Introduction
Throughout this paper I will discuss schizophrenia, what the characteristics associated with it are, how schizophrenia is diagnosed, treated, side effects, prognosis and how it impacts the person and people involved with them. I will also discuss how this disease has personally impacted my patient who has struggled with this disease for many years.
Client Assessment
My patient is a Native American female between the ages of 30-40. She is currently unemployed who has a history of being sexually, physically and emotionally abused by her father while growing up starting at the age of 2 years old. She is the oldest of four other siblings that she states she is no longer close with as she has “burned her bridges with everyone”. She started using drugs and alcohol when she was about 12 years old and dropped out of high school in the 10th grade. She is currently separated from her husband whom she has three kids with between the ages of 2-12. She was diagnosed with paranoid schizophrenia in her mid-20s and stated that her sister was diagnosed with bipolar a few years ago. She has a history of alcohol and drug use consisting of methamphetamine, heroin, oxycontin and marijuana. She has had multiple ER visits and psychiatric admissions over the past 10 years. Prior to her current hospitalization she was previously in a treatment program for drug use which she eloped from and was unable to be found for almost two months. Ms. Smith is a 30 year old single, Caucasian female referred for a psychosocial assessment by DOC Parole Officer Ward. She reports she was released from prison 2 months ago after a 3 year sentence for attempted escape due to not notifying her probation officer of her address change. Ms. Smith states due to her past substance use history and trauma experience her referral sources ordered counseling to address complex issues related to her emotional and physical well-being.
The patient is a 34 year old black male. The patient reports he is single with 3 daughter ages 3, 11, and 13. The 3 year old live with him. He reports currently live with his mother and brother. The patient reports he has 14 years of education however has not received his degree. The patient reports being on probation for Injury to a child for the next two years. The patient is scheduled to be complete his probation in September 2016. The patient reports Cannabis is primary substances and his last use July 20, 2015. He stated on that day he smoked about 5 joint. The patient also report that he tried Xanax . The patient denies any issues with HI/SI. Patient also reports he is taking his medication as prescribed. The patient appear to be in the pre-contemplation stage of change. The patient next scheduled individual session with the counselor is on Thursday, July 30, 2015 at
She has been to the following facilities: Counselor H Group (Carbondale, IL), Jewell Psychological Services (Edwardsville, IL), Percy County Counseling (DuQuoin, IL), Psychiatrist (Springfield, IL), and Pavilion Behavioral Health System (Champaign, IL). Cit Cat has been to several facilities trying to break her dependency on drugs. Drugs are a coping mechanism she has dealing with her life. Her father is not in her life and when she was 9/10 years old her father’s friend molested her. She lives with her mother and her stepfather. She began to get angry when she was middle school and then it continued to escalade throughout high school. During her 11th grade in high school she was expelled. When she was 16 years old she moved out of the house. Being raised Catholic she grew up with a lot of rules and emphasis on family. The values she was taught in church still cause her to feel guilty today with certain decisions she makes. Her father was accused of sexually abusing her, however she does not remember if that was to be true or not. He was then put in jail when Rhonda was four years old for drug trafficking. Rosa Cunningham (full name is Rosa Lee) is a 53 year old African American female client of average height, slight build, and is appropriately groomed. She has 8 children, 2 of them being female and 6 males, all adults. Rosa is a widow and reached this status after being separated from her deceased husband for many years. Rosa is currently hospitalized for pneumonia, and has been hospitalized several times in her life for diferent illnesses. Rosa’s medical history as self-reported is HIV and seizures. Rosa is a heroin addict and has been this way for several years. Rosa is involved with the local methadone clinic and receives 55mg of methadone daily. Even by receiving this daily dose of methadone, Rosa continues to use heroin. Rosa has several legal and health issues that are present also, despite which she continues to use heroin. Rosa has a lengthy criminal history to include arrests for prostitution, larceny, and selling drugs. The reason for today’s assessment is a referral made by the social worker at the hospital in which Rosa is a patient at and discharge planning is to be made for aftercare.

She has been dependent on alcohol for about 20 years (Confederation College, 2018). She has a track record of offenses, such as shoplifting and selling illegal drugs (Confederation College, 2018). At this present time, she has a court case coming up for stealing and assaulting a police officer while under the influence of alcohol (Confederation College, 2018). Mr. Allison is a 26 year of male who presented to the ED via LEO reporting feeling depressed and suicidal ideation without a plan. Mr. Allison reported to nursing staff relational conflict with a female he started to see after his wife and he separated. At the time of the assessment Mr. Allison presents calm and cooperative. According to Mr. Allison 5 months ago his wife and he separated after a 7 year long relationship and 2 daughter. Mr. Allison reports for the past two months seeing another female who he has become involved with. Mr. Allison reports this new female told him a few weeks ago she cheated on him. Mr. Allison noted this female has issues with alcohol. He reported being a supportive person in her life, which has caused additional stressors in his life. He expressed feelings of depression. Mr. Allison describe feelings as feelings of hopelessness, worthlessness, tearfulness, sadness, isolation, increased sleep and fatigue. Mr. Allison reports relational issues, family conflict, legal issues, recent anxiety attacks and substance abuse as stressors contributing to his distress. He reports no history of self harm, no hospitalization for mental health, a strong support system (referring to his father, mother, and wife). Mr. Allison was seen on 10/08/16 and 10/09/16 for anxiety attacks, however was discharged. On 10/9/16 he mentioned issues with his recent female friend and reports he felt better over his situation and want to go home. No mention of suicidal ideation

She states she cannot afford to care for her family and work. She is unemployed and has 2 dependent children. She has reported a drug habit of $200 1 to 2 times a week and has resulted in illegal activities to support her habit. Legal environment also having a huge impact on her ability to access services should she continue down the same destructive path. She also has some legal issues regarding the custody of her children and the State regarding safety and this most certainly affects one’s access to services especially if they are having ongoing legal problems or have low income needs. It is an incurable disease whose causes are unknown, yet whose effects are mind and body crippling.
