RECONSTRUCTION:
Pre-Crash:
On 05-25-2015 at approximately 1940 hours, Vehicle 1 was westbound on County Road 672, east of Dupree Road. The driver of Vehicle 1, for reasons unknown, drove off the north side of CR 672 and onto a grassy shoulder. Vehicle 1 drove approximately 229 feet through the grass before he returned to the roadway. In returning to the roadway, Driver 1 over-corrected his steering and continued in a southwest direction across both lanes of CR 672, for approximately 76 feet, as Vehicle 1 rotated slightly in a counterclockwise direction. As Vehicle 1 left the south road edge, Driver 1 again tried to over-correct and began to rotate in a clockwise direction, as he drove onto the grassy shoulder.

Hillsborough County Fire Rescue responded to the scene and pronounced Driver 1, deceased at the scene.
Patrol Deputies closed both east and westbound lanes of traffic on CR 672 west of Dupree Road, blocking the roadway from vehicular traffic.

Upon looking at the marks, one could see some of the grass was laid down in the direction Vehicle 1 was travelling, which would be an indication of a rolling tire. I was however, unable to make a determination as to whether or not the tire marks were made by Vehicle 1, accelerating or decelerating through the grass. If I was to make an inference that Driver 1 was neither braking or accelerating and merely rolling, one could use a rolling resistance of .07, for firm soil, (REC-TEC), for each rolling tire for Vehicle 1. For the distance of approximately 229 feet, there would have been a speed loss of approximately 43.85 miles per hour.
After Vehicle 1 left the north shoulder and re-entered the roadway, it left a tire mark, that continued to the south road edge. there were no obvious striation indicating a yaw mar, so I used a rolling resistance of .015 for smooth asphalt, (REC-TEC), for each rolling tire. At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. Trooper Otten said Mr. Wenzel admitted to having been drinking prior to the collision and was suspected of driving under the influence. All three occupants had been transported from the scene

Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. On 02-01-2017at approximately 1312 hours, a 2012 Ford Fusion being operated by Jon Molvie was eastbound on Fishhawk Blvd., at approximately 45mph, approaching the intersection of Bayberry Glen. Molvie was approximately 4 - 4.5 car lengths behind the HCSO marked squad car, when the HCSO vehicle, braked suddenly and began a U-turn. Molvie was unable to brake or take evasive action and struck the HCSO vehicle with the front of his car. The Ford Fusion came to rest within the eastbound lane of Fishhawk Blvd. facing in a southeast direction.
While on patrol, I was heading northbound on North Three Notch Street. While traveling, I observed a burgundy Chevy Tahoe traveling southbound in the northbound side of traffic, approaching my patrol vehicle. To keep from being involed in a head on collision, I pulled my patrol vehicle into the Sunoco on North Three Notch Street. I then engaged my lights and siren and attempt to catch up to the violator vehicle. The Tahoe then pulled into the Piggly Wiggly parking lot and that was the final. I then identified the driver as William Anthony Thomas. I advised Thomas that he alost hit me head on and he stated, "I'm sorry, I am too fucked up." Thomas had an strong odor of an alcoholic beverage coming from his expelled breath. Thomas also had red glossy eyes. I then advised Thomas multiple times to step out of the vehicle to perform FST's. I noticed that Thomas had unsteady balance as he was waling to the back of the vehicle.
On July 22nd I was driving North on Lambert Road in Glen Ellyn about to make a left onto Roosevelt Road. I was in a Enterprise Car Share vehicle that is a 2012 silver Toyota Yaris. As I approached Roosevelt Road there was another vehicle in which case it was a black Toyota. This Toyota sideswiped me as the driver said she was distracted and did not notice my car in the left turning lane as she was in the right lane trying to get over. After looking at her vehicle it appeared most of the damage came from her driver side mirror and right above the rear driver side tire on the black Toyota. Therefore it damaged the passenger side of the Enterprise Car Share vehicle.
SE on the north side of the road. I observed about 50 yards of tire tracks in the mud leading up to the final resting place of the BMW. The tracks were located in the mud and noticed on the sidewalk leading to the collision. The tracks indicated that the vehicle was moving northbound on 116th AVE SE.
While on duty, I, Officer J. Daniszewski, was traveling eastbound on W Westover St. As I was stopped at the intersection of W Westover St and Sawyer St. the suspect vehicle passed me traveling westbound on W Westover St. As the suspect vehicle passed me, I looked in my rear view mirror and could not see a license plate on the suspect vehicle. I turned around and positioned my patrol cruiser behind the suspect vehicle. As I got behind the suspect vehicle, I could not identify a license plate on the back of the vehicle. I turned on my emergency lights and initiated a traffic stop. The suspect vehicle stopped on W Westover St, just east of Roundhouse Ave.
The accident occurred on Monday, June 22, 2015 . The claimant was driving a 2008, 4 door Jeep. She owned the vehicle and was wearing a seat belt. The accident occurred on the Deegan Expressway while going southbound, near exit 7 where she entered the highway. She was in the right lane of 3 lanes and about 30 seconds
On July 20, 2016 at 19:00 I found Joseph Grass driving 15 mph in a 25 mph zone. After reaching the next intersection, Joseph ran the stop sign and continued down the road. I pulled in behind him and made a traffic stop on his truck. When I reached the window of his truck he was slumped over, behind the wheel, unlawfully in the middle of the road. I attempted to get Joseph’s attention by putting my flash light in his face, but that would not rouse him.
V1 and V2 both stoped in red light on the right turning lane of Renassiance West. V1 did not see V2 still stopped in red light and struck V2 from behind. Both vehicle had no apparent damages. The crash is non reportable at this time. Both parties exchanged information to resolve the matter among themselves at this time. V1 is at fault for this crash.
Not all the evidence collected were useful for the calculations. The fact that the car was going around a left hand corner helps to visualize the car in the right position but it does not provide any helpful information to use in the calculations. Another detail is that the car was traveling 10° west of north when it lost control but this information is not helpful since it does not provide any useful application for the kinematic calculations. Also, the crash occurred at approximate 2:40 pm which does not help us with the calculations since the crash time does not provide us with any specific information that could be used in the kinematic calculations . Another piece of information is that the engine is capable of 340 horsepower and this detail is not helpful since we do not know if the car was using its maximum engine capabilities. The posted
