Home Health Specialty Clinic During July 8th I had a fantastic opportunity to ride along with a nurse with Golden Valley Home Health Agency. This is something I went into with an open mind because I did not know what to expect as far as nursing and client care. I found out that there was a lot of independence as a nurse with not too much oversight as long as you did what was expected and had your paperwork in order and turned in on time. Home health care agencies accept many clients on a referral system. There are 3 types of home care provided and a company may have one or all three of them which include: Home health, In-Home care, Private duty/ Private pay. Each offer different services, consumer needs, agency criteria, and major payer. First, is home health and the major payer is Medicare. Home health offers skilled nursing, physical therapy, occupational therapy, speech therapy, Medical social services, and home health aide for personal care. In-Home care offers homemaker chore services which include cleaning and cooking, personal care assistance which include bathing and grooming, respite care which is caregiver relief, advance personal care which is assistance with activities of daily living when assistance require devices and procedure related to alter body functions, authorized nurse visits for example when a RN fills a medicine box, adult day care, consumer directed which is independent living waiver. The consumer needs are long term usually months to years, cover chronic needs like assistance needed to stay in the home and help with activities of daily living, services offered by physician. Home health care is also a service provided within a health care system. Home health is a wonderful option for patients who are not able to leave the house but need nursing care, physical therapy, and assistance with activities of daily living such as eating, bathing, meal preparation, and some house cleaning. Health care systems have created their own home health care agencies to help with the continuity of care and the ease for case management to setup a patient within their own system are great benefits on both sides of the spectrum.

Researchers conducted an observational study to assess the effect of Patient-Centered Medical Home (PCMH) implementation on patient satisfaction, quality of care, staff burnout, and emergency department use. Information was gathered from 913 Veterans Health Administration (VHA) hospital-based and community-based clinics through the use of several surveys. Composite scores were then arranged into eight core domains of Patient Aligned Care Team (PACT)—the VHA’s version of a PCMH model. Researchers then developed a PACT Implementation Progress Index (Pi2) tool which was used identify the effectiveness of PACT implementation at each participating clinic. Clinics were awarded a scored between-8 and 8. Those with scores between 5 and 8 were said to have effectively implemented their programs. Researchers ultimately found that clinics with higher Pi2 scores had less reported staff burnout, better overall patient satisfaction, improved patient outcomes and lower rates of hospital admissions.

The company has two divisions, home health care and healthcare innovations. The home health care division has two reportable divisions, visiting nurse services (VN) and personal care services (PC). Healthcare innovations also have a reportable division. The VN helps provide skilled medical services in patients’ homes to reduce prolonged stays in hospitals and nursing homes. A large amount of revenue derived from this program (96%) is from Medicare with private insurance and Medicaid making up the rest. PC services are also provided in patients’ homes but theses services are more toward custodial than skilled nature. PC revenues are derived on an hourly basis with Medicaid picking up 84% of the tab. The primary goal of this agency is to providing healthcare services that the health care members are caring, and patience. In addition, to have members that are competent in their areas of expertise. In 2002, this agency became the first agency in Nacogdoches that offers both the home health and the hospice. This agency also recognizes that there needed to be something in between home health and hospice and they later developed the AIM™ (Advanced Illness Management) which combines the easier access to home health with the palliative care of hospice. This agency wants to help the families that they serve stay well even though they are not in the care of the agency.

This organization has different programs that assist aging adult with, both public and private resources, in regards to their long term care needs (Utah Aging and Disability Resources Connection, 2017)
