I am enclosing the list of the scales for new GWC survey with screenshots and a copy of the programmed survey on Qualtrics. I shared the new survey on Qualtrics with you. I was wondering if you chose all the scales in the 'Identity, Belongingness, and Encouragement’ section (except the postsecondary one). I think it would be better to have only 'Belongingness' and 'Identity' scale from CS Principles Student Pre/Post Survey. If you agree with this or if this was your intention but I misunderstood it, please let me know. I changed two questions (STEM Career Interest ranking question, birth year question) for better accessibility. November, 1999 brought about a release of a report prepared by the prestigious National Academy of Science’s Institute of Medicine (IOM) making medical mistakes and their magnitude of the risks to patients receiving hospital care to common public knowledge. The IOM concluded that between 44,000-98,000 deaths occur annually because of medical errors. Among a general agreement was that system deficiencies were the most important factor in the problem and not incompetent or negligent physicians and other caregivers (Sultz & Young, 2010). An excellent example of a system deficiency that leads to a crisis and sentinel event was the highly publicized overdose of Heparin to Dennis Quaid’s newborn twins in 2007.
Case Scenario 1: It has become necessary to ration a vaccine for a contagious disease. There is only enough vaccine available to cover 75% of the U.S. population. The design included a number of survey questions with varying number of choices. Most of the questions required the surveyed person to decide on scales of 1-3 or 1-4. This will allow those taking the survey to find where they fit, but will not determine if certain parameters are a little above the middle or greatly above the middle like question 4a which states has increased or has decreased – we wont know by how much or how impressive it might be.
On Tuesday 06/27/2017, veteran Mr. Saenz walked very angrily in my office with his wife about 11:00 AM. The survey form was designed with 10 questionnaires about personality, each question is scale from 1 to 7 . Firstly, I answered the questionnaires for myself, then gave out survey forms to 5 co-workers and 5 friends.
In this case study, Ms. A is a 26-year-old white woman who is experiencing low levels of energy and shortness of breath. Ms. A also stated that these symptoms seem to get worsen during her menses. While playing golf in a mountainous area, she had gotten lighted headed. The attending physician had collected the objective data. The objective information is elevated heart rate and respiratory rate while having a normal temperature. Some of Ms. A’s history is have menorrhagia and dysmenorrheal problems for the last 10 to 12 years and takes 1,000 mg of aspirin every 3 to 4 hours during her menstruation. Also during the summer when she plays golf, she takes some aspirin to help avoid stiffness in the joints.
As seen in this case study, TM was listed as a full code and no GOC conversation was had between the physicians and the patient until the day of his discharge to home. This led to an unnecessary swallowing study in which the patient needed to be uncomfortably scoped. Due to the lack of GOC conversation the patient also chose to go home with general nursing services instead of end-of-life (EOL) hospice support. In the inpatient medical oncology floors of a large teaching hospital like this one, most patients are covered by rotating interns and residents that are not comfortable having GOC conversations with patients and leave it up to the primary physician to come and discuss.
JG, a 34-year old Caucasian female presented to the primary care physician office with complaints of losing weight and feelings that "her heart is beating out of her chest". On assessment it was noted that JG had lost twenty pounds in 3 months. JG exhibited tremors in hands, bulging eyes, and pretibial edema. On physical examination there was a goiter, or enlarged thyroid gland noted.
1. I do believe that large pharmaceutical companies as well as doctors prescribing opioid medication should be held partially responsible in cases when patients are over-prescribed drugs. Although they are in no way actually forcing those patients who are over-prescribed these drugs to misuse them and it is ultimately the fault of the individual misusing the medication, they are, however, allowing for the the misuse.In regards to pharmaceutical companies, I think that they should be held responsible for marketing and allowing such drugs available to virtually the mass public opposed to only those that need it in order to increase their profits. After taking the four surveys, I have a better understanding of myself. The surveys help me decide which track I should head toward in the future as well as which part of my personality I should improve.

In addition, the Dietetic Internship Program provides experiences in different areas such as clinical nutrition, community nutrition, research, service management, etc. Experiences from different extents are valuable as it will prepare me to be a well-rounded registered dietitian.
