On 03/25/2015, at approximately 1214 hours, your affiant was dispatched via radio by Schuylkill County communication center to 209 East Centre Street for a break and enter into a structure.
The witness (WEIKEL) informed the dispatcher that there is someone inside the residence cutting pipes out. The witness was in a residence above the one in question and advised the building below is empty and no one should be inside of it. The witness stated they could hear the pipes being cut because they were shaken them and the noise travelled up through the wall into their residence. The witness said the male inside the house is a WHALEN.

Next to the black bag on the floor I observed a large amount of copper pipe. The pipe was too long to fit in the bag and appeared to be stack next to the bag waiting to be shortened for the purpose of removal. I observed a large pair of bolt cutters next to the pipe. All of the tools had a metal copper transfer on them.
I asked the defendant where he got the diagonal cutters from. The defendant said he just grabbed them from inside and didn't know where he got them from. I asked where they were, upstairs or downstairs? The defendant said he got them from the kitchen table. I observed the kitchen table which is on the first floor.
I observed boot prints in the basement, the flooring was dirt and wet. After looking at the imprint I asked the defendant to lift his foot to look at his boot print. The markings were the same. The boot prints were observed in areas of the basement where the copper piping was already cut from the ceiling. One of the areas observed was the north east corner of the basement. This area was difficult to get to due to the amount of items scattered and stacked. To get to this area it required the person to crawl and climb over the items placed in the

When the interview started, Darnel stated that he was just left the court house and he was just walking down Pinelog Road. I asked Darnel did he go to the general direction of Barn Oak Court and he stated no. I exited the room and acquired the finger print cards that were lifted from the scene. I then relocated back to the interview room and asked Darnel were these his finger prints that were left on the window seal. Darnel advised me that they were not his and I left the interview room. When I left the interview room, Darnel took a tissue and wiped the dirt off his shoes and placed it back in the tissue box. Darnel then started to lick his finger and bite on his fingertips. I arrived on scene at 21:00 hours and met with the victim, Heidi M. Zivanovich (F/W, DOB: 07/16/1979). Zivanovich escorted me to the south side of her residence. She pointed to a window where she believed someone attempted to make entry inside her residence. The window screen was damaged. The window screen appeared as if it were cut with a sharp object. The cut was located approximately 8 inches from the bottom of the screen and cut parallel to the bottom of the screen. The glass pane window had been pushed opened. The window was half way open. There was a closed window blind affixed to the interior window frame. There was no entry made inside the residence from the window due to a piece of furniture inside the room blocking the window.

Upon my arrival I spoke with the complainant identified as Angela K. Houston (DOB 06/06/1978). Angela stated that her husband, identified as Don A. Houston (DOB 04/24/1975) had came to Angela’s house without her permission and hit her multiple times. Angela took me to the inside her residence while Sgt. McMichael spoke with the neighbor at 311 N. Kingdom identified as Gary L. Hill (DOB 05/13/48). Angela stated that as she was in her house, she heard a noise coming from the back of the residence. Angela went to look what the noise was, and heard Don trying to break in to the house through the back door. Angela stated that when Don came in through the garage door that connects to the residence, Don grabbed her by the neck and hit her in the head with his hand. Angela was trying to defend herself but was not able to do so. Angela stated that they ended up in the back left bedroom of the residence where more stuff was damaged. Sgt. McMichael took pictures of the damage in the residence and the damage on Angela’s body (see attachment). Angela was provided a Domestic Violence Victim form. Angela agreed to come to the Police Department for a video statement. I provided Angela a ride to the Bethalto Police Department.
On the above date and time I was dispatched to 205 W. Columbus St. for a non-active burglary. I arrived on scene and spoke with Ray NESTOR who stated when he returned home from work he noticed items were moved around and out of place in his house. First thing NESTOR noticed was his kitchen towel was moved to a different place. NESTOR stated he then noticed a washed plate was knocked over. NESTOR stated he looked at the window and the screen portion appeared that it was bent and someone had tried to bend it back in place. NESTOR stated he noticed one of the window hinges was broke.
The business already has a duty to find, correct and warn of potential dangers to customers. This duty only increases when customers are walking into the store with wet jackets and umbrellas, dripping water all over hard floors. Whether or not the substance Ms. Metroplex slipped on was rainwater, the employees should have either noticed and corrected the dangerous substance Ms. Metroplex slipped on, or they should’ve warned customers, including the plaintiff, about the condition of the tile floor. Neither Ms. Metroplex nor Ms. Arlington made any mention of “Wet Floor” warning signs around the entrance to the store, and the substance was present near the entrance when Ms. Metroplex slipped. Thus, Forever 21 breached its duties to discover, correct and warn of a clear and present risk to customers. There are two potential causes for the fall: Ms. Metroplex’s cork wedges and the oily substance on the floor. We will argue that the oily substance caused the fall. The defense will argue that Ms. Metroplex’s cork wedges were the cause. A third argument could be that Ms. Metroplex began to fall due to her wedge shoes, but that she could’ve recovered from the imminent fall if not for the oily substance she then slipped in.
My fieldwork took place in Miami Beach, Florida. I decided to focus my observations on the pool deck of the Riu Hotel. I chose the pool because it was the most central location, which had a very large and diverse group of subjects. As directed I spent thirty minutes observing the pool deck. My first observation was from 8:50 a.m. to 9:20 a.m., it was 77 degrees, partly cloudy and I sat in the back right hand corner of the pool deck facing the beach. My second observation took place from 8:43p.m. to 9:13 p.m., it was 73 degrees and mostly cloudy. I sat in the same spot as my first observation. The third observation was at 2:09 p.m. to 2:39 p.m., it was 83 degrees, mostly sunny and I sat in the back row to the far left. While taking these notes,
On April 05, 2016 at approximately 1518 hours, I was dispatch to 2004 Pine Street in reference to a death investigation. Upon my arrival, I met with Detective Campbell who was the lead Investigator at the scene. Detective Campbell stated that the victim neighbors reported a strong smell coming from the victim’s (Cortiz Lazarz) residence. The neighbors called 911 and Officer Volk was the first responded officer on scene. Defective Campbell and officer Volk stated that the victim located on the living room floor, fazing the south from the residence. Detective Campbell requested I process the scene for evidence and documentation.
