The two logic models that will be utilized in the community health needs assessment of the city of Atlanta are Precede-Proceed model and MAPP (Mobilizing for Action through Planning and Partnerships).1,2 The major pros of PRECEDE-PROCEED model is that it integrates several strategies to transform an individual’s ecosystem for their well-being.3 This model analyzes the different elements of the individual’s ecosystem such as environment, health, lifestyle, and quality of life when developing the strategies. Community health identifies common needs in the community and provides solutions for modifying behaviors for community members. According to Stanhope and Lancaster (2008, p. 347) the term community health is “the meeting of collective needs by identifying problems and managing behaviors within the community itself and between the community and the larger society.” According to Stanhope and Lancaster (2008) community health involves biological, emotional, and social aspects. The biological aspects are measured in terms of “ morbidity and mortality rates, life expectancy indexes, and risk factor profiles” (Stanhope & Lancaster, 3008, p. 345). The community I decided to research and compose this paper is one from which I live, Kent County Michigan. I gathered data from several different resources such as the Kent County Health Department and Michigan Department of Community Health in order to create a community assessment. The data I collected can be broken down into four different assessment pieces.

The ultimate goal is to create environments in which New Haven residents can take the power they need to transform their health and even their lives. To build community empowerment among New Haven neighborhoods means building advocacy capacities that are focus on addressing social and health issues, mobilizing resources for sustainability within the community, and addressing stigma related to health care services through educational resources. I also hope that by working closely with New Haven neighborhoods, I will gain a greater understanding of how decisions made for individual patients affect the entire community. Similarly, I hope to gain greater insight into the social determinants of health that affect New Haven communities, and to use this information to guide the creation of intervention
It is critical to understand the epidemiology of such diseases that overcome a population. With this skill, it would enable health care managers to incorporate such skills to make proper evidence and population based decisions, especially when health resources are limited. In addition, finance, planning, quality issue assessments, and evidence based public health practice would be considered (Healey & Lesneksi, 2011). This can allow them to not only develop best practices, but to also share them through the health care system (Healey & Lesneksi, 2011).
I am using the Mobilize, Analyze, Assess, Plan, Implement and Tract (MAPIT) intervention tool from Healthy People 2020 (People, 2014). I am creating a care plan for my aggregate, Chronic Obstructive Pulmonary Disorder (COPD) in adults living in Baker County Florida. Baker County ranks second in the state of Florida for COPD death rates (Cox, 2015). Baker County is in a rural area and there are not enough health care providers or services for this community. The majority of emergencies that occur in Baker County, approximately 97% have to go to another county for medical care. Baker County is the only Northeast Florida County the federal government has designated as medically underserved (Cox, 2015).
A major influence on health in a community is the availability of services and opportunities within the neighborhood. Public health can be further influenced by the social environments. Accessibility to health care along with education and employment opportunities are fundamental factors to be considered (Riegelman & Kirkwood, 2015). These elements create both advantages and disadvantages for those living within the neighborhood. If we want to improve public health outcomes, we must consider and evaluate these along with many other contributing factors.

There exist some limitations. According to the authors, the study was on a provincial level, and not so much on the regional, community, and municipals levels. This is seen as an issue, because it contained only social determinants of health that require a multi-sector change at the provincial level. Next, the intervention ONLY looked at initiatives like healthy eating and physical, without identifying social programs in sectors like housing. Last, the intervention was not an entirely successful one, because “it did not document attempt to document whether the searched initiatives achieved desired outcomes related to the social determinants of health; such results require many years to manifest themselves” (p.12). In this paper we will elaborate on three services which public health professionals may use in a variety of ways to perform their job more effectively. Our focus areas include the Community Guide, American Diabetes Association, and American Heart Association. For each area we will include a brief discussion summarizing the areas, their values in the field, relevant theories the area encompasses, and how each might be applied in the profession. While the topics of each area may differ, we hope you will gain a better understand for each and see how much of their goals and approaches overlap.
