On 2/17/16 at approximately 1351 ICS was activated at Industry Building for an offender OID 199625 who was having a medical problem and Medical was requested. Upon arrival on the scene offender was sitting in a chair in no acute distress. Offender was brought to the clinic and was evaluated by the primary provider. Offender was treated per the doctors order and returned to her unit in a stable condition.
On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
D-This writer met with the patient as he was placed on HOLD to address the status of the IOP. The patient provided this writer a paper that was provided to him with listing of IOP for him to explore. The paper shows scribbles of the patient taking down notes about his attempts of who he called. The patient reports Connecticut Addiction Recovery will call him back within 24-48 hours. The patient was able to schedule an appointment with New Direction for May 20th at 7pm; patient spoke with Dan. This writer commends the patient for all of his efforts; however, the patient needs to schedule something earlier than May 20th. This writer asked the patient about ICRC-Coventry House. According to the patient, he called the contact number and showed proof. The patient reports that ICRC gave him two different number and told him to do a walk-in at 8:30am. CCIB Intake received e-mail from John Merrill regarding the initial complaint. The information is the same as the LIC802. LA City Fire Department of the location being problematic due to the number of request for medical assistance. There is a resident who is diabetic and a victim of strokes and kidney failure. He is reported to be in need of care and supervision. He was ordered by the Court to be placed at the location and is assigned to a Probation
Upon arrival to Western State Hospital, deputy J was asked to pat search the inmate Kelly due to unavailable female staff at the facility. I informed the intake officer Robert that inmate Kelly has a history of mental health issues; she has assaulted a few police officers. Inmate has stated she wants to go to a spaceship. During transport inmate Kelly sustained a head injury. She was taken to a local hospital and evaluated by medical staff. A copy of the hospital evaluation/discharge document was given to the intake officer.
On Friday, September 30, 2016 at approximately 0536 hours, a Black Male Stab Wounds Victim walked into the Emergency Department lobby with a severe Stab wounds laceration to the lower leg and a human bite wound. Security staff was contacted by E.D. triage Nurse Tony Rusiano and escorted the patient to ED room#42. Security conferred with E.D. Charge Nurse, Johnathan Bacal and an E.D. Lock down was initiated at 0538 hours. All Security Officers responded and dispatched over front and back E.D. entrances. Orange County Sheriff's Office was contacted at 0539 hours at which time Security Manager Richard King was informed via text messages along with House Nurse Administrator Sabrina Presley. CCIB Intake received a call from owner/administrator Therese Kragness who disclosed that the local Law Enforcement was present at the facility. According to the caller, an employee who was having difficulty with her boyfriend received a text threatening to come to the facility. Fearing for her safety, the employee notified Law Enforcement and was escorted to the Police Station to file a restraining order against the boyfriend. Conversely the boyfriend did not appear at the facility. The Administrator will fax the hard copy of the report identifying the employee within the required time
On 2/14/17 at approximately 1138 ICS was activated for offender Peltier OID158563 for a medical ditress. Upon arrival at the scene offender was lying in bed anxious, restless , crying but in no acute distress. Offender was put on the wheel chair and brought to the clinic for evaluation. Offender was evaluated in the clinic by the provider and was treated. Offender returned back to her unit in a stable condition and was informed to contact medical if her symptoms worsens.
On 5/06/17 at approximately 1843 ICS was activated in Novello for offender Vergara OID 251641 for c/o dizziness. Upon arrival at the scene, offender was sitting on the floor but no acute distress. Offender was assessed and her vitals sings were normal. Offender was treated using standing orders and was scheduled with the provider on Monday for a follow up. In the mean while offender was educated to contact medical if symptoms worsens. ICS was cleared at 1852
The investigation was initiated sequel to a complaint lodged at the Correctional Institutions Division (CID) office on 22 December 2017 by Ms. Messisco Dawn on behalf of her son Offender Fry, Chance (TDCJ No 2046947) alleging retaliation and wrongful conviction of her son for an assault incident he never participated in. She further explained that the incident happened in Wallace Unit when her son was assigned to that unit.
On 06/01/2016 at 8:55 AM I along with Cpl. Pries, R. was escorting the above named inmate from the BHU Interview Room back to his assigned housing, BHU-13. Once we entered Section II of the BHU the inmate attempted to kick and head-butt Cpl. Pries and me. Because of this we had to place the inmate on the ground in order to stop his behavior and to gain control of him. Once the inmate was under control he was taken back to assigned cell. At approx. 9:15 AM the inmate was seen by the SMH nurse, RN Henkleman, P.
After reviewing Bill Smith 's case file, making a step by step guideline, and completing the Washington State Offender Needs Assessment, I have been able to develop a three month case management plan for him. I will go over four main sections that complete the three month plan. These four sections are the type of counseling modality I will use, the treatment referrals that I will make, a detailed plan for each of the twelve weekly meetings, and a plan for evaluating Smith 's progress. Once this plan is in motion, I 'm confident that Smith will have a good transition into the community.
This project will utilize Option B as a means to relay a policy/strategy implemented based on offender reentry. In 2005, President Bush implemented the Prisoner Reentry Initiative(PRI) to reintegrate ex-offenders into communities of society. The theoretical paradigm that supports the use of labeling offenders within the criminal justice is cleverly, labeling theory. Mentioned lightly above, was the premise behind labeling theory. This theoretical paradigm is predominantly interesting in exploring labeling of offenders, due to the fact that it both targets the offenders, as well as the individuals who are placing labels or stigmatizing the offenders in the first place based off the perceived deviant action.
