Question

One of the indications for prescribing nateglinide is:
A. Depletion of pancreatic beta-cells
B. Significant postprandial hyperglycemia
C. Tendency to develop lactic acidosis
D. Resistance to sulfonylureas
E. Insulin resistance

Expert Solution
Step 1
  • Nateglinide is a drug with antihyperglycemic properties that is used in the treatment of non-insulin-dependent diabetes mellitus (NIDIM).
  • The drug acts by binding with β cells of the pancreas and stimulates the release of insulin.
  • NIDIM is an inherited metabolic disorder that is characterized by the hyperglycemic condition followed by the resistance to ketosis. The β cells of the pancreas reduce the production of insulin in NIDDM condition.
  • Additionally, the body fails to respond properly to the produced insulin i.e insulin resistance is developed.
Step 2

Option A: Depletion of pancreatic beta-cells

The condition arises in type 1 diabetes in which an autoimmune response of the body develops resulting in inflammation in β cells referred to as insulitis that results in the loss of insulin cells.

Option B: Significant postprandial hyperglycemia

The condition occurs after meals. During this condition, the liver does not inhibit the production of sugar like in general body mechanisms. The liver stores sugar in the form of glycogen and if blood glucose level stays 180mg/dl after 1-2 hours of eating then it is referred to as significant postprandial hyperglycemia.

Option C: Tendency to develop lactic acidosis

Lactic acidosis is identified as the presence of lactate in blood in an abnormally high concentration. The lactate in high concentration interferes with the normal integrity and functioning of cells by preventing oxygen availability to the cells. The prolonged hypoxic condition results in multiorgan failure if not reversed.

Option D: Resistance to sulfonylureas

Sulphonylureas are a group of drugs that are used in the treatment of type 2 diabetes. In type 2 diabetes, the body fails to utilize insulin properly that results in elevated blood sugar concentration termed as hyperglycemia.

Option E: Insulin resistance

The insulin resistance is the condition when cells fail to respond to insulin hormone that results in the hyperglycemic condition. Insulin resistance is observed in type 2 diabetes.
