On Friday, June 10, 2016 at about 2332 hours, FHEO Security was dispatched for a (51D) Disorderly Patient at the Special Care Unit (SCU) room #37. All Security staff responded to the call and met with RN Francesca Plaza who stated that the patient, James Patrick Hogg (FIN: #85758458) was agitated and would not listen to medical staff. Mr. Hogg had come in by EMS with Altered Mental Status (AMS) and was uncooperative with medical staff. Nurse Francesca Plaza had requested Security staff to stand by while the patient get medicated. According to former employee Stephone Else, medication aide, alleges she heard Ms. Lyles tell Phillip Yovonnie, LVN to “Take her to China” with Amara Anele, RN, and Lilian Foster, Medication aide present at 6:20-6:30am. Per the investigation, the date was identified as 1/23/18 and resident was Shirley Depino. According to the nurse Amara, she did not give any medications
Trey Cavalier was assigned to M&N control for the night. That morning (which was May 14, 2016 because he was working nights) about 30 minutes to an hour (he could not give exact times because log books were being picked up for the investigation) before 4:00 a.m. Dy. Cavalier was notified by Dy. W. Benson that Brian Ducre (who was a new move that was escorted to M&N on a wheel chair) was acting strange and that medical needed to see him. Dy. Cavalier called the medical department and spoke with Nurse Minor and advised him of the situation. Shortly after Nurse Minor came off of N01 and said that inmate Ducre had mental problems and wasn’t taking his medicine, that there was nothing the Nurse could do for Ducre and that he would ultimately be okay. Castillo kept acting in belligerent manner and stating that he was going to leave. I, Steven Evans then spoke to him about the fact that he appeared to be intoxicated and that he could not leave at this time. I reiterated to him that any attempts at leaving would not work and he could possibly end up being restrained by medical staff with Security assistance. The patient did not like what I said to him, so he threatened to leave and then proceeded to step out of his room. Security Officer Alonso and I asked him twice to go back to his bed, at which time he became unreceptive and refusing to go back to his room. Officer Alonso and I had to physically and forcefully direct him back to his bed. Once on the bed, he became physically aggressive and attempted to hit Officer Alonso. We instantly took control of the patient's arms and upper body while Security Officers Paz and Weiland controlled his legs as he kept screaming and fighting with us. Nurse Baptiste proceeded to contact the patient's Doctor Cleveland so that a sedative could be given to him. He was brought in by his friend Gavriel Martinez. At 2338 hours Orange County Sheriff's Deputies Kenneth Jorgensen (Badge# 8180) and Deputy Michael Fairley (Badge# 6210) arrived and interviewed the GSW patient, both deputies were already on site for an unrelated event. Security Manager Richard King was informed by text of the incident at 2340 hours as well as Nursing Supervisor Debra Reilly. At 2350 hours OCSO Deputy Kristine Helms (Badge# 5021) arrived and requested to speak to the patient’s friends who was waiting in the E.D. lobby. At 0010 hours Deputy Michael Fairley contacted SO Alonso and advised that it was OK to open the Emergency Department. The lockdown was lifted and Security Manager and Nurse Supervisor were informed of the incident. On Friday, September 30, 2016 at approximately 0536 hours, a Black Male Stab Wounds Victim walked into the Emergency Department lobby with a severe Stab wounds laceration to the lower leg and a human bite wound. Security staff was contacted by E.D. triage Nurse Tony Rusiano and escorted the patient to ED room#42. Security conferred with E.D. Charge Nurse, Johnathan Bacal and an E.D. Lock down was initiated at 0538 hours. All Security Officers responded and dispatched over front and back E.D. entrances. On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
MICU 15A was dispatched to 30 West Ave, @ Genesis Healthcare, Wayne Center in the Wayne Business District in Radnor Township, for an ALS Emergency, Assault Victim. The weather conditions were cloudy, cool and dry. MICU 15A responded with care per protocol to the stated location with EMT Straub driving. On arrival, the EMS Crew proceeded to the nurse's station, and the individuals there did not know anything about an incident, there was a female standing in one of the hallways who advised that there was a person sitting over where she was. The EMS crew proceeded to that area and found a 43-year old female who was a registered nurse and was conscious and alert x4 and was sitting on a chair, the EMS crew asked her what was happening, "she replied that she was in a patient room and stated that the patient had an IV in her arm and became disturbed and picked up a pocketbook and then assaulted her by the swing it and striking her in the right frontal region of the head", the nurse then exited the room and went and sat down in the hallway, where the EMS crew found her.
