DATE & TIME: Friday, December 16, 2016, 2016. Dispatch time was approximately 11:25 p.m. VENUE: Sheridan Rd. and Potter Rd. / Clayton Township / Genesee County / State of Michigan. INFORMATION: On the above date and time, I, Officer D. Stone was dispatched to Sheridan Rd. and Potter Rd. for a vehicle in the ditch. When on scene there was beige Chevrolet Impala in the ditch on the west side of the road. The vehicle was backwards facing south in the ditch near the intersection of Cronk Rd. and Sheridan. The vehicle in the ditch obstructed the north bound view of Sheridan Rd. from Cronk Rd. The driver stated he was heading north on Sheridan before losing control and going into the ditch. CONTACT WITH THE DRIVER DERRICK THEOTIS BOX: I made contact with Mr. Box the driver of the beige Chevrolet Impala he was sitting behind the wheel of the vehicle. I asked him what happen, he stated that he was heading home north on Sheridan Rd. and lost control of his vehicle. Box for his Michigan driver’s license, vehicle registration and proof of insurance. Mr. Box gave me a Michigan identification card, a temporary instruction permit; he could not produce his vehicle registration or proof of insurance. I asked Mr. Box several times for his proof of insurance and he never could produce it. SOS showed that he did not have insurance. While talking with Mr. Box I noticed that the tire on the driver’s side was severely warn from the outside edge to almost the middle. I checked the tire on the passenger side and found the tire was completely without any tread and could see the wear bars showing through. I checked the rear tires and they looked fine. On 05/30/2015, at approximately 1816 hours, your affiant observed a silver Chevrolet Classic occupied by two males. The vehicle circled the block in a counterclockwise manner, repeating a portion of their route. During this time I located the owner of the vehicle utilizing a search through the Pennsylvania Department of Transportation. The registration indicated the plate belonged to another vehicle and registered to Brittney Fisher, who was not an occupant in the vehicle. I activated my emergency lights initiating a vehicle stop in the 100 block of West Washington Street and while stopping the vehicle I observed the passenger making furtive movements towards the center console.
Upon my arrival I spoke to Chris Holt, who stated that he had heard a truck door close and went outside to see what the noise was. Chris stated that when he walked outside he observed a County Road sign 4897 lying on his front porch. Chris then observed a white Chevrolet pickup possibly a 4wd with two kids in the back of the truck throwing two more County Road signs and a tripod to a transit into his yard. On July 22nd I was driving North on Lambert Road in Glen Ellyn about to make a left onto Roosevelt Road. I was in a Enterprise Car Share vehicle that is a 2012 silver Toyota Yaris. As I approached Roosevelt Road there was another vehicle in which case it was a black Toyota. This Toyota sideswiped me as the driver said she was distracted and did not notice my car in the left turning lane as she was in the right lane trying to get over. After looking at her vehicle it appeared most of the damage came from her driver side mirror and right above the rear driver side tire on the black Toyota. Therefore it damaged the passenger side of the Enterprise Car Share vehicle.
Approaching the scene from the east, HCSO Deputies had the entire intersection of Memorial Highway and Kelley Road closed to traffic. I was waved down by a female at the entrance to the Edge Apt. Alize Rivera advised that she had been involved in a motor vehicle accident on Elm St. she stated that the other driver involved had her boyfriend the stated driver of Unit 2, move from the scene of the crash to the Edge Apt, and then he drove off. Rivera advised that the driver of Unit 2 ,was stopped waiting to turn into the Troy Softball parking lot when a black Chevy Silverado struck her vehicle from behind. She stated that the Driver of unit 1 was a white male, around his mid 20, and had a blue hat on. Officer were unable to locate unit 1. I asked Eric for his license, registration and insurance. Eric was able to provide me with his license and insurance, but was unable to provide his registration. Eric explained that he didn’t have a registration because it was being redone.
In a sworn statement, Todd Ricketts said he left his home at approximately 0310-0315 hours, to go to work. RIcketts said he drove down Taragon Drive to Boyette Road and made a left turn. Ricketts related when he made the left turn onto Boyette Road, he drove into the outside lane and was travelling about 40-45 miles per hour. Ricketts advised he never saw the pedestrian in the roadway. Ricketts advised he heard something hit his car, then saw the pedestrian on the windshield of his car and applied his brakes. Ricketts said he came to stop, still within the outside travel lane and even with the arrow in the bike lane, west of the intersection. Mr. Hall stated he walked out on to his deck and he saw a male in a silver 4 door car. Mr. Hall stated he could not tell what kind of car or any distinguishing features of the male in the car. Mr. Hall stated he shouted at the guy and he left. Mr. Hall stated he was not sure if any of his property was missing because the business he purchased his knives and sharpening stones are not open until Monday, and they may have shipped items in different packages. Charles E. Collier said he driving eastbound on Lithia Pinecrest Road behind a gray pickup truck. Collier said he saw a westbound Ford F150 make impact with something but didn’t see what it was. Collier related he then saw the F150 slide across the road and into the grass on the south side of Lithia Pinecrest Road. Collier said the F150 ended up facing in an eastern direction. Colliers advised he stopped his truck and ran to check on the driver of the Ford F150, who seemed to be ok. Collier said he then ran to the man lying in the road and said he going in and out of consciousness.
John Carney said he was westbound on Shell Point Road approaching the intersection of 7th Street NW. Carney advised he going home and was slowing down to making a left turn onto 7th St NW. Carney said as he was making the turn, he saw a car, (Ford), approaching from behind him and it seemed to be driving pretty fast. Carney said as he was making his turn, he looked in his driver side mirror and saw the Ford just barely miss the rear of his vehicle. Carney said he then heard tires squealing, looked behind him and saw the car sliding across the road. Carney said after he turned, he turned around on 7th Street NW and went to the crash to see if he could help and parked on the side of 8th Street NW.

At approximately 9:50 AM, An Outbound Desk clerk notified Shift Supervisor Ashley Lorah about an accident in the truck yard. S/S Lorah asked Security Officer (S/O) Neil Zola to go out to the yard and collect the information needed. Upon arriving at the scene of the accident, S/O Zola made contact with RJ Davis driver Leroy Michaels, who was driving tractor 241. Mr. Michaels stated to S/O Zola that while attempt to pull EXPO trailer 097288 from slip location F049, the EXPO trailer hit and collapsed Pallet Express trailer C529 in Slip Location F950, and also causing the EXPO’s trailer door to also fall off. Mr. Michaels also stated that the EXPO’s trailer door that fell off was move by himself and another Yard Jockey to Slip Location F950.
On 12/7/16 at about 5:55 AM, Amazon Associate (A/A) Bryan Vincent (bvvincen) notified security that he found damage on his vehicle and it may have been caused by on site. A/A Vincent owns a Midnight blue Honda Accord. At the time of incident, he was parked in the B-Lot 2. Upon being notified by the officers in B-Building, Security Account Manager Enmanuel Cabrera instructed Security officer Matthew Pauline to assist with taking pictures of the damage and gathering the information needed. I asked Ricky where Joseph lived and what her drove. Ricky told me he lives at 104 East Grove Street Apartment 1 and he drives a 1998 Saturn Ion. I went to the residence, but no one answered the door. outside the building their was two 1990's Saturn Ion's but nether were registered to him. I informed Ricky I was unable to get ahold of Joseph.
