Hello Dr. Ullom, majority of the long term care facilities are under staffed. There is usually one registered nurse in charge to manage a 240 bed facility, with LPN's and nursing assistance. I feel that these patients would benefit from having one RN to every six -eight patients with a nursing assistance. Not only would this benefit the patient but the nurse as well. Patient are placed in long term care facility with a certain problem, but ends up with additional condition such as UTI, MRSA, pressure ulcers, and etc. These issues are related to poor care they receive because of unstaffing. I'm not placing blame on the LPN or nursing assistance, but with a RN and low nurse to patient ratio, they will receive better care.

Howard Terry Newman is 67 year old retired printer who suffered from Parkinson’s disease as well as arthritis, voice impairment, hiatal hernia, and heart trouble. Mr. Newman, the appellee, is contesting his stay between September 19, 1968 and November 11, 1968 at the appellant’s facility, Big Town Nursing Home. The plaintiff was admitted voluntarily by his nephew to the facility with the ability to leave as he pleased. However, on September 22nd, when Mr. Newman attempted to leave the facility, he was told that he could not use the phone or entertain visitors without the administrator’s permission. He proceeded to walk out; he was brought back and restrained on Wing 3 with “senile patients, drug addicts, alcoholics, mentally disturbed, incorrigibles and uncontrollables” according to the administrator testimony.

He is diagnosed with type 2 diabetes and is prescribed insulin four times a day. Nursing staff from Care Unlimited Home Health come to the house every day to check his blood sugar, check his blood pressure, and administer insulin as ordered. However, Jesus is reported to often refuse to have blood sugar checked and to have insulin shots. This noncompliant behavior to treatment poses a high risk for complications from poorly controlled diabetes.
Correct Care Solutions, LLC. (CCS) is contracted to provide medical, dental and mental health services to the correctional population housed at the Mecklenburg County Sheriff’s Office (MCSO). The SOW agreement between MCSO and CCS is dated November 1, 2008. Per the agreement, a quality assurance plan requires an “outside” correctional health professional to conduct a peer review of inmate health care services with documentation of deficiencies and plans of corrective actions on an annual basis. The American Correctional Association (ACA) was contacted to conduct a compliance audit of the inmate health services operation for SOW agreement between MCSO and CCS.

Member stating she had requested Encompass Home Health Agency but Bon Secours Home Health came out on 03/18/2017 and she stated she told Nurse she did not want that Agency. CM placed a call to Hanover Health and Rehab and spoke with Discharge Planner and she stated that Encompass could not take the case because of a shortage of staff. Member stated she would agree to have Bon Secours Home Health. Discharge Planner stated she would send the referral back to Bon Secours Home Health. Member stating she had post hospital follow up with PCP yesterday with no new changes. Member stating she reported the pain she is having to PCP
