UCM:CPSW did a home visit and met with Ms. Tina and all her children at her home. This worker interdouced myself as the ongoing case worker. CPSW talked about resources and services. Ms. Tina stated that her children missed schools because she needs child care for the younger children. CPSW talked about in home parenting referrals and children attending school regulary. Ms. Tina stated that her son Cedric has difficulties with paying attention. She reported that he may have attention defciate. CPSW did in home referral for Ms. Tina.
UCM:CPSW received a text from Morgan (Fits) reporting that she has received the referral from Amanda and that she is available on Monday's and Friday's at 11-1pm. Amanda will start on 1/28/17 at 11am Brookdale location. Also, CPSW called foster parent and Amanda about Monday appointment and that Morgan will contact them regarding supervisied visit. Goal 1-2
Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice.
Focus: Ms. Smalls (MHP) administered the Casey Life skills assessment, the Circle of support and the Child and Adolescent Needs and Strengths assessment with the assistance of Ms. Givens (MHS) and Antazia.
Social Services Meeting: On 03/01/2017, Ms. Hawkins and her daughter Emoni met with her assigned Case Manager for the family ILP Document Review. Ms. Hawkins’ next ILP Document Review appointment is on 03/14/2017. Ms. Hawkins is in-compliance with the terms of her ILP. Ms. Hawkins was reminded that she is expected to attend all scheduled meetings with assigned Case Manager and failed to do it would considered non-compliance and warning will be issued. Ms. Hawkins stated that she is aware. Case Manager asked Ms. Hawkins if there are any issues or concerns that she will like to discuss, Ms. Hawkins stated no.
Case management services were provided in garden Grove. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo. Service minutes are high due to WYP Ngo attempting to meet the youth. WYP Ngo reminded the youth of the meeting during the after noon through a phone call. The youth acknowledged the meeting and will be waiting for WYP Ngo. WYP Ngo arrived at the youth’s home. WYP Ngo contacted the youth through a phone call, but the youth hung up on WYP Ngo. WYP Ngo waited for the youth to show up. WYP Ngo contacted the youth again, but the youth did not pick up. WYP Ngo contacted WCC (Wraparound Care Coordinator), WPP (Wraparound Parent Partner) Lyneth Torres, and SUP (Wraparound Supervisor) Sheila Gaston-Cruz to inform them the youth
Intervention: Ms. Smalls (MHP), Mrs. Clark (MHS) and Taliana discussed Taliana adjusting to the home. MHP encouraged Taliana to ask questions if she does not understand something. MHP praised for willing to communicate with MHS and family. MHP, MHS and Taliana discussed Taliana’s emotions about family visits and no contact with biological mother. MHP praised Taliana expresses her feelings. MHP explained to Taliana and MHS that the DSS case worker will visit next week. MHP provide an update from DSS worker about clothing and eyeglasses. MHP administrated the Casey Life skills. MHP allowed Taliana to share stories about her biological mother and family.

On September 5th, the worker completed a home visit with The McClellan family and revised the safety plan. Upon arrival, the worker was greeted by Mr. McClellan. The worker went over the revised safety allowing Mr. McClellan back in the home with supervised contact with Hailey and Ja’Shonna. The McClellans were excited about the father returning home. The worker went over the expectation and requirements from Ja’shonna and Mr. McClellan (substance abuse treatment and drug screen. Home visit continued transited with Mrs. McClellan and Hailey. The worker noticed Hailey is gaining weight from the first time she saw her. According to Mrs. McClellan Hailey is doing well. The worker reminded Mrs. McClellan of Hailey’s sickle cell appointment.
She informed the family that the CPW CW had changed to Tamika Harvey and she provided me and the family with contact information. We discussed the concerns that the Garcia family shared in regards to the case. The family reported that they feel that they are the “bad guys” in the situation and they reported that they do not understand what is going on. The family discussed that the previous CPS CW has been extremely rude to them and during their last phone call she was yelling at them. The family reported that this has been a very hard walk for them. We also discussed the latest conversation in regards to the biweekly visits with the maternal family. The Garcia family reported that they have very busy schedules and they cannot make any promises to be available for transportation during each visit. I informed the family of my earlier discussion with the CPS CW and also informed them that Angelheart had suggested moving the meeting times on Friday to occur during business hours to allow Angelheart to help with that burden for transporting. UCM:CPSW spoke with Ms. Barksdale regarding her case plan and staying consistent with her UA's. Ms. Barksdale has not been consistent with her UA's and intaially we started color code Black and was changed to Orange. Her last UA's on 2/6/17 Positive Alcohol. CPSW called Ms. Barksdale and reminded to stay consistent with her UA's. Ms. Barksdale stated that she does not want to complete UA's anymore. CPSW reminded if she misses twice or positive than rule 25 referral needs to be completed . Goal 1-2
Action: CSP and Clinician introduced new case coordinator to MHS. CSP, Clinician and MHS discussed missed medical appointments. MHS explained the Medicaid transportation services did not pick her and the youth up for the scheduled appointment. MHS stated she received the letter from the DSS worker for Juliet’s medical physicians. CSP explained MHS must get the MUSC summaries updated with the correct information. MHS cannot no longer receive verbal instructions. MHS, Clinician and MHS discussed Juliet’s WIC appointment and the use of power milk. MHS explained Juliet is making milestones and responses to the voices of family members.
UCM: CPSW did a home visit and met with Ms. Barksdale and her son Calvin at her home. Calvin was playing and watching tv during the visit. Ms. Barksdale stated that she has completed her parenting assessment and did not complete parent and child observation because her attorney recommended not to complete one. Ms. Barksdale stated that she has documents stating that she does not need to complete her parent and child observation. CPSW requested the documetns from Ms. Barksdale and she stated that she has lost the docuemtns. Also, CPSW talked about her progress on UA's. Ms. Barksdale stated that she has been calling everyday about her color and has not missed any UA so far. CPSW reminded Ms. Barksdale about her next pre-hearing court date on
