Items 23, 45, and 54 – Richard’s left shoe, Ralph’s shoes, and the crowbar respectively – are
positive (+)
for the presence of human hemoglobin. The hammer, wrecking bar, and baseball bat are
negative (-)
for
the presence of human hemoglobin. Therefore, we can
[include/exclude]
the hammer, wrecking bar, and
baseball bat as the possible murder weapons and must
include
the crowbar.
Items 5 and 19 are both
positive (+)
for human hemoglobin.
Items that were confirmed to have human hemoglobin present are being sent to DNA to try to ascertain
to identity of the blood donor.

List the symptoms pertinent to the case.
Identify appropriate history questions for your patient to discriminate critical characteristics or attributes about the above presenting complaint. Incorporate OLD CARTS (Onset Location, Duration, Characteristics, Aggravating, Relieving, Treatment, Severity).
Delineate 4 differential diagnoses that could support the above symptoms in relation to pertinent answers given the history. Include the name of the disease and the appropriate ICD 10 code and provide a rationale for each choice based on the presenting case.
What is the physiology, pathophysiology, and/or etiology associated with each differential?
What diagnostic tests would you obtain to rule out medical issues that mimic each differential diagnosis?

2Approaching the scene, there is a green two-door GEO Storm in the parking space marked 3C.The vehicle is locked and does not appear to have been tampered with. There are no othernonemergency vehicles in the parking lot. (You are told that the reporting officers had the othervehicles removed from the parking lot.)Walking toward the stairwell, there is a series of red stains in the shape of shoeimpressions on the sidewalk. These shoe impressions appear to contain class characteristics, andthey may be composed of possible blood. There are four shoe prints present, all seem to be fromthe right foot and appear to have been made from the same shoe. The impressions lead from theelevator entry and disappear in to the grass. In the bushes just to the right of the sidewalk nearthe base of the exterior wall of the apartment complex is a large knife.Climbing the stairwell to the third floor, do not notice any further impressions. Uponarrival on the third-floor landing, there are two shoeprint…

Medical Case #1: DehydrationRaphael, in his third-year medical internship, was alone in the hospital emergency room (ER) onenight. It was unusually quiet in the ER, so the resident doctors were getting some much-neededsleep. A patient, Ms. Rose, was brought in showing signs of serious dehydration. Raphael triedto give her water but she vomited. Feeling that he must try something else and not wanting towake the residents, he administered 1 liter (L) of sterile distilled water intravenously (IV).Assume for simplicity that red blood cells (RBC) contain only solutes to which the RBC membraneis impermeable. The osmolarity of the RBC is 300 mOsm/L.

Discuss, using the text, how you believe this case should be decided. Remember to
reference specific ethical principles from the text: "A 13 year old girl, suffering from
toxic shock syndrome, is sent directly from school to a local emergency room.
Treatment is ordered, including fever-reducing medication and a strong course of
intravenous antibiotics. The parents arrive just as the medication and equipment
arrive in the emergency room. They announce they are Christian Scientists and wish
to take their daughter home." The clinical approach to this case requires the
application of the procedure by which medical indications, patient preferences,
projected quality of life and contextual features be applied. How would you decide it
given our readings so far?
