On June 8, 2017 Quentin Smith came to the staff office appearing to be disorientated and groomed. Mr. Smith express that he wanted to go to respite in the meantime while he wait for transportation to St. Joseph’s. Wellness Coach Ms. Cain sent a referral to Parachute respite and a representative called informing that Mr. Smith has been there three times thus far. In stating that Wellness Coach Ms. Cain stated that Mr. Smith feels as if he is need of a changed environment.
Assistant Program Director Parachute Respite Gerald Jenkins provided Assistant Program Director Kia Gore and Wellness Coach Teresa Cain with alternative respite programs that Mr. Smith would like to attend but advised that they have a maximum stay of three times. Mr.

On June 14, 2017 Wellness Coach Ms. Cain knocked on Mr. Smith door again and he was not home. Wellness Coach Ms. Cain left a letter of appointment for Mr. Smith.
On June 16, 2017 Wellness Coach Ms. Cain seen the letter still on his door and knocked, however Mr. Smith was not home.
On June 20, 2017 Wellness Coach Ms. Cain and Hud Vash Heather Slater went to knock on Mr. Smith door and Mr. Smith did not answer the door.
Apartment Inspection:
Wellness Coach Ms. Cain knocked and conducted a wellness check which enters in apartment and Mr. Smith was not home. Wellness Coach Ms. Cain checked the living room, bathroom, kitchen, and closets. The apartment has clothes on the couch and bed. Mr. Smith bathroom was okay and kitchen was organized. Wellness Coach Ms. Cain informed Assistant Program Director Ms. Gore that a Wellness check was conducted for Mr. Smith residence however, he was not home. Wellness Coach Ms. Cain informed APD Ms. Gore that the door was close and unlock and Ms. Gore replied that she was going to lock the door. Wellness Coach Ms. Cain said okay.

I spoke with Ms. Valerie on July 31, 2017, at 6:58 p.m over the phone. Ms. Valerie stated that the client Bryce Linder verbalized that an advocate Leslie McKeown slapped him in the face. Then later she stated that the advocate Catina Hampton slapped him on the left side of his face during a group session, held at the HCAP office Located at 4120 Directors Row Suite D. Ms. Valerie stated that Bryce informed her that Catina verbalized to the client, “I have to slap you because you were being disrespectful during group.” Ms. Valerie also mentioned that Catina filed a CPS report on the family because Bryce stated that his former therapist molested him in the past and no one reported the incident. The advocate went out of town for a family emergency the week of July 24 –July 30. Ms. Valerie stated that Catina Hampton asked her if it was ok to take care of her two dogs during the time that she was out. Ms. Valerie stated that one of the dogs died from overheating and the other dog was stuck under the house.

When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. He denied pushing or grabbing Andrew. James Boston state he was in the back area of the office and didn't see the incident, however, he did hear Andrew using profanity. Staff member Boston stated Andrew can minimize his behavior and stretch the truth. Consequently the RP contacted the group home administrator Robert Carter at (661) 900-6342 and informed him what was being reported by Andrew. Mr. Carter disclosed there was video footage and the situation was being investigated. In addition he stated the language Mr. Wilcox should have used was guided not escorted out of the office. When worker arrived Ms. Darty was in a hospital, bed located in the living room and did not respond to worker.
