Focus: Taliana, Mrs. Clark (MHS) and Ms. Smalls (MHP) review and sign Individual Plan of Care (IPOC).
Intervention: MHP, Taliana and MHS review the goals and other sections of the IPOC. MHP answered questions from Taliana and MHS. MHP explain the length of the IPOC, interventions that would be use during the 90 days and coping to assist with the reduction of symptoms. MHP encourage Taliana to review the IPOC again to prepare for the next visit.
Response: MHS expresses the goals are suitable for the youth in care. Taliana asks questions about the interventions and returning home to biological father and step mother. Taliana and MHS provide additional information about Taliana’s behavior patterns. MHS ensure Taliana ask all her questions before MHP explain the length of the IPOC. Taliana express comprehension of each section of the IPOC. MHS check her calendar for the next visit.

Today, MSC visited Maria at her residence for a face to face visit. When MSC walked in Maria was having a snack at the table. Maria waved and greeted her staff. MSC said, Hello, How are you?" She stated that she needs to talk to MSC in private after she finishes her food. MSC spoke wither Medical Direct Care Counselor Marie Rose. She stated that Maria has been doing good. He is cooperative at times, but requires redirection. Maria stated that she is medically stable and her behavior has improved over the past few months. She is communicates her wants and needs effectively. Maris continues to go to all of her medical appointments and enjoys attending program. Maria goes on all community inclusions and makes suggestions on places she would like to go. She struggles with balancing money, staff continues to assist her in budgeting.
Clinician provided support to youth during this transitional period. Clinician assisted the youth with positive ways to communicate with MHS and foster sister in the home. Clinician encouraged the youth to get involved once enroll into the summer enrichment program and community. Clinician will assist the DSS worker and MHS
Intervention: Ms. Smalls (MHP), Santonias and Ms. Givens (MHS) discussed Santonias complying with the rules of the home. MHP and Santonias had a discussing about his NFL football team as a way to build rapport. MHP requests feedback on possible goals from Santonias and MHS for Individual Plan of Care. MHP, Santonias and MHS discuss and process an incident that happened at the school’s gym.
Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice.
The team is in agreement to continue with four hours of ISS level 2 and one hour of ISS level 1 services. The team discussed IIH services for the youth in order for youth to learn how to express her feelings. CM asked if the family had any preferences regarding service providers. The family prefers a female IIH. The family is in agreement with the strategies discussed. CM explained the referral process to the family and CM will follow-up with referral. The team is in agreement to attend the Special Dragons program. CM will follow-up with the program in regards to availability.
Focus: Taliana will adjust to the new home and comply with all home and school rules.

My name is Joselin Puelles, I am the new summer intern for the Mental Hygiene Institute and I will be the one working to develop a support network for the CLMHD-YMHFA trainers. The main goal of this support network is to assist trainers who have been trained in the past, or who will be trained in an upcoming training session, with any challenges that may interfere with a successful training session.

MHP assist Taliana with identifying verbal and non-verbal clues when establishing or rebuilding a relationship. MHP, Taliana and MHS process an incident of Taliana making friends at school.
Intervention: MHP and MHS discuss Taliana refusal to communicate with the biological father and reasons Taliana wants biological father rights terminated. MHP provided MHS a report from Taliana’s DSS worker, Ms. Jenkins. MHP discuss the reason for court continuing till April 2017 and how Taliana’s behaviors are hurting her to returning home. MHP reviewed ways to assist Taliana with making better choices.
