A personal conflict that I have recently encountered was among one of my roommates and the rest of our household. Currently I live with four other individuals. We all have our own bedrooms and our own space so it is possible that we have our own places to go when our living situation tends to be a little too much around the house. Now our schedules are all different, so we are not all home at the same time, which can be both a positive thing and a negative thing. Now, one of our favorite restaurants to venture to is a local traditional sushi and hibachi restaurant in the area. One of our roommates, Travis, he is especially fond of this restaurant. I chose this type of critical analysis tool by reason of, it will provide me with solutions to all realms of this conflict and why the end result of our night was the way it was. Root cause analysis is helpful considering it is going to let us find the reason for why this problem occurred in the first place. According to our reading assignments for this week, RCA allows us to look at three different types of causes that could stimulate this type of response. The three types of causes are physical, human, and organizational causes. The first step in the RCA process is defining the problem. The conflict was determined that we chose to dine out for dinner at Travis’ favorite restaurant, without Travis. The second step is finding proof that this conflict exists in actuality. The evidence found was that Travis locked himself in his room, avoiding all contact with his roommates. There was a sequence of events that happened earlier in the afternoon that led to Travis ultimately being upset with us. Earlier that day he received a phone call from his cousins, in which they told him they would not be waiting for him to get off work in order to celebrate what they call, “cousins day.” This was extremely important to Travis, so to feel already rejected by his family, he then felt even more so rejected by his friends at home. Another one of our roommates was already aware of the “bad day” Travis was experiencing. So he thought that maybe it would have been given more consideration, because someone was aware of his unfortunate situation with his family members. In consummation, Travis felt abandoned by the most influential people in his life. While it may appear to not be exceptionally important and frivolous, it had true significance to Travis. By searching for other clues as to why this problem occurred, we were able to work backwards from a puzzle, if you will. We had a matter of contention and in order to find resolutions we had to dig a little deeper. The fourth step in understanding why there was a conflict of interest between our roommates and Travis is identifying the root issue at hand. What really caused Travis to be upset with everyone. The CM and CNS discussed the lack of findings from the common cause analysis and the goal to focus on fetal safety and the L&D caregiver stakeholders. Subsequently, they decided to involve the 4 L&D Shift Specialty Coordinators (SSCs) who supervise the shifts and work alongside staff nurses when unit status requires additional support. Since they are present on the unit during all shifts, the SSCs were able to contribute valuable input from the front-line caregivers including nurses, residents, and providers. The CM, CNS, and SSCs thought through the unit workflow and deliberated about potential issues with unit organization, processes, and dynamics to identify factors that might be detracting from fetal safety on the unit.

I then went to Alex’s in the middle of the afternoon to help him set up for the night. We moved couches, and set up the basement for optimal viewing. This restaurant is semi-classy, and I couldn’t help but feel like I had to be on my best behavior because the people there acted almost prestigious. My dad and I were dressed decently, but Jones was bumming it hard core for this restaurant. When we sat down Jones was his normal loud self, making every story he had come to life by being over dramatic, which always makes me feel like sinking down in my seat to hide, because there were always stares. When the drinks came Jones somewhat mellowed out, but then after we ordered, and received our food I was surprised to hear a huge laugh from across the table, and “are you freaking kidding me”, come out of Jones’s mouth. He had ordered surf and turf, and apparently the shrimp were “too small” for his liking. He demanded more, or a completely different dish because, “how do you call this surf and turf? I would call it just turf.” I looked to my dad and felt my face turn red. I could feel all eyes on our table and could see the embarrassment in the waitress’s eyes. When he said this, the waitress hurried, went in the back, and brought out the chef to our table. The chef apologized, and brought him more shrimp to compensate for the “small” ones. I then thought to myself, is Jones going to do this every time we go

The initial root cause analysis was completed in assignment millstone number two. It identified multiple problematic symptoms and ultimate causative factors. Therefore, a rational approach to millstone three, solutions and outcome will be driven by the causative factors. These factors are; ineffective communication, poor or unhealthy interpersonal relationships, lack of trust and respect, and deflated self-esteem and moral.
I agree, when a company’s reputation is damaged, the first step back to respectability must be intense self examination and reflection (Zeleny, 2008). The company must identify what aspects of their business practices and/or organizational culture enabled their fall from grace. Sadly, after going this process of root cause analysis, far too many times, corporations identify they simply strayed away from the organizational values and core principles they originally established at the inception of their company. Once they’ve identify the root causes of their issues, it’s critical that everyone in the organization, shareholders, executives, and employees are aligned, committed, and involved in the process of restoring the company’s reputation

However, in its most literal sense root cause analysis requires the performer to systematically break down a situation into individual components or processes in a search for truth that can be supported by facts (Eckhardt, 2007). I stepped out of the hotel and walked to the rental car with my older brother Sean. Me and my family are in California for summer vacation. We’ve never been to California, so it’s a nice break from Washington. I’ve only left Washington two other times, the first time was my Aunt’s wedding in Georgia. The second time was when we took my brother to Boston University. Usually, my family comes to Washington to see us, so it was weird to leave.My parents were already in the car when me and Sean got there. We were going to see my Uncle and his new wife and kids for dinner. Their house was about an hour from the hotel. My cousin, Scott, moved in with us a couple of years ago. I was very close to him, and I trusted him completely. He was always there to talk to whenever I was dealing with something, or just bored out of my mind. He listened to me rant about all the drama going on in my life, and would always give me advice on how to deal with it. My brothers and I loved doing things with him – he was truly genuine. He was loving, kind, thoughtful, and he always put others first. He got on his feet pretty quickly, and he had gotten a job not long after he moved in. Everything seemed perfectly fine. That is, until he started blaming my brothers and I for the voices in his head.

Lawson grew up about 50 yards from my house. He started coming to my house once his mother started dating Phil; Phil was addicted to drugs.
