Population
The population for this evidence based project are African Americans diagnosed with prediabetes at a local primary care facility in Tallahassee, Florida. The intended age group is for this evidence based practice change project are patients from the ages of 18- 60 years of age. Participants must be English speaking African American males or females between the ages of 18-60 years of age old and clinically diagnosed with pre-diabetes with an a1c of 5.7 % to 6.4%.
Intervention
The planned intervention for this scholarly EBP change project was to implement a culturally sensitive diabetes education program. Information that was part of the culturally sensitive diabetes education program included educational materials on type 2 diabetes, training guides,

However, the participants in this EBP change project will be given a pre-and posttest to evaluate the efficacy of a culturally sensitive education program regarding increasing healthy lifestyle behaviors regarding nutrition, exercise, and knowledge regarding Type 2 Diabetes.
Outcome
The intended outcome of the EBP practice change project is that participants will exhibit an increase in diabetes knowledge at the end of the 6-week course. An increase in diabetes knowledge would be confirmed by participants exhibiting higher posttest intervention assessment scores.
Summary
In the US type 2 diabetes is a major medical problem affecting all ethnic backgrounds and needs to be addressed. The pre-diabetic and diabetic population is steadily increasing every year despite medical advancements. Unfortunately, the African American population is at a disadvantage when it comes to diabetes. Preventing medical complications related to diabetes, initiating health promotion programs, increasing healthy lifestyle behaviors will essentially lead to a decrease risk of complications secondary to diabetes and overall improve patient
The incidences of diabetes are growing annually, and predicted to increase within the coming decades. The number of cases yearly is not only increasing, but the age of onset for diabetes has decreased. This means more individuals are developing diabetes at an earlier age. Diabetes not only affects individuals and communities but countries as well. The prevalence of diabetes has gathered billions of dollars in healthcare costs. [1]Type 2 diabetes also leads to other serious medical conditions such as heart disease, renal failure, blindness, and tissue damage in extremities that will eventually lead to amputation. These illnesses, as a result, result in further costs in the form of disability and the loss of a portion of our workforce. The health disparities among the Blacks/African American is on the rise which is of being mostly affected by diabetes the United States and Maryland. Diabetes and cardiovascular diseases are connected which can lead to increase mortality among this population. Diabetes is a serious health condition that is a chronic illness for the African Americans. Diabetes preventions strategies in African American community can be a tricky task to contain and prevent for several reasons I will discuss in this paper.
The rate of diabetes in the United States is one of the highest compared to other developed countries. An estimate of 9.3% of the population have diabetes, of those with diabetes 27.8% have yet to be diagnosed (Centers for Disease Control and Prevention [CDC], 2014). This means that approximately 8.1 million people are currently living with diabetes, but are unaware of it. As of 2012, 12.3% of people with diabetes were 20 years old or older, the largest population diagnosed with diabetes were adults 65 years old or older. 25.9% of this population lives with diabetes (CDC, 2014). On a national level, the CDC have launched initiatives that focus on prevention and disease management. The National Diabetes Prevention Program is an example of one such initiative. According to the American Diabetes Association, more Americans die each year from diabetes than from AIDS and breast cancer combined. As a result, researchers have extensively studied the causes, treatments, and interventions for diabetes. Despite efforts to ameliorate its effects, diabetes remains a prevalent danger in society. In 2014, 7% of U.S. adults were living with diagnosed diabetes (Centers for Disease Control and Prevention [CDC], 2016). In Louisiana that number was even higher - 10.4% of adults have been diagnosed with diabetes. Breaking it down by age group, however, in Louisiana 3% of people aged 18 and 44 have been diagnosed, and 15.2% of people 45-64. (Centers for Disease Control and Prevention [CDC], 2015a). Several studies have predicted future rates of diabetes both in the United States and worldwide - nearly all of these studies reached a similar conclusion: rates of diabetes will continue to rise (Boyle et al., 2001).

