Case management has been perceived as more than a nursing modality. Its description entails clinical approaches that concentrate on an individual's accountability enhancing the attainment of cost, clinical, and quality outcomes. Case management has been described as the process of coordinating, procuring, outsourcing and negotiating resources and services required by clients. It entails intervening at points where there is a significant variation for individual clients resolving and addressing patterns in variance likely to have adverse impacts on quality cost. The explanation spans that case management is the process of creating systems and opportunities that enhance outcomes. In this regard, I can clearly articulate that case management is the process of service coordination: the case manager is responsible for coordinating services that lead to quality outcomes in congested heart failure patients (Basavanthappa, 2007).
