Also is a linkage between the public health and social services, it helps to improve the quality of life within the community. Community health workers work with individuals and with communities, they build the capacity of health knowledge and self-sufficiency through a range of activities such as health outreach, community education, social support and
This author’s personal perceptions concerning patients facing a lingering terminal illness, have been shaped by over 20 years of critical care nursing experience. Facing death and illness on a daily basis requires self-examination and a high degree of comfort with one’s own mortality, limits and values. Constant exposure to the fragility of life forces respect for the whole person and the people who love them. A general approach to patients who are actively dying is to allow them to define what they want and need during this time. The nurse’s role

In the United States we are very fortunate. For the most part we do not live in fear of disease outbreaks from day to day. Our government along with the Centers for Disease Control and Public Health Departments have managed to have adults and children vaccinated and set up recommendations regarding those vaccinations. Despite all that is done our country allows freedoms. One of these freedoms is to not vaccinate your child from communicable diseases due to personal beliefs. With the worlds populations becoming more mobile, that sets your child up for dangerous encounters; because some countries are not as strict on laws regarding vaccinations and some
Measles was discovered in the 19th century by a Persian Doctor, but was not recognized until 1957 as an infectious agent in human blood by a Scottish physician. In 1912 measles became a notifiable disease and in the first decade that records were kept there was on average 6,000 deaths per year reported from the disease. (Center For Disease Control). There is no definite origination of measles but scientists believe that it dates back to the Roman Empire about the 11th and 12th century (NCBI, 2010). The first outbreak known in America was in 1657 in Boston,

Community health nursing is a vocation; striving to improve the health of the community by promoting, stabilizing and protecting the health of individuals, families and identified aggregates. We are able to have the furthest reach when we can isolate a specific deficit of an aggregate through assessment and subsequently the development of a community support program. Community support programs promote health, access to services and self-advocacy.
As a community health nurse, I would be most interested in a focus group to assist elderly residents in developing emergency and disaster preparedness plans. I choose this population because elderly citizens face great health and safety risks when emergencies and disasters strike (Clark et al., 2003). It is important for health care practitioners to understand the needs of the elderly in the communities they serve (Barratt, 2007). There may be a need to quickly evacuate the local area and elderly residents may lack proper transportation. Some seniors may become confined to their homes in a disaster. Others may not know what to do if they lose services such as water, heat, electricity and basic communication. Proper advance planning can mean the difference between survival and suffering or death.
The purpose of this paper is to identify one health problem that is most prevalent in my community. Residential demographic information will be provided. I will use Milio's framework for prevention to analyze this health problem. Then, compare the health problem to Healthy People 2020 national goals. Lastly, I will introduce one long term goal and three short term goals with their interventions.
Community health is the process or characteristics that allow a community’s population and economy to survive. Community health is defined as the meeting of collective needs through identification of problems and
A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. It also identifies those areas where better information is desired, particularly information on health differences amid different subpopulations, quality of health care, and the incidence and severity of disabilities in the population. The Community Health Assessment is the foundation for all local public health development, giving the local health component the instance to recognize and network with key community leaders, businesses and concerned residents about health priorities and concerns. This information shapes the foundation of improving the health status of the community by way of a strategic plan (The Municipal Public Health Services Plan Community Health Assessment Guidance and Format, n.d.).
Community health nurses find out information about the community’s involvement and knowledge. They use their clinical understanding and experience to bring information to the community’s about different health issues in that area. For example if HIV was increasing in their area they would collect the numbers and data and bring it to the attention of other health care agencies to bring
The elderly have multiple health problems and struggle to stay healthy and able to care for their own needs. The elderly in the rural areas tend to have a number of needs that may be compounded because of their choice of dwellings. The rural person may need to seek health care in different nearby cities, or may find some health care providers in their own local communities. Many elderly or older adults would benefit from a health promotion plan that would focus on the needs of this age group. The components that this plan would focus on includes physical activity, proper nutrition, and coping with mental stressors. While all older adults could potential benefit from this plan, this plan will be offered primarily in rural communities in order