The EBP change project was able to increase diabetes knowledge among African Americans with prediabetes by discussing risk factors associated with type 2 diabetes, proper nutrition and promoting health promotion behaviors. Currently, there is no diabetes education program or policy that’s in place targeting patients with prediabetes at the EBP change project site. Therefore, it is the project manager goal that the EBP change project site will begin to implement a diabetes education program focusing on the prediabetic patient population.
Diabetes is a prevalent health disparity among the Latino population. Diabetes is listed as the fifth leading cause of death among the Latino population in the website for Center for Disease Control and Prevention, CDC, in 2009. According to McBean, “the 2001 prevalence among Hispanics was significantly higher than among blacks.” (2317) In other words among the Hispanic or Latino community, there is a higher occurrence of diabetes as compared to other racial/ethnic groups such as Blacks and Native Americans. The prevalence of diabetes among Latinos is attributed to the social determinants of health such as low socioeconomic status and level of education. The Hispanic population is constantly growing and we have an estimated 13.7% of Hispanics making up the United States. “The annual percentage of patients with prediabetes in whom overt type 2 diabetes develops is about 5% in the general US population and may reach 15% in the Hispanic American population” (Idrogo & Mazze, 2004, para. 7). This a community health issue because of the percentage of Hispanic individuals that may end up with diabetes. These individuals need to be educated to help the promotion, protection, and maintenance of diabetes in this group. That is why I have created the intervention program for this ethnic group.
There are nearly 30 million people living with diabetes in the U.S. African Americans are disproportionately affected by diabetes, with 13.2% having been diagnosed with diabetes, compared to 7.6% of non-Hispanic whites. They are also at an increased risk for serious complications of diabetes, which include blindness, kidney disease and amputations. Although the risk for complications can’t be eliminated, good control of diabetes has been shown to reduce those risks.

Type 2 diabetes is a chronic disease where patient education is imperative and requires education that surpasses the primary care office. According to Cha et al. (2012) basic education and family involvement is an essential component for pre-diabetic and diabetic population to achieve glycemic control. Time management and time constraint are obstacles that are affecting the primary care provider in initiating health promotion topics and disease prevention information (Kowinsky, Greenhouse, Zombek, Rader & Reidy, 2009). Recognizing the time constraint at the EBP change project site for health promotion and patient education a culturally sensitive educational program would be created to promote healthy lifestyle behaviors targeting African Americans with prediabetes.
My teaching on primary prevention of type II diabetes adheres to HP2020 objectives D-1 to D-16 on diabetes, specifically objective D-1 that deals with reducing the annual number of new cases of diagnosed diabetes in the population. (Target is a 10% improvement from the baseline of 8.0 new cases of diabetes per 1,000 population aged 18 to 84 years occurred in the past 12 months to the target of 7.2 new cases per 1,000 population aged 18 to 84 years). Education on the preventive measures needed to be taken by an at-risk population tackles this issue of reducing the annual number of cases.
Carter, Barba, and Kautz (2013) report culturally tailored education can lead to significant improvements in self-care in African Americans with type 2 diabetes. Also, appropriate dieting, exercise, medical checkups and medication regimens for prevention and management show efficient improvements. The Health Belief Model is realistic and acknowledges that having a desire to change health behavior may not be enough. Therefore, two elements that may be helpful include cues to action and self-efficacy. The cues to action are external whereas, self-efficacy focuses on his or her ability to make a health-related change (Boskey, 2014).
The review of literature section will define diabetes and prediabetes, identify the prevalence and major risk factors for diabetes, and present behavioral changes that can reduced the risk of developing diabetes. This section discusses the types of presentations and teaching methods that have been utilized in diabetes prevention and the health belief model. The final paragraphs in the review of literature present the learning theory and delivery method of instructional designs, which are ideal for a diabetes prevention program.
According to the Centers for Disease Control and Prevention's National Center for Health Statistics, "[By 1993] death certificates listed diabetes as the fifth leading cause of death for Blacks aged 45 to 64, and the third leading cause of death for those aged 65 and older in 1990.” (Bailey, 2007). These statistics show how serious the problem of diabetes has become in the black community. Epidemiological studies can focus the efforts of the healthcare community to effective interventions aimed at lowering the prevalence and incidence of diabetes among African Americans.
