I took a health assessment test off of a website called, testwell.org. This test asked questions about physical, health, self-care, safety, environment, sexuality, emotional, intellectual, occupational, and spirituality. They grade the test on a 100point scale. 80-100 is excellent, 60-79 is good and anything below a 60 has room for improvement and could be improved in your life. They also add up all the points you got for an overall score. For all of the subjects and also the overall score, I did not do as well as I thought I would do. My overall score was a 680, which barely passes over the “good” score. There were four of the health categories that I did not do so well in. These all did not come as much as a shock to me. Most of these have to do with simply being in college. Most college kids emotional and social wellness are not going to be great due to the stress that everyone gets when in college. Regardless, there are still things that could be done to help get these categories a few more points in this scale system. I have only made about 3 close friends that I actually sit and have conversations with. Although three is better than none, I have not gone to a social event since moving into the dorms. Going out and meeting new and more people with doing something fun could really help with my social health. My emotional health is slightly a lost cause just like most college kids. However, to reduce stress it is best to have a normal and constant sleeping pattern along with good study habits to make sure you are getting work done. People who are sexually active should be tested for HIV’s and STI’s. There is also a few test that women should have done every few years such as PAP smears and mammary test. These things can help you stay safe while being sexually active. Another hard thing for college students is occupational wellness. I do not have a job down here at school so I do not have to worry about balancing a job and school work. Lastly, the categories I did do well on were: physical and safety. Both of these categories I scored over 80 on the scale system. The only question the test really asked about for safety was if I wear a seatbelt. Since I always wear a seatbelt, there isn’t much else I can do to improve that particular action. As for physical, I work out most morning in the gym at school. In order to develop a wellness program there are several things that should be taken into consideration. First, you must develop your own personal perception of what you feel wellness is and how to stay well. Secondly, you must know what the pros and cons are for being fit. Then finally you must set immediate goals to obtain your level of wellness. The main thing you need to keep in mind is that developing a healthy life style early in life is a key factor in preventing diseases and improving one quality of life.
I have to agree my assessment results are congruent with my personality. As an extravert, I am extremely talkative and social. I enjoy interacting with individuals on a daily basis and being able to communicate through speech. It was no surprise when I saw my extraversion score at a high level. I tend to be a very agreeable person. I am always helping others and putting their needs before my own. Sympathy and caring are personality traits learned over the last few years in my life. I am an extremely conscientious person, who is always controlling my surrounding environments. I rely heavily on myself and am determined to achieve my life goals. I figured my neuroticism score would be high, since I am filled with anxiety, depression, and fear. My ability to adjust to change is not the greatest, which affects my emotional stability. The
One of my highest scores was physical wellness. This is because being physically fit is very important to me. Since 6th grade I have always been very involved in sports. Through middle school I played football, cross country, track, and was captain of the wrestling team. Because of this physical fitness is deeply important to me. I avoid anything that can possibly hinder my physical abilities like drugs and smoking. I eat well a majority of the time and am currently on my school’s raider team which has kept me very physically fit.
It can sometimes be too late. I have learned over some years by watching those that mean a lot to me let their lives become of nothing because they do not worry about their physical appearance or the foods that they eat, and even though you can give them all the advice to try and persuade them they are determined to do what they feel and what they want to do.

There are many forms I use to assess an individual’s needs. The first bit of the information comes from Derby City Council, which is called a outcome based support assessment. This is what they use to identify someone’s needs and how much care they require. The information on this document is great for Derby City to use, but I also need to do my own and adapt it so it’s easier for a care worker to understand as they are the ones who will be doing the care. This assessment was conducted after interviewing the Nelson family using Gordon’s functional health pattern. Marjorie Gordon developed a method to be used by nurses in the nursing process to provide a comprehensive nursing assessment. It includes eleven principles for the collection of data and helps the nurse identify two or more wellness nursing diagnoses. It is a systematic and standardized approach to data collection (“functional health” n.d.). In the Sequence category, I scored a 16. I was actually surprised by this score being as high as it was. I'm not a clock watcher at all. I may show up to work 5-10 minutes late but I'm normally working through my lunch hour and after hours without hesitation. Generally I hate reading directions, not because I don't care about the requirements that's being asked of me, but solely I just want to get to the meat and potatoes of the matter. After doing the assessments there were parts of it that came as a surprise to me and there were parts that I was sure of my results before I even answered a question. What came as the biggest surprise for me was the fact that I seem to be pretty average according to the results. There are things that I need to work on and there are things that I excel at. Some things I reside in the middle, and appear to have average scores.
The patient denies shortness of breath or recent chest pain. The patient states he currently takes Coreg and Aspirin daily.
For the 123 Test, my results showed that my openness to experience is low, my conscientiousness is very low, my extraversion is very low, my agreeableness is high, and my natural reactions are very high. The descriptions given to me included down-to-earth, spontaneous, reserved, compassionate, and prone to worry. I think that the test describes me pretty well, however I feel that my score on conscientiousness should have at least been in the mid range. I also think that the test is valid and tests what it is supposed to. I don’t think the test is reliable in the sense that a person can choose whatever choice they want. I also think that sometimes a person may make different choices, especially on questions that were harder for the test taker to answer.

Health assessments are conducted every day in nursing and other health profession.
