to effectively manage the huge number of patient information. Many physicians and hospital staffs have stated that the EMR systems have had a huge impact on their management of patients' records and their communication with other staffs and patients. Consequently, these systems have enhanced the quality of patient care and lessened clinical errors though their financial implications remain uncertain.
Electronic Medical Record or EMR is basically the digitized version of the patient's medical record which might include demographic information, scanned copies of the patients report and any other information that might be collected by the healthcare provider about the patient. With EMR, it is usually collected and maintained by a single entity. The single entity can use EMR for diagnosis of treatment and the diagnosis reports cannot be shared with other entity because EMR are designed not to be shared by outside entity.
