The internal database has been selected as MS-SQL server 200.The basic constructs of table spaces, clusters and indexes have been exploited to provide higher consistency and reliability for the data storage. The MS-SQL server 200 was a choice as it provides the constructs of high-level reliability and security. The total front end was dominated using the ASP.Net technologies. At all proper levels high care was taken to check that the system manages the data consistency with proper business rules or validations. The database connectivity was planned using the latest “SQL Connection” technology provided by Microsoft Corporation. The authentication and authorization was crosschecked at all the relevant stages. The user level accessibility has been restricted into two zones namely.

* Cannot Upload and Download the latest updates. * No use of Web Services and Remoting. * Risk of mismanagement of data when the project is under development. * Less Security. * No proper coordination between different Applications and Users. * Fewer Users - Friendly.
1.5. SOLUTION OF THESE PROBLEMS

A database administration framework (DBMS) is a PC programming application that connects with the client, different applications, and the database itself to catch and dissect information. A universally useful DBMS is intended to permit the definition, creation, questioning, upgrade, and organization of databases.
Recent reforms in health care, such as the passage of measures mandating electronic health care records, make unified public health information systems an increasingly viable option for providers, insurance companies, and care facilities. An examination of the experience of the Missouri Department of Health indicates that the construction of such a system based on information engineering has a number of benefits but only if properly implemented with prudent foresight. Some of the most valuable lessons learned that can assist other health care providers with the implementation of a similar system include protecting client confidentiality, procuring funding, sharing information with other agencies, gaining executive sponsorship, and allocating a sufficient amount of time and resources to such an undertaking.
Access Hospital Dayton provides mental health services and substance abuse/detoxification services. Access Hospital Dayton offers both inpatient and outpatient treatment. The services that are provided are for adults only. I currently work in an inpatient hospital setting but we do not accept patients that have substance abuse or detoxing. I am very interested in addictions and like to broaden my experience within that field.

New applications will utilize off-the-shelf software components that have been customized per Riordan’s specifications and further messaged to ensure that each application will integrate smoothly with all the others in order to create a single cohesive whole. Great effort will be made to ensure that the data structures used in each are consistent in order to simplify the creation of the enterprise’s database. To help facilitate this, we will create an umbrella application that will integrate each other system as a module. This umbrella application will be extendable as needed and will act as a single-launch point for the various systems utilized by Riordan. We will also be working closely with Riordan’s IT department to develop a bridge that will enable them to easily port their existing databases into the new one automatically.

If case management services were used across all hospital and emergency room settings, instead of in only the larger hospital setting, the cost associated with super utilizers would be able to be significantly decreased. If the gentleman I discussed had been able to work with a case manager, it is possible that they would have been able to recognize that the reason he was utilizing the ER instead of a regular physician clinic was because he didn’t have reliable transportation or anyone to bring him to the office. They may have realized that his home environment was less than satisfactory and that his home was falling down around him. If assessed early enough; and referrals had been made to programs such as the CHOICES program, early intervention may have changed the outcome of his situation. Most of the time, the emergency department doctors and nurses are too busy to do the outreach that needs to be done. They likely looked at him as a frequent flyer or a “crazy” old man. I recommended the “HUD 242 Loan Insurance Program for funding EHC’s expansion project. I felt as though this was the best option for EHC. The Net Present Value (NPV) for the project is $221 million when funded through HUD 242 Loan Insurance Program. This is higher than the NPV for the other two options. “One reason being, HUD 242 Loan Insurance Program enables hospitals to have their debt financed as an investment grade, either AA or AAA, which provides the lowest borrowing rates available in the capital markets(Freund, 2007)p.g.53.” Another advantage with these bonds is that they are callable after eight years (Freund, 2007). If the interest rates go down, this will be profitable for EHC to buy back the bonds and reissue the debt at a lower cost. EHC can profit from the fall in interest rates, if it were to occur within the first eight years. Last, there are no deadlines for using the funds unlike Tax-Exempt Revenue Bonds and they do not have foreclosure fees as in Private Bank Funding (Tobin, 2007).

The LOS is used as a guide of performance and is an element of costs as well as resource allocations. Health coverage has been a difficult issue since there is no real health care system that covers all citizens providing them with a comprehensive health care that includes prevention, promotion and preservation of solid state of health and wellness. Treatment of illness and injuries and disabilities, is more expensive in aspects such as emotional, economical and financial affecting not just the individual involved but also their families, jobs and societies. Once a person becomes temporally or permanently ill, they become unproductive for the society generating high costs for themselves, their families and the system in order to recuperate their health or maintain their current disability. Health is the treatment and prevention of disease (Holland, 2008).
People have always been in search of wellness. Hundreds of years ago, this search was easier said than done, nonetheless, humans were continuously looking for a way to extend mortality. Until the early 1900s, there was not the plethora of institutions and hospitals that are so accessible to citizens today. Even though hospitals took root in the communities across the globe, there has been a recently wane of hospital usage in the past few decades. This shift in hospital use is due to a myriad of reasons such as cost of healthcare and the rapid development of technology, all accompanied by certain advantages and disadvantages.
