Deonte remained in the MHS Pringle home from 07/012015 to 07/31/2015. The youth continues to learn and practice positive ways to communicate with adults and peers. Deonte completed summer camp and is currently enrolled in karate. The youth had thrived in the current placement demonstrating comprehension of interventions. Deonte remains active and respectful in social environments. Deonte had no family visit during this period. The youth shared he is ready for the new school year by practicing his Reading, Math and Science. Clinician role-plays with the youth to demonstrate age appropriate interventions. Adolescence is a critical time of development. During this period there are significant changes in brain development, emotions, cognition, behavior, and personal relationships. It is during this time that most major mental health disorders appear, many of which carry over into adulthood. Behavior patterns such as substance abuse also often develop during this time and may continue throughout adulthood. Many adolescents struggling with mental health issues begin to exhibit symptoms such as acting out at home or in school, showing a decreased interest in activities that they previously enjoyed, or bringing home poor grades. In Boyle Heights, East Los Angeles, Father Greg Boyle organized a job and resource center for gang youth (Rodriguez, 2005), called Homeboy Industries. Homeboy Industries, a nonprofit organization, has received international attention for its bakeries, cafes, tattoo removal, schooling, job referrals and counseling (Rodriguez, 2005). The organization also provides hope, training, and support to formerly gang-involved, and/or previously incarcerated, men and women. Full-time employment is offered for more than 200 men and women at a time through an 18-month program, allowing them to redirect their lives and become contributing members of the community (Homeboy Industries, 2005).

Reyna remained in MHS Beaton household from 07/01/2015 to 07/31/2915. The youth attends weekly individual therapy and bi-weekly family visits. She has weekly BMod visits that assist with healthy living and behaviors. Reyna has improved her communication skills but struggles at times expressing her feelings. She completes weekly independent skills independently. Reyna volunteers at the summer enrichment program providing assistance with younger participants. Reyna will have weekend day visits for the youth to transition home.
Intervention: CSP, MHS and the youth discussed weekly improvement in the home and community. CSP and Miasia review the youth’s feelings and thoughts for the past week in the home and school. CSP, MHS and Miasia discussed ways to improve mornings and keeping on task. CSP and MHS praised Miasia for her progress on this goal. The youth had zero incidents of disrespecting others belongings. CSP provided reinforcement interventions for respecting boundaries in diverse settings. CSP discussed medication management concerns with MHS.
A major concern for children who are being treated with medications are that they are either being overprescribed or under prescribed (Rey & Sawyer, 2003). When prescribers have not had sufficient training in use of these medications, especially practicing on youth that are in underserved areas where access to prescribers with expertise in treating youth with psychopharmacology is extremely limited, they are at risk for being under or over prescribed. Medication may be overprescribed when there is insufficient attention paid to other supports and services that may benefit the youth, as well (American Academy of Child and Adolescent Psychiatry’s Committee on Community-Based Systems of Care [AACAP], 2012). According to the CDC and a study performed during 2005 through 2011, millions of youth ages 3 to 17 live with some type of mental health issue which includes behavioral and emotional problems (Brody, et al., 2013; Table I). This statistic brings up the concern of premature and/or over prescribing of medication to these youth and the effect that it has on them. Continuing studies are necessary to provide reasonable guidelines of when a child or youth should be prescribed medication based on short and long term effects. In addition, earlier diagnosis can provide an opportunity for behavioural therapy or alternative therapy, such as music, in place of the immediate prescription currently being written. At approximately 10:03pm after arriving to the community base home from Elizabeth NJ, Mr. Taylor observed Student George running into the Haddon house in a violent manner. Mr. Taylor then entered the home and observed Student George using profanity and throwing the television on the floor. While approaching the student Mr. Taylor prompted the student with several concern verbal’s. Student ignored staff directive and immediately ran out the front door. At this time staff exited the Haddon Home to look for the student. Unknowingly to staff the student ran around back and kicked the door into the Collings House in attempted to physically harming another peer Once Mr. Taylor entered the Collings House he observed Student George throwing a chair
Mental health/behaviors: CM was informed Dwayne has made a great improvement in the program; Dwayne has been on the right path and has not Awoled from the program since June’17. Ms. Harmon noted youth has not been cleaning his room, however, youth made an agreement with the team that he will clean his room. Dwayne has been engaged in other chores around the home, which has been allowing him to earn allowance.
Case management services were provided through a face to face interaction in Brea. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo and the youth. WYP Ngo took the youth to try some new food. The youth enjoyed the food and would recommend the restaurant to his parents. WYP Ngo discussed with the youth about a concern. The youth wants to go back to a regular high school. The youth would like WYP Ngo to discuss this issue with his mother. WYP Ngo will bring up the topic to tomorrow FTM (family team meeting). In the United States health system, there is a lack of professional specialization in adolescent health. We have adult and pediatric doctors, but there are few that are trained specifically for the growing needs of the adolescents. In the places that address adolescence heath, emphasis is mostly on curative medicine. It is necessary to modify the structure and design of primary care so that adolescent’s health service and place greater emphasis on health promotion, disease prevention, and youth development.
