From the table given, 

a) Originated within the organization :

The rating has been taken from the employee within the organization and outside the organization. 

The percentage of employee within the organization is 58%

b) Percentage of employee from both internal and rated very good :

Which means, the employee must be within the organization and at the same time, he rated as very good.

Thus, the percentage of employee from both internal and rated very good is 23%

c) Percentage of employee needs improvement are poor :

Let A be the case of needs improvement and the percentage is 10%

Let B be the case of poor and the percentage is 4%

And both the case are independent, since a person rated needs improved not rated as poor and vise verse

Thus, the Percentage of employee needs improvement are poor is 10%+4%=14%
