2 Community I currently live in the community I have chosen: Yorktown, Virginia (used interchangeably with York County). York County, Virginia, is situated in the Tidewater region of the state and is part of the Hampton Roads metropolitan area. It is known for its rich historical significance during the American Revolutionary War (YorkCounty.gov, n.d.). York County is a unique mix of rural and suburban living. According to the U.S. Census Bureau, as of 2022, the population of York County is approximately 70k, with a median age of 39.7 and a median household income of $96,144. The largest three ethnic groups in York County are White (Non- Hispanic), Black or African American, and Asian (Non-Hispanic) (Data USA, 2021). With a significant military presence in the area, several residents are active duty, veterans, retirees, or government employees. Regarding healthcare, 96.4% of the population has health coverage (employee plans, Medicaid, military or V.A. plans, and non-group plans). The poverty rate for York County is approximately 4.85% (U.S. Census Bureau, 2022). Transportation Options When exploring transportation options for those with disabilities or in need of non- emergency medical transportation (NEMT) services, I identified two service providers: the Department of Medical Assistance Services (DMAS), which offers services for those with Medicaid, and the Peninsula Agency on Aging, which provides services for those not eligible for Medicaid who lack access to public or private transportation. The Virginia DMAS offers fee-for-service (FFS) NEMT services to and from Medicaid- covered services. Modivcare is the transportation coordination company used to arrange transportation for members. Transportation services include taxis (minivans/sedans) for members 2
3 who don't require assistance, wheelchair vans for members who are unable to transfer from chair to vehicle, van stretchers for members who need to be on a stretcher but don't require medical care, and stretcher for those that need to be on stretch and require medical monitoring (Virginia Medicaid, n.d.). Members must make transportation reservations five business days before their scheduled appointment by calling the reservation line, using the app, or using the Medicare website. Passengers are authorized to bring one additional person to accompany them and must be reported at the time of reservation (Virginia Medicaid, n.d.). DMAS also offers mileage reimbursement for transportation provided by family, friends, or caregivers (Virginia Medicaid, n.d.). Members living near public transportation may request bus tickets/tokens to attend their Medicaid-covered appointments (Virginia Medicaid, n.d.). The Peninsula Agency on Aging (PAA) is a non-profit organization that supports independence and quality of life for residents 60 years and over (PAA, Inc., n.d.). PAA offers transportation for individuals 60 years or older who are not eligible for Medicaid transportation, have no other means of transportation, have no access to public transit, or cannot afford private transportation (PAA, Inc., n.d.). Reservations for transportation services must be made in advance, and appointments must be scheduled M-F between 0900-1430 (PAA, Inc., n.d.). Funding & Policies State and local funding policies for transportation services significantly impact their utilization. Funding availability directly influences the extent to which transportation services can be offered, the frequency of service, and the eligibility criteria for accessing these services (Wright, 2022). Lack of funding can create challenges for both providers and clients. However, increased funding allows for expanding services, which in turn allows access to medical care, 3
