Identify what measures are necessary to improve skills that seem – at this time – underdeveloped: 1. Review the handouts that my preceptor gave me in order to broaden my understanding and confidence in cardiac patient management and care. 2. Ask appropriately during observation of specific skills. 3. Review ATI related resources. Continue to do Cardiac specific NCLEX questions in order to assess my theoretical knowledge. 4. Continue to be prepared and being punctual to allow time to settle in and gather patient information and receive report. 5. Be prepared and safe to perform skills I have not performed yet. Watch skill videos on D2L, nursing simulation page.
Once someone has achieved a new skill this must be encouraged to continue on a regular basis through providing activities that help this
Situation: Two patients in their 70s present to the office at different times today, each with documented heart failure: one diastolic and the other systolic, and both are hypertensive. First, discuss the difference between systolic and diastolic heart failure, providing appropriate pathophysiology. ACEI/ARBs are the only medications prescribed for CHF that have been found to prolong life and improve the quality of that life. EXPLAIN the mechanism of action of ACEI/ARBs and how they affect morbidity and mortality in CHF. Be specific. Diuretics must be used very carefully in diastolic ventricular dysfunction. EXPLAIN this statement using appropriate physiology. Now considering all of the above, describe an appropriate comprehensive plan of
There will be a need for additional catheterization labs, surgical suites and expanded cardiac rehabilitation programs. There needs to be better coordination of care to ensure timely scheduling of procedures. Education programs need to be developed emphasizing cardiac risk factors, healthy living and lifestyle modifications.
8. List the nursing implications and education needs for each of the following categories of medication related to heart failure: See Attached
You must be able to understand what the person requires, act on it from the initial information until the matter is concluded, pass on the information to the relevant person/s to ensure the patient’s needs are met, this process takes time to have faith in your colleagues, to challenge them if you feel the right
All staff meets briefly to make everyone aware of how many patients are booked; how many appointments are confirmed; and how many appointments are available. Staff is also informed of who will be out, who will be late, and what things need to be done if time permits, so that priorities for the day may be established.” (Schuman, August 2015)
Based on a review, this was key to determining if Mr. Parker was on the cardiopulmonary arrest (Migliore et al., 2011). To understand the cues that I was collecting from the patient, I had to recall knowledge so as to understand the concepts associated with coronary
With over 1 years experience working in various wards throughout St Vincent’s Hospital as well as my placements throughout my diploma studies, I have gained a high level of knowledge of various elements of safety, effectiveness, and efficiency in nursing processes. I have been responsible for delivering a high quality of patient care, by completing tasks included but not limited to the following:
Reasoning and problem solving as well as planning and carrying out even the simplest of tasks are also skills that are lacking.
One of the scariest medical diagnoses a patient can receive from a physician is, their heart is not functioning properly. A malfunctioning heart will eventually lead to death if correction of the issue is not attempted. The heart supplies the entire body with oxygenated blood but also needs its own supply of oxygen rich blood. This is why coronary arteries are vital to the hearts ability to pump. A final option to correcting malfunctioning coronary arteries is a surgical procedure, coronary artery bypass grafting (CABG). Every year approximately 450,000 patients have CABG surgery in the United States (Mullen-Fortino & O 'Brien, 2008). Based on continuing healthcare trends, more and more people each year are having coronary artery issues, causing the need for CABG surgery to rise. Also patient demographics are changing, they are older, sicker, have more extensive coronary artery blockage / damage, and likely have poorer functioning ventricles of the heart (Sidebotham, McKee, Gillham & Levy, 2007). The nursing profession has a critical role in caring for the patients but also teaching and educating them before and after any procedure. Education and teaching has been implemented into evidence-based practice for years to facilitate better patient
Use your Student LPI report and what you have learned about Encourage the Heart to take action based on one of the six behaviors associated with this practice. Please provide the following in this forum:
Like I mentioned in my last monthly report, directors from other units in the hospital keep requesting my help, so the work is never ending. Since I have officially finished cardiac rehab’s policies, I have started working on those in cardiology. Doing cardiology’s policies is much easier because most of them already exist, and not much information has changed regarding the requirements of the procedures. However, there are some policies that Renee and Frank have requested I make to ensure that all staff are held
Next, support for everyday skills is needed because of lack of opportunity to develop a skill. For
Credentials: I feel strongly about this topic because there are quite a few people in my own family that struggle with heart issues, one relative even needing a transplant.
After I did my morning assessment on my patients, passed medications, and made sure I was caught up with my charting. I would help my patients with completing as many as their ADLs as I could. This helped me to ensure that the patient was bathed, cleaned, and dressed, and also to observe for an deficits in their ability to provide self care. I make sure to have all the supplies I need before providing care and if I was missing something I asked for the CNA to help me gather the needed supplies.
