Narrative- On 29Nov16 at 1427 hrs. I, Deputy Halbasch, was dispatched to Co Rd 4 & Intrepid Dr. for the report of a 10-50 accident. I was informed a car went off the roads and the driver, Danielle Hauk, was not injured and already requested a tow truck. Enroute, Inv. Olson informed me he stopped there, checked on Danielle & she was not injured. I arrived and located the car in the west side ditch up against some small trees and a Paul Bunyan Communication box. Danielle informed me she was traveling alone south bound on Co Rd 4 when she lost control on the slush, spun out, entered the ditch backwards, and hit the trees. One of the vehicles involved pulled away from the accident and headed north on Sprinkle Road. Two witnesses called this in and gave a description of the vehicle. From this description the vehicle was pulled over on the corner of East Main Street and and 26th street at 14:07.
On 02/11/2016 at 1436 hours, I was dispatched to Wawa (1060 Delsea Drive) for a report of an intoxicated male fumbling to unlock his blue Ford F-150 that was parked in the parking lot. Upon my arrival, I observed the vehicle in question leaving the parking lot traveling northbound on Delsea Drive. P/O Ziegler #5196 who arrived with me was able to conduct a motor vehicle stop on the above vehicle as I turned around in the parking lot of Wawa. After I arrived to the stop, P/O Ziegler and I approached the F-150 bearing NJ registration E80GFJ from the passenger side. As I requested the driver now identified as Shawn J. Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. The driver of Unit 2 then noticed Unit 1 coming from behind in the right lane at a high rate of speed. Unit 1 tried to avoid the puddle without slowing down and impacted Unit 2. Margaret Harris (W) contacted me and told me she had witnessed the collision. Margaret stated she was stopped behind Cheryl's vehicle on the eastbound Hwy 18 off ramp to West Valley Hwy S, and Krystyna's vehicle was stopped next to, to the south of, Cheryl's vehicle in the right turn only lane. Margaret said the signal turned green, Cheryl's Impreza began to make a right turn onto West Valley Hwy S, but Krystyna's Odyssey went straight ahead through the intersection instead of turning right, and the front of Krystyna's vehicle struck the passenger side of Cheryl's vehicle.
On Friday, November 17, 2017 at 1821 hours I, Officer Radswillas responded to a vehicle accident on Route 422 West. I found the vehicle on Route 422 in the area of mile marker 170.2 (west of the River Bridge but east of the Stowe Exit).
Amy Miller related she was in the yard at her mother’s residence at 11713 Lithia Pinecrest Road. Miller said she saw a black pickup truck “hauling ass” westbound on Lithia Pinecrest Road. Miller said she maybe 55-60 miles per hour. Miller said shortly after seeing the truck go past, she heard a loud boom. Miller said she ran out to the road and saw a tractor in the westbound lane of Lithia Pinecrest Road and saw a body lying in the roadway as well. Miller said there was also a pickup truck in the grass off the south side of the road, facing east. Miller related she never went to the crash, but did call 911.
On 04/17/2016 deputies were dispatched to the entrance of the river on the south side of Stone Lake for an injury accident. The driver of a 2007Polaris Sportsman was taken to that location after he had rolled the ATV in the river. The driver stated he was driving through a puddle of water when he lost control. The driver said the ATV rolled over several times before he was thrown form it. The driver is a juvenile. He was wearing a helmate at the time of the accident.
All five of them were wearing their seatbelts according to state police, and alcohol was a factor in the crash.
On Monday, May 23, 2016, at approximately 12:45 a.m., I, Trooper Bradley Brachear, received a telephone call from Sergeant Robert Ventura. Sgt. Ventura advised Trooper Bradley Otten of Illinois State Police District 13 requested my assistance with a two-vehicle crash on Illinois Route 4 approximately 0.23 miles south of Norton Lane in Tilden Township, Randolph County. I responded from the Lebanon area to assist. The crash resulted in one fatality.
In the fall semester of 2012, four teenagers at the high school I attend died tragically in automobile accidents. One of the students who lost their life was my cousin Acasia Lee. She neglected to wear her seatbelt while operating her vehicle and she died because she did not have it on.
On 18Jun16 at 1506 hrs. I, Deputy Halbasch, was dispatched to 16683 Co Rd 18 for an ATV accident with injuries.
John Carney said he was westbound on Shell Point Road approaching the intersection of 7th Street NW. Carney advised he going home and was slowing down to making a left turn onto 7th St NW. Carney said as he was making the turn, he saw a car, (Ford), approaching from behind him and it seemed to be driving pretty fast. Carney said as he was making his turn, he looked in his driver side mirror and saw the Ford just barely miss the rear of his vehicle. Carney said he then heard tires squealing, looked behind him and saw the car sliding across the road. Carney said after he turned, he turned around on 7th Street NW and went to the crash to see if he could help and parked on the side of 8th Street NW.
On 04-01-2017 at 1252 hours, Det. Carter # 181968, notified me of a traffic crash fatality that occurred on County Road 39, approximately two-tenths of a mile south of Thompson Road. The crash occurred at approximately 1200 hours. I responded to the crash scene and upon my arrival at approximately 1408 hours, took digital photographs of the scene and also forensically mapped the scene.
Upon arrival, I observed a two vehicle accident with no injuries. I asked, the drivers of the vehicles to remove there vehicles out of traffic way and to give me there proof of insurance, drivers license and regesitration. All documents were provided and I discovered there names were Mr. Charile Moore and Mr. Hezekiah Pierce. I investigated the accident scene before the vehicles were moved.
Life is unpredictable and an accident can happen anytime. on our way back home from church my dad was driving a black 2012 Toyota Highlander, which is a family car that seven people can ride in it. My younger brother, Taw Nay Gay, and I were sitting on the seat behind the driver seat by the door. My other two younger brothers, Gay Nay Soe and Soe K Maw, sat in the seat behind me, and my mom sat in the front passenger seat. For the first time a nineteen year old girl like me started to believe that I had a reason to live and my life could be taken away anytime. This happened on October first 2017, 7:30 pm when we got into a car accident by the traffic lights intersection. Three cars were damaged, but everyone in the cars were fine.
