He made the adjustments after Todd's visit and m/i inspection that was completed 5 months later by FC. I recommend that we adjust his bill with the condition that he pays off the balance right away.

Shadale Gillespie is a Direct Care Provider for our agency since February 2015. Work is not conistant due to the amount of Respite hours authorized by the AZ DDD department for the child she services. The authorization avaialble hours have ran out and she has not been able to work with the child since July 9, 2017. Last paycheck on July 25, 2017 for gross amount of $400.00. Bi-monthly pay periods, rate of pay $10.00/hour on average of 10 to 20 hours per week when we have authrizations available to deliver services.

Keith Foster a former deputy police chief was sentenced to four years in prison for being a corrupt police officer that was profiting off of selling drugs. I do believe that since he is an officer of all that they gave him some sort of leeway. For example, Foster had three days to show up at the Denver prison by himself. If I were to get arrested I would have to be processed with the rest of the felons and be transported on a bus with other inmates. The fact that he was able to take himself shows that he already has special treatment, just because the title he used to have. I think that the reason why he was able to go on his own to prison is because he is a former police officer and he mostly has a higher risk of getting hurt while in prison.

UCM: CPSW did a home visit again and talked with Annette regarding her relationship with her mother Ms. Clifton. Annette stated that some times that they both disagrees on things and that and than Annette gets upset with her mother. CPSW talked about therapy with Annette and Ms. Clifton. Annette looked at her mother when this therapy talked about family therapy with her mother Ms. Clifton. Ms. Clifton refused to participate with Annette any family therapy. Ms. Clifton stated that Annette has anger issues and that they don't need family therapy at this time. Annette was very quite and mostly was looking her mother's responses regarding family therapy. CPSW told Ms. Clifton andf Annette to think about it and that we will talk about it on next
I presented Esteban’s case during case consultation because I noticed that I had countertransference when doing his intake. My expectation when presenting this case was to receive feedback that would help me to provide effective treatment to Esteban without my countertransference affecting his treatment. This section of the paper will talk about the feedback I received about my countertransference and how to support Esteban.
After further review of Claude Michaelis current Peoples gas bill. He owes a total amount of 96.81 of which 73.19 is passed due. At this point Claude is in the process of applying for employment, but is unable to keep up with his increasing balance on his cooking gas. Currently, HUD is paying $5.00 per month on this bill, but it has barely made a dent in the deposits and service fees that are attached to this account. Today, we are asking for 96.81 to cover Mr. Michaelis current people’s gas account.
