Reyna remained in MHS Beaton household from 07/01/2015 to 07/31/2915. The youth attends weekly individual therapy and bi-weekly family visits. She has weekly BMod visits that assist with healthy living and behaviors. Reyna has improved her communication skills but struggles at times expressing her feelings. She completes weekly independent skills independently. Reyna volunteers at the summer enrichment program providing assistance with younger participants. Intervention: CSP, MHS and Shamerica met in the community for visit to observe Shamerica’s behaviors. CSP and Shamerica discussed being at home for the long holiday and at home with MHS. CSP, MHS and Shamerica reviewed coping skills that assist with the youth’s anger and sadness missing her aunt. CSP and MHS discussed Shamerica’s display of aggression before Christmas. CSP redirected Shamerica’s thought process that MHS should not monitor her with technology. CSP encouraged the youth to express her feelings and learn to trust MHS.

Stephanie was referred to FACT for CMO services by Trinitas Hospital – CCIS unit. Stephanie was admitted to CCIS on 4/18/17 for running away from her mother’s home and making threats to hurt self if she were to return to reside with her mother. While in the hospital, Stephanie has also been refusing to visit with mother. At the time of the FCP meeting, Stephanie (Youth) resides with her Natalia Ramos (Mother), Elsa Sanchez (Maternal grandmother) and Johnathan Sanchez (Uncle) who disabled. Mother and stepfather recently divorced. Her father Hamilton Ramirez (biological Father) lives in Elizabeth with his girlfriend and younger daughter. Each biological parent has children with other people. Parents do not get along at all. Youth wants to live with father.

QP explained to Shonquasia, how negative thinking can increase depression mood. QP asked Shonquasia to list some negative thinking that keeps her in a depress mood. QP provided Shonquasia and her mom information on summer camp and summer activities. QP discussed with mom and Shonquasia the approaching of the end of IIH services. Nevaeh was open and responsive the first 10 minutes of the counseling session. After that, Individual answered most questions asked by the clinician with an "I don't know" or shrugging her shoulders. However, Individual insisted she was feeling good and that everything was fine. Regarding the school, Ind stated that she was getting along better with her classmates. Individual self disclosed that the boy who didn't want to talk to her last semester, is now talking to her. Ind stated that it's fine with her if he now wants to be her friend. Nevaeh showed more interest when the Clinician turned in all the paper work she did during summer time. She satisfied and stated she had a good time during the camp. Silently and with curiosity, Ind engaged
CM spoke to Kiara Gelin (youth) regarding a follow-up on services. Youth reported she continues to attend the Partial Hospitalization program at the Jersey City Medical Center. CM was informed that youth stopped attending the TASC program due to time conflict and youth will resume the program in September. CM and youth disused employment; youth had a job interview at Wendy’s, Fast food restaurant. CM encouraged youth to apply at local jobs in the mall, CM verbally provided youth with a list of jobs that are hiring in the community. CM inquired about the Cook, Eat and Talk program. Youth reported the family will follow-up with the program once the family phone is working. CM agreed. CM and youth discussed mentor
Deonte remained in the MHS Pringle home from 07/012015 to 07/31/2015. The youth continues to learn and practice positive ways to communicate with adults and peers. Deonte completed summer camp and is currently enrolled in karate. The youth had thrived in the current placement demonstrating comprehension of interventions. Deonte remains active and respectful in social environments. Deonte had no family visit during this period. The youth shared he is ready for the new school year by practicing his Reading, Math and Science.

Intervention: CSP and Reighn discussed ways the youth is expressing her feelings at home and school. CSP assisted Reighn in processing her thoughts about upcoming court date and adjusting to having another youth in the home. CSP addressed MHS concerns the youth behaviors are regressing.
