After completing the windshield assessment, it is apparent that the community seems to be thriving more in the last 5 years. Newer establishments have been made and more are expected to come. There is a need for more law enforcement. The building and officers are there, but they spend most of their time at the jail instead of out patrolling. There is also a need for more work to be done to the lake to help the community thrive more. There is a need for more services to assist the older population with staying functional in their homes. The need for these services is seen by the separation of run down homes and newer homes in the community. Many of the elderly are living in rundown homes and are not near the grocery stores. The nurse would be responsible for identifying people in the community that are at risk for poor health management. After the nurse has identified a person at risk for, the nurse then needs to identify what the particular needs of that individual are. Once the nurse can identify specific needs that an individual has, then the nurse can initiate a referral to different programs to meet an individual’s needs. For example, an individual who struggles with making meals because of arthritis may need a referral to the ROCS office to participate in the meals on wheels program. Another example includes, an elderly woman who is having troubles with performing household chores. The general condition of the homes in Rockport is diverse. Many of the homes are multi-million dollar structures while others are dilapidated and in need of serious repair.
