Primary care teams and Long Term care teams
There are many different types of interprofessional teams in our health care facilities. Scientific research evidence has shown that while long term care and primary care teams differ in some methods of care delivery, their concepts and focus are to provide a high functioning team, care managing and interprofessional communication and collaboration with patients, residents and families. Primary Care Teams, (PCTs) provide a defined population, (primary’ or first point of contact with the health service), a holistic approach to community needs on a social, welfare, housing, education and health needs basis and wellness Ergen, L. The final course project was creating an interprofessional team (IPT), function as a leader and take part in the stages of group development, setting of the team roles and goals, identifying communication patterns, and team analysis to discuss an issue at the microsystem level at our facility. The issue that is being addressed is post-transplant steroid associated hyperglycemia. For patients that have new onset diabetes after transplant (NODAT) whether immediately post-operative period or during a rejection episode is an issue that needs attention. The patient has a poor understanding of the blood sugar monitoring, insulin administering, and dieting. A date, time, and location was secured that was conducive to the team members schedule.
Working in a team is an important responsibility by understanding each other’s role which may include doctor, nurse, occupational therapist, physiotherapist and many more. Team members divide the work based on their scope practice such as acute care, metal health care, homecare etc. Interprofessional collaboration practice is decision making and communicating between individuals for their patient’s health based on their knowledge and skills. It helps to promote habits, maximizing health resources, leading care to be safer with patient’s satisfaction and Canada’s health care (Kenaszchuk, Reeves, Nicholas, & Zwarenstein, 2010).
Literature suggests that stronger collaborative relationships across healthcare disciplines is associated with improved patient safety, quality of care, and outcomes. For example, Tomblin Murphy et al found that when collaborative models of care initiatives were supported staff used evidence to develop care plans. Additionally, patients and their families were more involved when collaborative teams were in place (Chan A.K., Wood V., 2010; Tomblin, M. G., MacKenzie, A., Alder, R., & Cruickshank, C. 2013; Bethea, D. P., Holland, C. A., & Reddick, B. K. 2014). Often times, however, practitioner groups are reluctant to work closely with others from different professions for a variety of reasons, but mainly because they are not familiar with others scopes of practice and/or methods of care delivery (Vedam et al, 2014). Not all practitioners are ready to trust each other in a team setting and knowing who is and who is not to trust, may have an impact on successful collaboration for individuals over time. Additionally, interprofessional communication is a critical pillar for successful team delivered care. Interprofessional teams need participants who are not only excellent communicators but excellent listeners as well. Skilful and disciplined communication among and between individual practitioners is needed to carry out team-based goals and objectives.

Care coordination within health care systems ensures the client of an effective and short stay. Care coordination refers to the coordination between and among professional teams that serve valuable roles involved in providing care to clients. Nowadays, teamwork is an incredibly important part of an organization. As defined in “Fundamental of management”, work teams are “groups whose members work intensely on specific, common goals using there positive synergy, individual and mutual accountability, and complementary skills” (Robbins, DeCenzo and Coulter, 2015, p.311). To simply understand, every time when we share a specific job with other people, we participate in a work team. For myself, working in a team bring about many interesting experiences of the way we interact with other members and deal with work to achieve the last target. Therefore, the chance to cooperate with three other students in my class during the group presentation assignment in last fortnight has left me numerous significant lessons.
In the living or work, it is usually need to work together as a team. This could be an informal event, like a social or a group, or at formal work. The formal work environments often need to co-operate with each other, linking individuals who have some different techniques or similar skills in order to achieve a common goal. Abstract: The main purpose of creating the team is to get an actual and explanatory outcome of the selected topic. Team formation is very important as it can positively affect the team work while creating opportunities for each member to participate and share their viewpoints as well as gathering the feedback and ideas from others. Collecting information, analysis and interpretation of the topic will be more relevant and more useful for a team to rely on. This will not only help the members to have the freedom to work on their respective tasks according to their own, but also help to motivate them to work more effectively and efficiently. The experience of working in a team can provide valuable insights of the real-life business, whereas individual projects come very rarely which makes an individual to work in cross-functional teams. All the members of a group are different from each other, so it will be a good learning experience to gain from each other.
Similarly, one thing I came to learn during the module when in a team working activities is that different values are placed by different individual when it comes to interpersonal relations and hence it has to be considered as far as team working is concerned despite the personal penchant. For example, some of my colleagues during the module – and through extending this logically could mean professional colleagues – made it clear how directedness in interpersonal relationship was important, action made immediately action taken on a deferred approach. On the contrary, different people had preference of the incremental approach towards issues that arose, preferred the delaying in the action to be taken until enough information and method of solution is obtained. Moreover the relationships that was built through the interaction made was highly valued and considered this are the best network for trust building, before dealing with the problem that is specific. In the meantime, other individuals found conform in what was exactly opposite of this, focus towards the dimensions of the issue was of utmost importance to them, and therefore left the dimensions of interpersonal of the issue to sort itself. After my suggestion, I was met with the confrontation from Jisoo Park, one of my fellow directors, that I have attempted to disintegrate the group and I was denigrated as having the intention of dismantling the team unity. That was most certainly not what I had meant, and in many cases, I have felt that words have been put in my own mouth forcefully by my team members. Thus, the power dichotomy was replicated once again and the freedom of expressing one’s own opinion was immediately turned down and bashed upon. There were definitely no equal grounds in speaking one’s mind, hence, there was only conformity to higher ups in this so called humanitarian club on campus.
