The American people deserve quality, effective healthcare. Unfortunately, those who can afford healthcare have been paying more and more every year. The Center for Medicare Services (CMS) has developed a plan to incentivize change in the healthcare delivery system. Triple Aim focuses on simultaneously improving the patient experience, improving the health of populations, and reducing the per capita cost of health care (Berwick, Nolan, & Whittington, 2008). Triple Aim affects all areas of the healthcare delivery system; this paper will focus on how hospitals are improving care, encouraging healthier lifestyles, and reducing costs.

In the article written by Ford, (2009), the doctorate of nursing practice (DNP) has an important role to play in the current health care system which is constantly changing. As healthcare around the world is transforming, the Institute of Medicine (IOM) 2011 report mentioned that in the United States, Advanced practice nurses play a vital role in the rapid complex changes based on the full extent of their education and training. This article describes some of the positive role of the advance practice Nurse as a facilitator in the regulatory and policy developments. The Advance practice nurse participates and provide cost-effective care models that will improve the access and quality of care of the population. Another positive area for the APN in this current healthcare system is health promotion, health maintenance, and prevention. In this evolving health care system, it gives the Advance practice nurse the opportunity to set up acute and primary care settings that will positively impact health care delivery as they meet the needs of an expanding, aging, and chronically ill population. As healthcare needs are steadily on the rise, there is also an increasing demand for primary care physicians. According to the American National Association (2011), “an estimated 66 million Americans face threats to their health because of limited access to front-lines primary care.” There has been an increasing focus on utilizing the role of advanced practice providers like clinical nurse specialists, certified nurse midwives, certified registered nurse anesthetists and advance practice registered nurses to compensate for these demands. These forms of providers are master’s-prepared nurses who are have specific certifications and licensures to assess and treat patients independently or under the supervision of a physician. Hospitals implement HCAHPS with the support of the Hospital Quality Alliance (HQA), a public or private partnership that includes key hospital and medical associations, consumer groups, measurement and accrediting bodies and government agencies that have the same interest in improving the quality of hospitals. The Hospital Quality Alliance (HQA) program that is overseen by and public and private entities, that include the Centers for Medicare and Medicaid Services (CMS) as well as the Joint Commission, is dominating this effort in the hospital district, generating reports quarterly on the delivery of effective services for mutual conditions. Even though the Hospital Quality Alliance has made this data more available to the public, there has not been enough information on the quality of hospital care from a patients ' point of view. As the Institute of Medicine shows, the foundation of patient centered care is a key factor to having a premium health care system. The HQA backs HCAHPS.

As the young and rapidly-aging population continues to increase, the demands of primary, acute and chronic disease management will also increase. As a result, more health care professionals who provide primary care will be needed to meet these demands. Thus, the emergence of Advanced Practice Registered Nurse (APRN) evolve. APRN is a nurse who has completed a graduate degree and has acquired advanced knowledge and skills. APRNs are grounded with theory, concepts and principles that enable them to assess, diagnose, treat and manage their patients. APRNs can work in conjunction with other health care professionals or independently. APRNs improve access to health care by providing care in the rural and underserved areas. APRNs also reduce the cost to health care (Joel, 2013).

The last key component discussed in this paper as noted in the IOM report states “Nurses should practice at the full extent of their education and training” (Institute of Medicine 2010, pg. #). Because of the goals and recommendations with in the IOM report nurses are going to have to change the way they currently practice health care. At this time there are many different laws and regulations from state to state that are barriers for nurses. The report acknowledges the importance of removing these barriers to allow nurses to perform at their fullest educational competency. Advanced Practice Registered Nurses (APRN) continue to have a very restrictive state scope of practice therefore inhibiting their ability to contribute to the health care delivery system. Whether an APRN is a Primary Care Provider (PCP), or a RN is a patient advocate and bedside care provider they are all important roles in the nursing profession in a primary care setting (Institute of Medicine 2010).

Middlefield Hospital is in an urban area where patients should have access to the best quality healthcare possible. Middlefield needs to offer their patients a triad of care, which includes access, cost and quality of care. There are many factors why a patient may not be able to access care, such as lack of health insurance or financial resources, therefore Middlefield needs to ensure the population that we are affordable and the patient will receive the best possible care regardless of their financial situation. Cost refers to the total expenditures related directly and indirectly to the provision of healthcare. The role of the Advanced Practice Nurse (APN) is expanding internationally throughout the healthcare system. Since the initiation of the Patient Protection and Affordable Care Act in 2010, there has been an increased need for APNs due to the growing demand for primary care services and increased population that have gained healthcare coverage (Lanthrop & Hodnicki, 2014).
