On 04/27/16 law enforcement was called to the home and Lacee was arrested for domestic violence on her brother. The reporter stated that the family has history with DHS and Kevin has been in DHS custody in the past because of his sister’s violence. The reporter stated Sharon regained custody of the child because it was reported that Lacee was no longer in the home. The reporter’s concern is for Kevin’s safety now that Lacee is back in the home and the child’s mother is unable to protect the child from Lacee. Ms. Castle stated both Kevin and Sharon are afraid of Lacee and Lacee is allowed to run rampet in the home. 7/20 at 8:45am worker received a phone call from Todd, Crystal's probation officer. Todd stated that the surveillance officer received information that Crystal is still associating with drug court clients. Todd stated that the surveillance officer went out to the house and Crystal answered. Crystal needed to go pick someone up and would return home. At 11 pm there was no answer at Crystals door. Tood just received an email about Crystal engaging in several relationships with drug court men. Todd wanted to know how important the ACR was that day due to he was going to request an emergency court setup that afternoon. On 11/4/16, FFA social worker (SW) Maria Morales Munoz visited CFP Gina Medina. On her way out, Ms. Medina accompanied SW to the gate. A young girl was passing by and SW pointed out that she reminded her of a foster child Ms. Median used to have in placement (Juliana Marmolejo). Ms. Medina informed SW that Juliana was again attending Bakersfield High School which is the same school that Ms. Medina’s bio daughter (Raymie) and the Ms. Medina’s other foster children attends. Phillip Wilson states his mother called him citing Tammy Sayre, Christina Smith’s mother called her and asked her to go over to Christina Smith’s home and check on her. Phillip Wilson states once his mother arrived at Christina’s home, that Christina was unresponsive and that she needed, him, Phillip Wilson, to come over and kick in the door of the home. Phillip states he did not go over because EMT was present and they had found an open window and made entry into the home. Phillip states that night his mother cared for his daughter, Kylie and his mother called James Smith to come and get his two minor children, Lani and Joshua. Phillip Smith states the day that Christina got released from Spartanburg Regional Medical Center James Smith gave Joshua and Lani Smith back to her. Throughout the course of 6/22/15- 6/27/ Igoe REACH has come into the home of Jessica to observe her behavior. The behaviors, which Jessica engages in, place her in harm of herself and her peers. Monday she engaged in elopement, which then led to a sit down strike in the middle of the road. The incident required two staff to physically escort her in the home while trying to deescalate the behavior, which included screaming and kicking staff, was punched in the face another staff was also forced in a ditch that was filled with mud. All these incidents happened while in the presence of her support coordinator for Prince William County. Collin another in home counselor came to observe Jessica. I made contact with him on his second time with Jessica. Collin was unaware of Jessica normal behavior so staff shared with him that he should, “observe” her getting of the bus. Immediately entering the house she started to engage in behaviors that led staff to verbal redirection. This took place for about 45 minutes until the AD for Community Concepts (Regina Goldston) advised him to back away and observe from a distance. It has been noted that Jessica will engage in more behavior with an increase presence of staff. Collin spoke with the home manager about the normal behaviors. Holly stated that Chad SR called to inform her she has been barred from visiting at the prison due to her wardrobe changes from prior visit attempts. she has been barred from visitation until August. Holly was informed that she could write a letter to the prison asking for the bar to be revoked. Quoturria and her three children have not had a permanent place to stay since school started for the children. The family lives from home to home and Quoturria will stay with anyone that will allow her and her children to stay. The family has lived in a home with a known pedophile and with the perpetrator’s brother in the projects; the brother’s home has gone without lights and gas. On this date, Ms. Lona Williams and Mrs. Belinda Lamb visited Walker Co. DHR, for the purpose of giving information to worker. Ms. Williams stated she was currently staying at her sister, Kathy Tisdale's home. She left her home after her daughter Melissa and her husband were verbal abusive. Her daughter, Belinda Lamb, stated she was taking her mother to the police station to file evection notice. 12/28/2017: Crisis Session was conducted in Spanish. Dayana and her family arrived to NJCC for a crisis session. Dayana was oriented x3 and reported that she has been very stressed with everything going on with Stephanie. Dayana is the mother of Stephanie who is a survivor of a sexual assault. The incident took place about 2 weeks ago. Dayana informed this CIS that the family is going through a very hard time after the incident and they are all currently sleeping in the same room because Stephanie cannot sleep alone.

On the event date above, CP attempted to conduct a schedule home visits to Selena Goodall residence. The CP was advised by Selena adoptive brother that she was not home.
