1) Patient is a 3 year old male brought to the ED by his parents. The patient was found
unconscious on the floor in his parents' bedroom in their single dwelling home with an
empty bottle of his mother's oral clindamycin next to him. The emergency room
physician notes that the patient is in a coma and has begun to blink his eyes
spontaneously to his name. Code the emergency room record. (3 codes)
2) The patient is a 5 year old girl who was carried into the emergency department by her
parents due to extreme weakness, abdominal pain, and vomiting. The emergency room
on-call pediatrician examined the patient who found a chip of paint in her mouth. Blood
tests show that she was poisoned by eating lead paint. The patient's father admits that
the house that they just moved into has a garage door that has old lead paint and that
his daughter has been playing out in the driveway with her sidewalk chalk. (5 codes)
3) The patient, a 43 year old man, was found unconscious in his car, which was parked
in his house garage. The garage door was closed and the motor was running. EMS at
the scene also found a suicide note from the patient on the passenger seat. Upon
arrival to the emergency room, the patient suffered cardiac arrest and was pronounced
by the emergency room physician. His final diagnosis was, carbon monoxide poisoning.

her infection for the last two days. The rash became evident the night before last. She
waited, hoping that the rash would go away. She came in today because it got worse,
not better.
O: Ht 5'7" Wt 155 lb. R16. Upon examination, it is suspected that the patient is having
an allergic reaction to the penicillin. Patient is given an injection of antihistamine and
given a new prescription. She is told to immediately discontinue the Augmentin.

urinary retention and was scheduled for a TURP the next day. He was taken to the
operating room. After the initiation of general anesthesia, the patient developed atrial
fibrillation. The procedure was canceled and the atrial fibrillation was treated. It was
determined that the arrhythmia was due to the anesthetic.
