Community development is used to target issues such as environment issues, housing, crime, pollution, support and isolation. For this paper, Ms. Minister Melishia Bansa MPH was interviewed. She is a thirty year old African-American woman. Her occupation is the director of community outreach and provider Relations for “Coordination of Health Care for Complex Kids” (CHECK) Program and Project Coordinator of “Promoting Adolescent Health” (PATH) Study. She was interviewed about her work with the CHECK Program. The Coordination of Health Care for Complex Kids program aims to improve overall health and quality of life for underserved populations. This programs aims to fight the social injustice of unequal health systems for low socioeconomic communities. The CHECK program aims to increase doctor appointments and improve communication between the patient and the patient’s health provider. However, the program is focusing on young individuals with chronic illnesses within low socioeconomic communities.

Community health nursing is a vocation; striving to improve the health of the community by promoting, stabilizing and protecting the health of individuals, families and identified aggregates. We are able to have the furthest reach when we can isolate a specific deficit of an aggregate through assessment and subsequently the development of a community support program. Community support programs promote health, access to services and self-advocacy.

As a community health nurse, I would be most interested in a focus group to assist elderly residents in developing emergency and disaster preparedness plans. I choose this population because elderly citizens face great health and safety risks when emergencies and disasters strike (Clark et al., 2003). It is important for health care practitioners to understand the needs of the elderly in the communities they serve (Barratt, 2007). There may be a need to quickly evacuate the local area and elderly residents may lack proper transportation. Some seniors may become confined to their homes in a disaster. Others may not know what to do if they lose services such as water, heat, electricity and basic communication. Proper advance planning can mean the difference between survival and suffering or death.

The purpose of this paper is to identify one health problem that is most prevalent in my community. Residential demographic information will be provided. I will use Milio's framework for prevention to analyze this health problem. Then, compare the health problem to Healthy People 2020 national goals. Lastly, I will introduce one long term goal and three short term goals with their interventions.

A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. The elderly have multiple health problems and struggle to stay healthy and able to care for their own needs. The elderly in the rural areas tend to have a number of needs that may be compounded because of their choice of dwellings. The rural person may need to seek health care in different nearby cities, or may find some health care providers in their own local communities. Many elderly or older adults would benefit from a health promotion plan that would focus on the needs of this age group. The components that this plan would focus on includes physical activity, proper nutrition, and coping with mental stressors.
