SUMMARY:
Between 6/3/2018 and 6/6/2018 CV1 was a victim of rape, oral copulation and domestic violence by her boyfriend S1 Ahmed, Mujib. These incidents occurred at the Mylo Hotel located at 3211 Geneva Ave, Daly City. CV1 transported herself to Stanford Medical center where she was examined and released. She was then transported to the Keller Center for a Sex Abuse Response Team (SART) exam. C.O.R.A was notified. Investigation continuing. NOTIFICATION:
Daly City Police Department received a mandated report of rape from Stanford Medical Center. The reporting party was identified as Nurse Julie Racioppi. This victim had disclosed she was raped at the Mylo motel, located in Daly City.
INVESTIGATION:
On Friday, 06/08/18, at approximately 0933 hours, I was on duty, dressed in full police uniform and driving a marked patrol vehicle, in Daly City. I was dispatched, along with Officer Chittenden, to Stanford Medical Center in Palo Alto, on a report of a rape that occurred at the Mylo Hotel in Daly City between the dates of 06/03/18 and 06/06/18. She stated he left and came back drunk, banging loudly on the door. CV-1 opened the door for S-1 Ahmed. CV-1 said S-1’s mental disorder kicked in, and he forced her to give him oral sex two times the night of 06/03/18. CV-1 said S-1 Ahmed stated to her that he was having an anxiety attack. CV-1 did not know how or when S-1 Ahmed left on the 06/03/18. CV-1 said he came back 06/04/18 and forced vaginal sex four times. CV-1 stated that she told him no. S-1 Ahmed grabbed her throat with one hand. CV-1 stated she did not lose consciousness and that her throat was sore and hurt when she talked. On Thursday 11/12/2015 at 1905 hours, during shift briefing, Officers Jason Peterman and William Miller pass down information concerning what was believed to have been an elopement from CDU by a male Baker Act patient. He was reported missing by Registered Nurse Ophie to Doctor Gomez at approximately 1530 hours. Doctor Gomez who was concerned for the patient’s safety and wellbeing then contacted Primerose Vernet from the Florida Hospital Risk Management Department assigned to our facility and she in turn contacted the Orange County Sheriff's Office and reported the issue.
On September 27th, 2017, we watched a documentary made by Priyanka Boghani called, Rape on the Night Shift, which was made June 23rd, 2015. This document discusses the issues with a large number of women getting raped while doing their jobs, more specifically janitors, who aren’t protected by the company that they work for in the way that they should be. The documentary also goes over how rape is one of the most common crimes to go unreported, and unprosecuted. These rapes still occur because when a rape is reported, companies are taking very little to no action in making their workplace a safer environment and don’t give each employee the necessary education and tools to allow them to stay safe should they ever be in a rough situation. The investigators interview and reach out to many women who have been raped and sexually abused allowing them to tell their stories, including Maria Bojorquez, Maria Magaña, Erika Morales, Leticia Zuniga, and many more.
At approximately 5:28 p.m., I arrived on scene and was briefed by Officer Nicholson. I was provided with the name of the victim Amanda Holmes (white, female DOB 10/17/1982). Officer Nicholson advised me that the victim was last seen by her roommate at approximately 12:00 a.m. I then spoke to the roommate to obtain consent to process the residence.
The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.
In 2006 (Chicago), An 21 year old lady was arrested for erratic behavior at the airport. The commander instructed the officers to take Christina Eilman to the hospital where she can have a psychiatric evaluation. The officers instead took her to jail where she was release the following day into one of the cities highest crime area. Christina wander into a high-rise of the Robert Taylor homes where she was rape and thrown from the seventh floor window. Christina suffered from severe brain injury, shattered pelvis and collapse lung. The injuries have left Christina mentally impaired for the rest of her life.
On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
Andrea, a Cuban America, who was a student at University of North Carolina (UNC) was sexually assaulted at an on-campus party. She recalls her head being banged up against the wall. Talking with friends, she found out that many other people were raped the same weekend she was. She did not want to talk about it, or admit that it happened. She had nightmares frequently.
On 01-07-2016 at 1715 hours I was dispatched to Poplar Bluff Regional Medical Center in reference to a sexual assault.
I arrived on scene at approximately 02:11 hours. I approached the front door and the victim, Hollie J. McIntosh (F/W, DOB: 07/26/1988) opened the door and let me inside. I inquired with McIntosh what happened. McIntosh stated she was shoved and choked by her boyfriend, James Dean Carvell (M/W, DOB: 01/25/1988). I observed a red mark on McIntosh’s neck. On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. CM traveled to consumer home for the purpose of providing consumer with transportation to her clinic appointment at AOP. CM arrived at consumer home and consumer said, “I am not going to clinic.” Consumer stated she had been in the custody of the Mobile Police Department overnight. CM encouraged consumer to come to her clinic appointment. Consumer said, “I will go because I know you trying to send me to East Pointe.” Consumer said,” You don’t understand that Poo (her son) is my dead husband. Consumer is delusional as evidenced by thinking her son is her husband. CM asked consumer was she ready for her appointment. High ranking executives in US companies play an important role in the generation of policies that promote safe work environments. That means for ALL the employee, including janitors. Executives have the responsibility to create effective mechanics that will allow them to detect irregularities at the work place. By the same token, the US government needs to reinforced laws that punish violence against women. If violent acts against women are closely penalized with severity, then companies will be more incline to make sure all violent acts against women are avoided. The reporting party (RP) disclosed that on 1/9/17 at approximately 5:30PM while working in the office she was approached by administrator Elnolito Llarenas who requested to leave early to go to the gym. While in the office the administrator attempted to kiss the RP who pushed him off. As the administrator was walking towards the door to exit the room he turned around, flick out the lights and grabbed the RP and began forcibly kissing her. While she continued to struggle the administrator was able to unlatch her bra and fondle her breast. He continued to grope he body and pull down her pants in an attempt to touch her vagina. However the RP was able to prevent the administrator from touch her vagina under her clothes by holding on to her pants.
Myself and my partner were called to a 46-year-old woman with deteriorating mental health issues, experiencing paranoid psychosis, increased violent outbursts, she was non-compliant with instructions, had a loss of appetite and had increased her alcohol consumption. The patient was located at her residence in Sippy Downs, Queensland. The residence was disordered with empty alcohol bottles, newspapers, and general mess throughout. The patient had covered all electrical devices with linen, the power points were covered with aluminium foil to prevent external sources from stealing her work and preventing her from breaking “The Code”.
