Field Safety Observations
Expectations:
McCutcheon Enterprises, Inc. has implemented the use of Field Safety Observations and they are to be used on any job at any time. It need not matter whether or not the job was scheduled, or emergency response, Field Safety Observations may still be completed.
Outline:
MEI encourages all of our employees to observe and mitigate hazards in the work place. This is achieved through by using the Field Safety Observation Process. The focus of the observation is on the work task being performed and how employees react to it. Potential hazards and concerns are shared and discussed as well. The benefits of such discussions are:
Immediate positive reinforcement of the tasks performed which can dramatically increase the chance that correct behavior will be repeated and become habits.
Prompt feedback and correction of questionable activities or conditions. The constructive criticism reinforces to the personnel that performing the job safely is their highest priority.
Methods asking for recommendations to improve the safety of the job being performed enable personnel to be involved in the entire observation process.
Finally, the discussions are a tool used to track and identify both positive and negative safety trends. Combining the trends of the two is most effective in changing behavior for incident-free operations. Observation data enables the organization to focus proactively (bottom of pyramid figure 1) and note those key areas where greater risks are present.

MEI can improve this process during bi-monthly company-wide safety meetings and through the meetings of the Safety Committee. MEI Supervisors shall assist all employees in the proper completion of FSO forms. MEI Safety Personnel can also assist an employee when on field visits or whenever the employee returns to the office. The observer must be trained so that they can:
Note work practices that are correct and consistent with the onsite JSA.
Identify potential deviations from standard job procedures that could possibly result in an incident.
Assist in the development of sound solutions to prevent the root cause from recurring.
Tracking and Trending Data
MEI employees submit their completed Field Safety Observation Forms to the MEI Safety Department for review. The MEI Safety Department then takes the reports and classifies them under the 4 major categories:
Recognition
Unsafe Acts
Unsafe Conditions
Stop Work Authority
The MEI Safety Department then further breaks down the data to include Critical Factors such
Employers, managers and supervisors should all ensure that workplace practices reflect the risk assessment and safety statement. Behavior, the way in which everyone works, must reflect the safe working practices laid down in these documents. Supervisory checks and audits should be carried out to determine how well the aims set down are being achieved. Corrective action should be taken when required. 3.1 Interpret and comply with given instructions to maintain safe systems of work and quality working practices.
Changing circumstances within the workplace can cause hazards because other workers can be put at risk because they haven’t been made aware of the changes and management needs to be informed. Risk assessment is a method put in place that is suitable and sufficient to control the risk to health and safety to both employees and other persons who may be affected.
To report potential health and safety risks I would contact my line manager/duty officer/manager, I would report something when I think it could cause possible harm and create a health and safety risk.
Records and statistics will be evaluated by inspection and investigation reports, workers orientations logs, equipment and logbooks, emergency response plans (drills, enforced safety, first aid), supervisory notes, and safety contacts (past and present) The reasoning to evaluate these areas is to ensure due diligence. This ensures the employer is taking responsibility for workers and strives to conserve a safe working environment.
Check we are fulfilling health and safety policies and risk assessment appropriately, keep them up to date and modify them whenever it is necessary.
We can also identify new risks which need to be documented in the service users’ risk assessments in order to prevent accidents or injuries and to alert other colleagues about them.

Identify the standards of hazard administration, recognize arranging writes, and perceive what's associated with the investigation of various dangers and vulnerabilities.
For example if a service user wants to go swimming then you must assess how you’re going to get the service user into the water this could be by a hoist, you must then assess if there are any hazards or risks of doing this task, then if you do find a hazard then you must think of different ways of how you can reduce the risk of it.

Receiving effective information to allow the right information to assist with analyzing the risks involved. The effective information can be broken down in a work breakdown structure and tracked through a checklist. The tools and techniques that will assist in the accurate tracking and monitoring include an activity information checklist, interview methods that involve personnel with their direct work responsibilities. Additional techniques include studies conducted throughout the organization on processes and producers to include lessons learned from past projects. In analyzing risk there are two methods such as qualitative and quantitative risk analysis to assist in making the analysis process straightforward.
Training of supervisors in how to communicate and demonstrate safety practices to employees (Gomez-Melia et al., 2011).