On Thursday 07/21/2016 at approximately 2223 hours, Security Supervisor Steven Evans was contacted by Assistant Nurse Manager Robbie Philips via landline and asked to conduct a (44V) Enforcement Escort Visitor Off Property for the discharged female patient in E.D. #48. The patient, Susan Harris (DOB: 03/22/1952), had been quarrelsome and refusing to leave. SOs Christopher Paz and Ariel Weiland responded to the scene. Upon arrival, we observed the patient laying down on her bed, we approached the discharged patient and spoke with her. Mrs. Harris agreed to leave without further incident. On Saturday, December 26, 2015 at 2029 hours. FHEO Security Officers were dispatched to the East tower, room #409 bed 2 for a Disorderly Patient (53D) who was combative with nursing staff. All Security staff responded and on arrival we were met by Administrative Nursing Supervisor Mary Cisco and Charge Nurse Darren Trofibio outside of room 409. The Charge Nurse, Daren explained to us that patient’s boyfriend who was later identified as Monty Monroe was very aggressive and threatened him with bodily fluid and to punch him. On August 26, 2015 around 2335 hours, Security Supervisor Steven Evans, Security Officer Allan Topher, and I was dispatched to room 5104 in response to a non-Baker Act patient, Lillie M. Smith (FIN:84487321) who was arguing with medical staff. Upon arrival, we made contact with Nurse Julia, who stated that Mrs. Smith did not want to leave her room. She further stated that Mrs. Smith was going to be transported to room 510 bed #2. Security staff went inside the room and observed a very confused 86 years old patient who stated that she pays rent in the hospital and that the hospital is her house. At approximately 11:36 hours, the writer and S/O C.Mason responded to a call from POC to attend MHSU for an eviction. The officers attended and met with the clinical staff (Tav Randhawa), who informed the officers that the discharge patient in consult room #9, need to be escorted off property. The officers were also told that the patient is really agitated and might put into a fight. The writer requested for two additional officers to attend and assist them with the call. S/O C.Mason broke up from the call and returned to the main booth to relieve SSS J.McCuaig, so he can attend.
Donald Johnson was agitated by this but understood that his father was being transported. After getting off of the phone with Donald Johnson S/O EMT Perez talked to S/O EMT Anti to which he stated that the aide did not tell him about any sort of special blood pressure medication or any special processes that accompanied this patient. S/O EMT Perez was escorting 911 off campus when S/O EMT Anti stated that that Donald Johnson wanted his father to take the medication before leaving the campus. S/O EMT Perez stated to S/O EMT Anti that he just got off the phone with Donald and stated to him that his father was being transported and that he is being brought to the hospital to seek further medical care and that 911 would not delay patient care by waiting for medication. The patient, Rachel Gardner (DOB: 12/12/1985; FIN# 85335948) was uncooperative and not willing to go voluntarily to the Lakeside Psychiatric Facility transport van. She stated that she was missing her clothing, shoes, and purse. Security Officer Alonso met with Charge Nurse Tonya Smith who could not find any information in the patient's chart about personal belongings logged in. The nurse kept answering Patient B that they needed her bed because she was no longer telemetry monitored and the bed was required to monitor another cardiac patient. Patient B, thinking that she was the first patient in that room and Patient A is no longer telemetry monitored also, responded by asking why can 't Patient A be move instead. The nurse abruptly replied, "Due to the possibility of Patient A having C-diff, we have to move you as a precautionary measure." This explanation occurred within the earshot of Patient A. Patient A became visibly upset and tearful, and vocalized that she believes her rights to privacy of her medical condition were violated. Patient B was moved to another room within the same unit. Subsequently, Patient A asked for another nurse to be assigned to her. However, to add fuel to the flame, in the process of providing bedside report to the replacement nurse, the outgoing nurse mentioned Patient A 's new cancer diagnosis in front of Patient A 's family. The patient, once again became visibly upset and tearful because she hadn 't had the chance to talk to her family about her terminal cancer diagnosis after speaking with her physician earlier that morning. On several occasions, the patient complained about her privacy being violated and threatened to file a lawsuit against the nurse and QMC for violating her rights.
