During the course of this piece of work, it is my intention to outline and discuss a clinical audit that I undertook into one aspect of care delivery and reflect upon the experience using Driscoll’s model of structured reflection (Driscoll, 2000). It also proposed new systems to enhance patient care, an emphasis upon health and wellbeing rather than illness and the increased devolution of decision making to local organisations. The need for more partnerships and joint working arrangements between Primary Care Trust’s, Local Authorities, independent and voluntary sector organisations, was also emphasised. A new performance framework was also implemented in April of 2005, Standards for Better Health (Department of Health, 2004) which sets out the level of quality all organisations providing NHS care are expected to meet (core standards) or aspire to (development standards) within UK healthcare. The standards to which the document refers are organised within seven domains ranging from safety and governance, to patient focus and public health and are designed to cover the full range and spectrum of healthcare as enshrined in the Health and Social Care (Community Health and Standards) Act 2003. National Service Frameworks and National Institute for Health and Clinical Excellence guidance are integral to this standards based system, whilst The Healthcare Commission has an ongoing and major function to play in the assessment and review of all healthcare organisations.

Reflecting on the situation that had taken place during my second placement working in the community. This will give me the perfect opportunity to develop and utilise my commutation skills in order to maintain the relationships with my patient. In this assignment I will be reflecting on a clinical skills formative assessment, to do this I will be using the driscoll’s model of reflection (2007) This model will be used in the essay to show that I have thought critically, relating the theory to practical session I will also be using this model because this is a recognized framework that will assist me in demonstrating how able I am to think and reflect critically on my assessment.
This essay discusses and reflects upon patient care in the post anaesthetic care unit (PACU) and is linked to my experiences on placement. It discusses how my approach to patient care has been challenged and analyses how evidence based practice can create a change in the way patients are cared for. It reviews the processes of managing the perioperative environment and evaluates the implications for practice when applying a change in healthcare. Wicker and O’Neill (2010) state that “The lack of immediate medical support in the recovery room means that practitioners work in a more autonomous role than any other area of the operating department” (p.379). By reflecting upon my experiences I am able to link practical and theoretical aspects of the operating department practitioner (ODP) job role. This will provide me with a greater understanding of professional practice and it will develop my personal knowledge and self-awareness (Forrest, 2008). Using a model of reflection is important as it provides a framework that can be systematically followed and acts as a guide through the process of reflection. For this essay I have chosen to use the Gibbs’ Reflective Cycle (1988) as it provides a methodical guide to reflection using a series of ordered questions that each lead to the next stage of the cycle (Forrest, 2008).
Dale Gordon has been a patient in the ICU for 6 days after developing complications after open heart surgery. He is an 82-year-old African American who is disoriented to place and time. He lives with his daughter Claudia in her home. Claudia and her two brothers visit Mr. Gordon daily since he has been hospitalized. Mr. Gordon has not been eating well since the surgery and has lost 3 pounds. Mr. Gordon has type 2 diabetes and is on oral antihyperglycemic medication. Before he came to the hospital, Mr. Gordon was able to only ambulate for short distances. He has orders to get up in a chair twice a day. Joan, a student nurse, is caring for Mr. Gordon this morning. She has reviewed his medical record and is now ready to start caring for him.
During my clinical placement, I have done the nursing standard 2 which is engaging in therapeutic and professional relationships. An example of one event in my placement where I have done this standard is when doing interviews in my two-week community placement at the Community Rehabilitation in Armadale Hospital. For my reflection, I will be using the Gibbs’ Reflective Cycle (1988).
Reflection is described as a way of reviewing experiences from practice so that it can be described and analysed and used to change future practice (Bulman and Schutz, 2004).
Gustaffson and Fagerberg point out that reflective practice has relevance for clinical practice as by understanding the contents of nurses’ reflections, it is also possible to understand the advantages of reflective practice and how and when such measures should be used by the nurses for further professional development.
In this assignment I am going to reflect on a clinical situation that has taken place during my placement. I will be using the Gibbs’ reflective cycle which will include the skills of assessment, planning, implementation and evaluation of care and link it to the registered nurse standards for practice – standard 4 , comprehensively conducts and assessments (nursing and Midwifery Board of Australia [NMBA], 2016). The Gibbs’ reflective cycle consists of 6 stages which starts with a description of the situation and what I observed, the second stage is to describe what were my feelings and thoughts were at the time, third stage is an evaluation to explain what worked well and what was good, fourth is an analysis to link the incident to what we have been learning , fifth stage is the conclusion to talk about what else could have been done and the final stage is an action plan to set out next steps for if it was to happen again (Gibbs' Reflective Cycle, 2014).

The ability to become reflective in practice has become a necessary skill for health professionals. This is to ensure that health professionals are continuing with their daily learning and improving their practice. Reflective practice plays a big part in healthcare today and is becoming increasingly noticed.
In recent years, reflection and reflective practice have become well-known term with in the health care arena. They are words that have been debated and discussed with in the health care setting (Tony and Sue 2006). Reflective practice is essential for nurses, as nurses are responsible for providing care to the best of their ability to patients and their families (NMC, 2008). Reid (1993) states reflection is a process of reviewing an experience of practice in order to describe, analyse, evaluate and so inform learning about practice. The aim of this ten minute reflection is to show my experience of how my mentor and I used the ABCDE approach when dealing with a patient with chest pain. The concept of this ten minute reflection is to outline the areas that I can develop on for my future placements. This ten minute reflection will show my emotional state and my knowledge that I applied in this situation. It will outline my learning and development when in my placement area and help me to decide if a better outcome could have been achieved if I performed differently.
