Bahadori and Fitzpatrick conducted a study regarding the actual level of autonomy of the APRNs in primary care settings. They compared clinical outcomes for patients assigned to either APRNs or physicians and found there was no significant differences in reported health status between patients treated by NPs or by the physicians. Also Bahadori and Fitzpatrick (2009) stated within their report although the APRNs had more restrictions regarding their professional authority and struggle with maximal autonomy they recommend APRNs have more autonomy and decision-making authority to allow them to work as competent professionals, and improve patient care outcomes.

An advanced practice registered nurse (APRN) is an umbrella term consisting of but not limited to roles such as Psychiatric Mental Health Nurse Practitioner (PMHNP); Adult Geriatric Primary Care Nurse Practitioner (AGPCNP); Certified Nurse Midwife (CNM); and Certified Registered Nurse Anesthetist (CRNA). Across the Commonwealth of Massachusetts, it is an undeniable fact that APRNs care for multitudes of patients across the entire human lifespan and across many certain from the nursing homes and home care to hospitals, pain clinics, and substance abuse centers.

The healthcare system in America, is a system that the nation is working hard to improve and perfect. This means the rules and regulations in healthcare continue to change, and often times change rapidly. In the years to come, nurse practitioners (NPs) are predicted to hold a prominent role as healthcare providers. Currently, there are many rules and regulations that prevent NPs from practicing to the full extent of their educations. While the NP certification is standardized nationwide, regulations vary from state to state and many NP are still fighting to be recognized as highly qualified care providers. These current issues make uniting the voices of NPs everywhere important for the progression of the NP career.
A problem solving approach is a process of planning care for a patient (McFerran and Martin, 2003) using ASPIRE, based on APIE (Yura and Walsh, 1988); Assessment, Plan, Implementation and Evaluation. ASPIRE is a nursing process which includes six stages: assess, a systematic nursing diagnosis, plan, implement, recheck and evaluate (Barrett et al, 2012). Using a range of assessment tools, some potential risks can be identified although, these tools should be used as guidance for treatment and not used in the decision making process as to who and who does not receive treatment, (National Institute for Health and Care Excellence (NICE), 2011) but through gaining consent during each process, as it should be before any care or treatment (NMC, 2008).

There is a crucial need for Nurse Practitioners, especially in an area of shortage such as in the state of Missouri. The trending search for NP’s is at an all-time high. Although the need is great, there continues to be restrictions on NP’s License in regards to practicing. Many of the restrictions make it hard to practice at one’s full capability. My dreams of becoming a Nurse Practitioner still gives me hope with fears at time. Continuing to network in order to help with marketing my skills and resume for a position after school appears to be mandatory.
As this author continues his journey toward becoming an APN the patterns of knowing are very important to understand. By implementing these patterns to practice, and using evidence based findings to guide decision making, the author will be better able to care for his patients.
As the young and rapidly-aging population continues to increase, the demands of primary, acute and chronic disease management will also increase. As a result, more health care professionals who provide primary care will be needed to meet these demands. Thus, the emergence of Advanced Practice Registered Nurse (APRN) evolve. APRN is a nurse who has completed a graduate degree and has acquired advanced knowledge and skills. APRNs are grounded with theory, concepts and principles that enable them to assess, diagnose, treat and manage their patients. APRNs can work in conjunction with other health care professionals or independently. APRNs improve access to health care by providing care in the rural and underserved areas. APRNs also reduce the cost to health care (Joel, 2013).
Education is an important aspect of life no matter what age you are at and many seniors continue to want to learn as they get older. For older adults to be able to pursue education there are some obstacles that they may face, one of which is long walks from the parking spaces to the classroom and the cost of parking can deter them when even one class requires you to go to campus multiple times a week (Novak, 2012). A third reason is that they may have memories from when they were in school when they were younger (Novak, 2012). Not all older people go back to school to earn more money, some are focused on helping others with their education and not worried about the money (Novak, 2012). Throughout this paper I will look at some of the major theories of aging and how they coincide with education as an older person, staring with the life span development model.
