Introduction Community health is an essential aspect of wellness of a community’s citizens. In order to ensure that a community is safeguarding the health for its residents, the healthcare professional must recognize that the process of evaluation, assessment, implementations and planning is essential for the community health nurse to assure adequate outcome of indicated interventions. In the community assessment, diabetes was noted to be a major problem in the Irvington community. It is only adequate to identify suitable interventions that can help curtail this ailment amongst the residents of Irvington, New Jersey. As identified during the community assessment of Irvington Township, several aspects of the community and environmental risks relating to the high incidence of diabetes amongst this population were recognized. During the course of this paper, specific interventions and quantifiable goals will be identified. The main goal of this paper is to curtail the prevalence of diabetes in Irvington Township. Brief overview of Diabetes Diabetes is a condition that affects the way the body breaks down glucose for energy and development. The insulin-dependent and noninsulin –dependent diabetes are not the same disorders. Although the origins, short-term effects, and managements for the two types fluctuate, both can cause similar long-term health complications (Monahan & Marek, 2011). According to the

This illness is a result of an imbalance of hormones, insulin, produced in the pancreas. Insulin plays an important role in how the body uses food. Insulin enables the cells in the bloodstream to absorb and use glucose for fuel. If the pancreas produces too little or no insulin or if the insulin doesn’t work properly the person may become diabetic. Therefore, diabetics are not able to properly convert food into fuels needed by the body to function, which can seriously lead to physical consequences.

Health Agencies, such as World Health Organization (WHO) and Center of Disease Control and Prevention (CDC), have developed objectives to tackle diabetes. Some of these objectives include conducting surveillance and obtaining diabetes data to identify trends in the population, spreading awareness about the condition, and developing programs that will enhance diabetes care and ensure the longevity of the patients. Various programs have been developed but while some excel, others fail to benefit the lives of the patient.

For a patient suffering from a chronic form of diabetes mellitus understanding how these mechanisms lead to the condition can be used as preventative measures. Potential consequences as well as the causes and clinical manifestations will ensure a better knowledge on the issue to monitor the condition.
