With child abuse and neglect being so prevalent globally, it is important to recognize why these types of maltreatment occur and is a relevant topic for all to explore. Many are interested in how child abuse and neglect affect the individuals who are victims as well as the use and efficacy of treatment procedures. Child abuse has serious physical and psycho-social consequences which adversely affect the health and overall well-being of a child. It seems as if medical neglect happens too often in pediatrics. Medical neglect is when a parent fails to adequately care for their child, when those cares are pertinent to treating an illness or physical injury. In the two years that I have worked in pediatrics, I have seen many of these cases. The majority of the time, it occurs with our chronic patients who are medically heavy, time consuming, hospitalized a lot, and the parents do not adequately take care of them. In this paper, I am going to discuss a case of medical neglect that I have encountered in a child who had received a heart transplant.

The issue of child neglect is one of the most current forms of child maltreatment especially in the United States. According to the National Child Abuse and Neglect Data System, child neglect is referred to as a kind of maltreatment which relates to the failure of a caregiver to offer the needed, age-appropriate care, despite the fact that the parent may be financially stable to do so (Conte 13). Neglect in many instances is described by an ongoing pattern that relates to lack of care and is observed by individuals in close contact with a child.

Child abuse and neglect is a significant problem in the United States that should demand direct attention. There are approximately three million child abuse reports made each year, and these reports involve more than six million children. Out of four to five children that die daily, 70 percent are under three years of age. There are 90 percent of cases involving of children who know who know their perpetrator. The main factors of child abuse are physical, emotional, and sexual abuse. In many cases involving sexual abuse, there is usually not any medical evidence to be shown, and there are usually not any witness due to the sexual acts taking place in private places. That is why it is considered the “hidden” form of abuse. Neglect is a type of maltreatment related to the failure to provide needed, age-appropriate care.

Neglect and abuse is a serious issue for the elderly living in our community, especially for those living in nursing facilities and it needs to be taken seriously by society. Due to chronic illness in this population, primary care providers are frequently in the best position to diagnose, intervene, and report potential victims of abuse and neglect. Although all caregivers who encounter the elderly whether in outpatient setting, nursing home, or even at home should know the warning signs and be able to report any signs of abuse immediately.

Reports of child abuse, including sexual abuse, beating, and murder, have jumped in the United States and some authorities believe that the number of cases is largely under reported. Child neglect is also included in legal definitions of child abuse to cover instances of malnutrition, desertion, and inadequate care of a child's safety. When reported, inadequate foster care services and a legal system that has trouble accommodating the suggestible nature of children, who are often developmentally unable to distinguish fact from make-believe, complicate child abuse cases

As most of us know childhood obesity is increasing at an alarming rate and it something that needs to be addressed. However, few have thought about severe childhood obesity as possibly being considered a problem with medical neglect. The specific case for medial neglect pertaining to childhood obesity raises numerous positions that I both agree with, as well as disagree with. It does however, arise a great question about who needs to be held accountable when families cannot or choose not to deal with their child’s severe obesity. This issue needs to be dealt with because the conditions and comorbid problems that result from severe childhood obesity reach far beyond the physical complications.

The issue of child abuse and neglect is serious, controversial, and is escalating in today’s society. Many people are not aware, but child abuse is rampant in our society. Many child abuse and neglect cases go unreported because a person may not know the signs and symptoms of child abuse and neglect or perhaps the person or person may feel that this is an private issue and needs to be handle with in the home and no outsiders should be involved. Without the proper awareness of child abuse and neglect and the involvement of everyone this issue will continue to raise our eyebrows.

Abandonment and neglect of the elderly is the greatest injustice they suffer from individuals that care for them. As the life expectancy in the United States increases, there will be an increase in care services for the elderly.
