The Coxswain qualification is one of the hardest fought for qualification of a Boatswain mates’ career. For a Reservist to obtain this qualification it is even harder. The Coast Guard has tried to ease this by allowing Reservists to attend Coxswain “C” school. However, while they are attending the school, another policy is quietly wading in the waters for the perfect time to strike. This is the policy that states that the hours these members earn while underway on a standard Coast Guard boat, while at school, do not count towards their currency maintenance hours for the crewman qualification that they are operating under while at the school. In District 9, most of these members end up losing their certifications after returning to their units because there is not enough time left in the boating season to obtain the hours they need to retain their qualification. This is causing a time consuming process to recertify, it is demoralizing to the member, and it is a waste of Coast Guard money. If the hours counted while at Coxswain “C” school for currency maintenance requirements, the school would be more beneficial for the member and the member would be more beneficial to the Coast Guard. Within a few months of reporting onboard, he completed seven qualifications well ahead of schedule including earning his Enlisted Information Dominance Warfare Specialist qualification. His motivation to complete Personnel Qualification Standards outside of his rate resulted in improved watch bill flexibility for the command and increased job knowledge.
Recent changes to the High Year Tenure (HYT) waiver process timeline at the E6-E8 level for fiscal year 2016 are too short. This change has caused extremely compressed timelines for members to make the necessary preparations after they have been identified for mandatory retirement. It has also led to uncertainty months into the assignment year for units with HYT candidates. The Coast Guard needs to expand the E6-E8 HYT waiver timeline to align with the E3-E5 HYT waiver timeline.

1. The ramifications of the Coast Guards' High Year Tenure (HYT) program have caused essential enlisted members to seek new employment, not by choice of their own. Bench marks set by the program has shortened the careers of both junior and senior enlisted members. Because of this Aviation rates are top and bottom heavy with a lack of middle management, over 300 short at the E4/E5 level to be specific. High Year Tenure benchmarks known as Professional Growth Points (PGP) need to be updated.
A continuing care assistant are individuals who assist clients like the elderly and the mentally ill by providing them with the support services and the personal care that they need while promoting a healthy and independent living place in their own homes and long-term care facilities. CCA’s often work in long-term care facilities because, at a long-term care facility, they provide clients/residents with their managing of their medications, the residents have 24 hour supervision, assisted meal service, and personal care and/or nursing. CCA’s have a lot to offer to the nursing homes. They offer personal care and palliative care, assistance with meals, and also with household management.
Most of the junior sailors come from backgrounds where education and professionalism is missing due to unstable financial stability, lack of parent involvement or just trying to make money the easy way. We as Navy leaders, have the task to assist those sailors to achieve their goals. First of all, we need to continue training junior sailors. Training is a crucial tool for them to become technical experts and subject matter experts. It is in our hands to make sure that all juniors’ sailors are well-trained, mentally, professionally, and personally. They have an entire tool box of resources, programs and opportunity that many of us did not have during our early years in the Navy. It is our job to show them where they can find them and how to use them. Also, we have to keep in mind that we are not only train sailors, we are training our replace; we need to train leaders. Education is another essential piece that we need to keep advertising, especially when they can complete a degree at no cost. But to be successful and guide in the right direction, we need to know them first. If we take time to know them, we can determine what are they are looking for. I believe that no major changes need to be made because the opportunities and programs are out there. The challenge will be to engage them and keep them on

You are working in the internal medicine clinic of a large teaching hospital. Today your first patient is 70-year-old J.M, a man who has been coming to the clinic for several years for management of CAD and HTN. A cardiac catheterization done a year ago showed 50% stenosis of the circumflex coronary artery. He has had episodes of dizziness for the past 6 months and orthostatic hypotension, shoulder discomfort, and decreased exercise tolerance for the past 2 months. On his last clinic visit 3 weeks ago, a CXR showed cardiomegaly and a 12-lead ECG showed sinus tachycardia with left bundle branch block. You review his morning blood work and initial assessment.
Clinical practice guidelines (CPG) are designed to improve the quality of healthcare services, decrease unwanted, ineffective and harmful interventions for patients. CPG are used to facilitate treatments for each individual patient’s by maximizing the benefits, minimizing the risk of harm and obtain treatment with an acceptable cost. Researchers had proven that CPG is a bridge for change and improving health outcomes. The effectiveness of CPG is perceived to be helpful in clinical decision making. CPG are developed to assist healthcare providers such as doctors and nurses in decision making for specific clinical outcomes (Vlayen, et. al. 2005)
